About this file

This is an Information and System Security Requirements appendix document that supplements the contract between Roswell Park Comprehensive Cancer Center and vendors providing services that involve access to Roswell Park data or technology systems. The document establishes comprehensive security obligations for vendors, including those engaged to provide a contract lifecycle management (CLM) system with software, configuration, implementation, project management, training, and support. Vendors must comply with all applicable federal and state laws governing data confidentiality, privacy, security, integrity, and availability, and must implement industry-standard security protocols aligned with frameworks such as NIST Cybersecurity Framework, HITRUST, ISO 27001, and CSA Cloud Controls Matrix. Key security requirements include formal patch management programs, annual penetration testing with attestation provided within thirty days of written request, antivirus and endpoint protection deployment, annual cybersecurity awareness training including phishing exercises, and annual incident response plan testing. Vendors must also designate a qualified security lead and primary cybersecurity contact for Roswell Park.

Vendors are prohibited from offshoring any access, receipt, storage, or transmission of Roswell Park data or technology outside the United States. All Roswell Park data must be encrypted in transit and at rest using NIST-compliant, FIPS 140-3 validated encryption solutions, and vendors must maintain audit logs for minimum one-year retention. For vendors handling cardholder data, PCI DSS compliance with annual attestation is required. Upon contract expiration or termination, vendors must permanently destroy Roswell Park data according to NIST standards or return it in platform-agnostic format, and must immediately revoke all system access within thirty days. Roswell Park retains audit rights to review security programs, request SOC-2 Type II reports or ISO 27001 certifications, and monitor all technology access. Vendors must notify Roswell Park of material security threats within seventy-two hours, and failure to do so permits Roswell Park to suspend access and terminate the agreement without liability. These requirements take precedence over conflicting contract terms, except where PHI-related requirements in a separate Business Associate Agreement apply more restrictively.
