About this file

This is a printing specification addendum issued by the South Carolina Department of Public Health for the commercial printing of the DPH 1327 Newborn Screening Form. The contract is structured as a one-year initial term with four optional renewal years. The project requires the production of approximately 65,000 multi-layered forms annually, featuring complex specifications including multiple ink colors, serial numbering on Layers 1, 2, and 3, extensive bar-coding, FDA-approved blood collection filter paper (either Whatman 903 or Ahlstrom 226), carbonless imaging technology, and a protective fold-over flap with biohazard symbol. The form comprises four layers: a 16# CB lab copy, a 14# CFB sender copy, a 33# CF lab copy with attached collection paper, and a buff ledger protective layer measuring approximately 9-10/16" x 5". Annual packaging requirements specify 3,000 forms in packages of 25, 12,000 forms in packages of 50, and 50,000 forms in packages of 100. The selected vendor must provide a black laser proof in a timely manner, with the agency permitted up to 10 working days for review and approval; any agency delays beyond 5 working days grant the vendor equivalent production time extensions.

The successful vendor must be registered with the Federal Food and Drug Administration for medical device production and must submit verification of this registration either with the bid submission or within two days of request by the procurement official. The vendor is responsible for all typesetting and printing services meeting precise quality control standards for spacing, alignment, and assembly. At the agency's option and at no additional cost, the vendor may be required to provide paper samples within 10 working days or a non-printed operating sample to verify form design and functionality. All materials and designs supplied to the vendor remain the property of the South Carolina Department of Public Health and must be returned upon project completion prior to payment. The specification sheet was last revised on July 24, 2026.
