About this file
This is a Past Performance Questionnaire (PPQ) template issued as Amendment 1 for solicitation W50NH9-25-R-0006 by the Army Contracting Command-TACOM for the G-7 Engineering Technical Assistance (ETA) Support Services contract. The questionnaire is designed to evaluate offerors' past performance on recent and relevant contracts.
The PPQ requires respondents to rate contractors' performance across multiple categories including quality of engineering analysis and technical support, test and evaluation support for intelligence and cyber capabilities, personnel qualifications, management/business relations, schedule adherence, and regulatory compliance. Ratings use a scale from Exceptional to Unsatisfactory. The template includes sections for contractor identification details, performance evaluations with narrative comments, and respondent contact information. Completed questionnaires must be submitted to the specified contracting officer via email, with responses required within 24 hours of the solicitation closing date. This is a draft version issued for a 100% small business set-aside opportunity that requires offerors to have a Top Secret Facility Level Clearance upon proposal submission.
