About this file

This is a Performance Requirements Summary (PRS) attachment for a custodial services contract that outlines key performance objectives, standards, thresholds, and incentives/disincentives. The document specifically addresses four main performance areas: (1) Contractor employee identification requirements, mandating company ID badges with photos for facility access; (2) Restroom servicing requirements for maintaining supplies and cleaning dispensers; (3) Detailed restroom and shower area cleaning standards; and (4) Security check-in/check-out procedures for contractor personnel at government facilities.

For all performance objectives, the threshold is set at 100% compliance, with incentives including positive CPARS narratives and full payment for services. Disincentives for non-compliance include Corrective Action Requests, potential negative CPARS narratives, and possible payment reductions. The contract is associated with solicitation W911SA25QA008 for custodial services at multiple Indiana locations (Bloomington and Richmond), with a base period from March 1, 2025, to February 28, 2026, plus four 12-month option years.
