About this file

This is Amendment One to an Invitation for Bid (IFB) issued by the South Carolina Department of Public Health for printing of Newborn Screening Forms (Solicitation #5400030055). The amendment addresses questions and clarifications regarding the form specifications and bidding requirements. Bids must be submitted by September 22, 2026, at 2:30 PM ET, with questions due by August 28, 2026, at 5:00 PM ET. No pre-bid conference or site visit is required. Award notification is scheduled for September 28, 2026. The contract has an initial term of one year with four optional one-year renewal periods, providing a possible maximum contract duration of five years. Delivery is required no later than ten weeks after purchase order receipt to the Public Health Laboratory at 8231 Parklane Road, Columbia, South Carolina 29223.

The amendment clarifies that the forms are multi-layered constructions bound on the right side with perforations on each layer for removal. Bar-coding must be printed on the top and third layers and must encode a 12-digit alphanumeric value. Serial numbering on layers one, two, and three may be in either black or red font, with black being preferred. The state indicated it will work with the selected vendor on the optimal format for the forms. Pricing must be submitted for each line item as outlined in Section VIII of the solicitation document, which specifies annual packaging requirements of 3,000 forms in packages of 25, 12,000 in packages of 50, and 50,000 in packages of 100. South Carolina resident vendors and vendors using in-state subcontractors are eligible for statutory preferences under Section 11-35-1524 of the South Carolina Code of Laws. The printing facility must be registered with the Federal Food and Drug Administration for medical device production, with proof required at bid submission. All pricing must be submitted on a fixed unit-price basis with all setup, shipping, and delivery included.

QUESTIONS FROM OFFERORS - AMENDMENT (JUN 2017)

The solicitation is amended as provided herein. Information or changes resulting from questions will be shown in a question-and-answer format. All questions received has been reprinted below. The “State’s Response” should be read without reference to the questions. The questions are included solely to provide a cross-reference to the potential offeror that submitted the question. Questions do not form a part of the contract; the “State’s Response” does. Any restatement of part or all of an existing provision of the solicitation in an answer does not modify the original provision except as follows: underlined text is added to the original provision. Stricken text is deleted. [02-2A097-1]

The following prospective offeror questions and agency response is hereby incorporated into the solicitation.

Question 1: What type of extensive bar-coding is required? What is the barcode size?

State’s Response: The barcode must be printed on multiple layers of the form – the top and 3rd layers. The barcode encodes 12-digit alphanumeric value.

Question 2: Based on the layer 4 specs, our team is wondering if the forms are formatted as a book construction or multi-layered forms?

State’s Response: The state will work with the selected vendor on the best format for the forms.

The form is multi-layered and bound on the right side of the form. Each layer has a perforation on the right side where it can be removed from the binding.

Question 3: For the numbering it looks like you okay with either Black or Red, is this correct?

State’s Response: The state will accept either black or red colored font for the serial numbering, but black is preferred.

5 | P a g e

Question 4: When we submit our bid, are you wanting one price, or three for the different packaged quantities?

State’s Response: Refer to SECTION VIII BIDDING SCHEDULE / PRICE-BUSINESS PROPOSAL of the Solicitation document and ensure that pricing is provided for each line item.
