Contractor Past Performance Questionnaire (PPQ)
Questionnaire responses will be considered as part of Source Selection award decision; therefore, it is important the information is as factual, accurate and complete as possible. The completed questionnaire must be returned to the designated Government contracting office no later than the 20 January 2020.
INSTRUCTIONS: The Contractor/Offeror responding to the solicitation completes Step 1, and then forwards the questionnaire to the designated Questionnaire Respondent. The Questionnaire Respondent (identified by the Contractor/Offeror) completes Steps 2 and 3. Questionnaire responses will be in relation to the Contract Information designated by the Contractor/Offeror in Step 1, Item b.
Notice to Questionnaire Respondent: The Product Lead, Automated Information (PL AMIS) is currently conducting a source seleciton. As part of this evaluation, we have requested that Contractor/Offerors provide information about their past performance on same or similar federal, state or local government or commercial contracts as compared to North American Industry Classification System (NAICS) 334111. You have been identified as the point of contact. Your assessment of the Contractor/Offeror’s performance is appreciated and extremely valuable to our evaluation. Please submit the completed questionnaire as directed in Step 3. Questions may be directed to the Government Contract Specialist: Luke Welling; Luke.m.welling.civ@mail.mil, 309-782-3121.
