About this opportunity

The South Carolina Department of Public Health is requesting bids for printing of the DPH-1327 Newborn Screening Form under IFB, with bids due September 22, 2026 at 2:30 PM ET and questions due August 28, 2026 at 5:00 PM ET. The central capability filter is FDA registration: the printing facility must be registered with the Federal Food and Drug Administration for medical device production, with proof required at bid submission or within two days of request.

South Carolina resident vendors and vendors using in-state subcontractors are eligible for statutory preferences under Section 11-35-1524 of the South Carolina Code of Laws.

The scope is production of approximately 65,000 multi-layered forms annually in four parts: a 16# CB lab copy, a 14# CFB sender copy, a 33# CF lab copy with attached FDA-approved blood collection filter paper (Whatman 903 or Ahlstrom 226), and a buff ledger protective layer with biohazard symbol and fold-over flap. Forms require multiple ink colors, serial numbering on Layers 1, 2, and 3, extensive bar-coding, and carbonless imaging technology. Dimensions are approximately 9–10 5/8" × 5". Annual packaging requirements are 3,000 forms in packages of 25, 12,000 in packages of 50, and 50,000 in packages of 100. Vendors must comply with the S.C. Government Printing Services Manual (effective January 1, 1998). The contractor is responsible for all typesetting, printing services, quality control for spacing and alignment, and provision of black laser proofs with ten working days for agency review and approval. The agency may request paper samples or non-printed operating samples at no additional cost within ten working days.

The contract term is one year initial with four optional one-year renewal periods, for a possible maximum of five years. Delivery is required no later than ten weeks after purchase order receipt to the Public Health Laboratory at 8231 Parklane Road, Columbia, South Carolina 29223. Pricing must be submitted on a fixed unit-price basis with all setup, shipping, and delivery included. Award will be made to the lowest responsible and responsive bidder. Award notification is scheduled for September 28, 2026. Estimated start date is October 14, 2026. All materials and designs supplied to the vendor remain DPH property and must be returned upon project completion prior to payment.
