L.8.1. SCENARIO #1 - While you are providing support, the field offices Subject Matter Expert (SME) for the electric power system seeks your input regarding recent problems associated with the system. Together you are interviewing the systems contractors Cognizant System
Engineer (CSE) for the electric power system. The interview is about recent failures of the electric power system. During the interview, a nearby operation is interrupted when a motor stops operating. One of the operators complains loudly that this happens too often, walks to a nearby panel board, and cycles a circuit breaker. The motor restarts and the operation resumes. After everyone acknowledges the coincidence, the CSE explains that he was not aware of this particular problem and promises to replace the existing circuit breaker with another circuit breaker with a higher rated current. The CSE explains that he has made several such changes in the last few weeks to improve system reliability. The SME asks about the protective device coordination study. The CSE answers that a coordination study was done for the initial design but has not been updated since then. The SME says that National Fire Protection
Association (NFPA) 70B requires that coordination studies be kept up to date, and the CSE responds that NFPA 70B is not mandatory per the systems contract. The interview ends at that point.
L.8.2. SCENARIO #2 - While you are performing a plant "walk down" inspection of an operation involving the manual handling of chemical munitions, you observe the following:
In Bay A, you notice the operators are not complying with the literal requirements of the Standard Operating Procedure (SOP).\~ There does not appear to be anything serious or imminently hazardous with the operators' action; however, they are placing the munitions on the conveyor in a manner in which the spacing between the munitions is further than that specified by the SOP.\~ (The SOP stated the munitions will be spaced 24" apart on the conveyor.)\~ When you ask the operators about this situation, they reply that their supervisor had "Safety" look at the situation, and their supervisor said it was "OK" as long as they did not place the munitions closer together than that specified by the SOP.\~ When you visit the next bay, Bay B, of the munitions processing line, you observe the operators handle explosive energetics from the munitions in a manner that you consider to be excessive force.\~ You believe there is a reasonable possibility that the energetics could initiate from this excessive force resulting in a fire or explosion.\~ When you ask the operators about this situation, they respond that their supervisor had told them to handle the munitions in this manner.\~ There is not a
Government employee in the area; however, the immediate supervisor is in the general area.
L.8.3. SCENARIO #3 - The PEO ACWA updates the schedule basis for its Program Office Estimate annually. Interim updates are conducted once per quarter for the purpose of tracking status of the schedule. The schedule is broken into phases (systemization, operations, and closure). The Systems Contractors Integrated Master Schedule is used as the basis for systemization, a discrete event simulation model as the basis for operations and a model for closure has not yet been developed. As operations begin, actual data will be available through which future assessments of schedule performance can be made. Propose a process to incorporate processing data into the discrete event simulation model or some other process to assess future schedule performance of operations. Discuss a methodology that could be used to develop a closure estimate for the PCAPP and BGCAPP. Identify potential risk areas and how that could be handled through the acquisition risk management process.
L.8.4. SCENARIO #4 - While you are providing support, the field offices Subject Matter Expert (SME) for the heating, ventilation, and air conditioning (HVAC) system seeks your input regarding recent problems associated with the testing and balancing of the MDB HVAC system. In an interview with the systems contractor's HVAC engineer, you learn the system could not maintain the required differential pressure between Category B and C areas. The engineer added that there were no issues achieving the minimum air changes per hour.
