QUALITY ASSURANCE SURVEILLANCE PLAN
Furniture and Ancillary Services
1. Purpose. This Quality Assurance Surveillance Plan (QASP) provides a basis for assessing performance quality for this contract.
The Contractor’s performance will be monitored to determine compliance with established performance objectives. This QASP establishes benchmarks that ensure a quantifiable basis for measuring performance and explains the methodology for surveillance, including who will conduct monitoring and what monitoring will take place.
This QASP does not detail how the Contractor accomplishes the work. Rather, this QASP is created with the premise that the Contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance. In addition, this QASP recognizes that unforeseen and uncontrollable situations may occur.
2. Government Roles and Responsibilities. The following personnel shall oversee and coordinate surveillance activities.
Contracting Officer (KO). The KO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and shall safeguard the interests of the United States in the contractual relationship. The KO shall also assure that the Contractor receives impartial, fair, and equitable treatment under this contract. The KO is ultimately responsible for the final determination of the adequacy of the Contractor’s performance.
Contracting Officer’s Representative (COR). The COR is responsible for technical administration of the contract and shall assure proper Government surveillance of the Contractor’s performance. When requested by the KO, the COR shall provide quality assurance documentation to the KO. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. The Contractor shall refer any changes they deem may affect contract price, terms, or conditions to the KO for action.

3. Surveillance. Surveillance of Contractor performance is the method used by the Government to evaluate the Contractor’s compliance with all terms and conditions of the contract. This QASP is designed to limit surveillance to that which is necessary to verify that the Contractor is performing satisfactorily with respect to the Statement of Work. The following surveillance methods will be used in the administration of this QASP, at the discretion of the COR:
· Analysis of Contractor’s monthly reports
· Progress or status meetings (including voice teleconferences)
· Communications through email and telephone
· Delivery/removal of furniture
· Completion of assembly of furniture Surveillance results may be used as the basis for actions against the Contractor. In such cases, the Inspection of Services clause in the contract becomes the basis for the KO’s actions.
4. Ratings. A rating scale will be used to describe whether performance exceeds, meets, or does not meet a given standard or acceptable quality level. The following adjectival ratings will be used:
· Excellent—Performance exceeds contract requirements to the Government’s benefit
· Acceptable—Performance meets contractual requirements
· Unacceptable—Performance does not meet contractual requirements The performance ratings implemented via this QASP may be used as past-performance relevant information for future contract awards.
5. Communication. The COR will conduct informal interim counseling sessions with the Contractor’s Project Manager to identify any active contract performance that is not meeting acceptable quality levels. These sessions will be conducted at least on a quarterly basis in order to provide the Contractor a fair opportunity to improve its performance level.
In the event of unacceptable performance, the Contractor will be notified, in writing, of the Government’s determination of the performance levels for all instances where the Contractor failed to meet acceptable quality levels. The Contractor shall acknowledge, in writing, receipt of this determination and shall prepare and submit a corrective action plan to document how the unacceptable performance will be corrected. The COR will review the plan to determine acceptability. All communication may become part of supporting documentation for contract payment deductions, fixed fee deductions, award fee nonpayment, or other actions deemed necessary by the KO.
