| 3.2.2 | System Safety Analysis | A Hazard Analysis for the process for Assembly of the M8E1 CBPS. The System Safety Analysis should be based on the contractor's existing process and will allow PBA to "Engineer in Safety" into their process. | 29CFR 1910.119(e)(3); MIL-STD-882D Department of Defense Standard Practice on System Safety | The process hazard analysis (System Safety Analysis) shall address: The hazards of the process, the identification of any preivousl incident which had a likely potential for catastrophic consequences in the workplace, engineering and administrative controls appllicable to the hazards and their interrelationships such as appropriate application of detection methods, consequences of failure of engineering or administrative controls, human factors, and a qualitative evaluation of possible safety and health effects of failure of controls on employees in the workplace | Review by Arsenal Hazards Analysis Working Group |
| 3.2.2.1 | Hazard mitigation recommendations | Hazard mitigation recommendations including engineering controls, machine guarding, lifting techniques/apparatuses for assembly, production pre-operation safety checklists, personal protective equipment (PPE) and training materials needed. | 29CFR 1910.119(e)(3); MIL-STD-882D Department of Defense Standard Practice on System Safety | Written list of mitigation recommendations based on the Contractor's produciton experience | Review by Arsenal Hazards Analysis Working Group |
| 3.3.1 | Training | All the training required for PBA employees to completely assemble and test a n M8E1 CBPS unit IAW the Work Instructions. | The training shall consist of instruction on the installation of each subsystem based on the work instructions for this system. It shall also include training on the in process and final Inspection for each operation in the building of a CBPS system | Training for between 7 and 10 Operators that provides clear instructions for safely conducting activities involved in each process iAW the Work Instruction and shall address the following elements: Operating procedures; steps for: initial startup; normal operations; temporatory operations; emergency shutdown procedures; conditions which require emergency shutdown; startup after (normal and emergency ) shutdown; operating limits; consequences of deviation from operating limits and the steps requiered to avoid deviation. Safety and health considerations including: Properties and hazards of chemicals used in the process; and proper PPE and handling | (1) Review of Training Plan by Project Engineer and Chief of Mobile and Powered Equipment Division. (2) All operations covered in the Work Instructions covered in Training. |
| 3.3.1.1 | On-call Technical Assitance | Ensure that issues/problems which surface during the first year of production are handled in an efficient and timely manner. | The Contractor shall make available for discussions/teleconference appropriate engineers and/or technical specialists within forty-eight (48) business hours of an email request by the Government to assist in failure analyses and other technical issues. | Contact by Contractor technical experts within 48 business hours 100% of the time. | Each case will be evaluated by COTR |
| 3.3.3.1 | Standby during FA Production | Ensure that the units produced for the FAT are assembled correctly and that all in-process QC testing has been performed. | The Contractor shall provide two (2) engineers and/or technical specialists on-site during the production and testing of the First Article Quantity of the M8E1 CBPS. | FAT passed, or if FAT not passed, the reason for failure is not due to assembly or in-process testing failures. | (1) FAT Passed. (2) Root Cause Analysis of test failure determines that the failure is not due to failure to assembly or test correctly. |
| 3.3.3.2 | On-call Technical Assitance | Ensure that issues/problems which surface during the first year of production which cannot be handled by a phone call are handled in an efficient and timely manner. | If technical issues cannot be resolved by teleconference within one (1) day, the Contractor shall provide two (2) engineers and/or technical specialists to PBA for one (1) week duration (four (4) workdays, during the life of the contract toprovide technical expertise in failure analysis...with a seventy two (72)-hour notification. | contractor technical experts on-site within 72 business hours of request by PBA 100% of the time. | Each case will be evaluated by COTR |
| 3.4.1 | Work Instructions | A complete set of work instructions which will allow PBA employees to completely assemble an M8E1 CBPS Unit | 29CFR1910.119(f) | Written operating procedures that provide clear instructions for safely conducting activities involved in each process: Work Instructions shall address the following elements: Operating procedures; steps for: initial starteup; normal operations; temporatory operations; emergency shutdown procedures; conditions which require emergency shutdown; startup after (normal and emergency ) shutdown; operating limits; consequences of deviation from operating limits and the steps requiered to avoid deviation. Safety and health considerations including: Properties and hazards of chemicals used in the process; and proper PPE and handling | PBA production employees will use the work instructions provided to assemble an M8E1 CBPS unit. |
| 3.4.2 | Test Protocols and Procedures | A complete set of Protocols and Procedure for testing the M8E1 CBPS. | The Contractor shall provide one (1) electronic copy in in the Contractor’s format, all documents used to verify compliance and/or performance of the CBPS during the assembly/integration of the M8E1 | All documents used to verify compliance and/or performance of the CBPS during the assembly/integration of the M8E1 | Review by Project Engineer, Quality Management Specialist, and Quality Assurance Engineer |
