About this file
This is an instruction document and form template for vendor authorization with ASRC Federal and its subsidiaries, used in their Costpoint accounting system for domestic vendors only.
The form requires vendors to provide taxpayer identification information (EIN, SSN, or ITIN), legal business name, physical and remittance addresses, and contact details. Vendors must identify their entity type (sole proprietor, corporation, partnership, trust, LLC, or other), entity classification for tax purposes, and business size (small or large business, foreign-owned, non-profit, or other). The form includes optional fields for business identifiers including UEI number, CAGE code, and DUNS number. Vendors must also indicate applicable business classifications such as SBA certifications (8(a), HUBZone, WOSB, service-disabled veteran-owned, HBCU, and Alaskan Native Corporations) and disadvantaged business status. ASRC Federal defaults to Net 45 payment terms for large businesses and Net 30 for small businesses unless otherwise specified in the purchase order. The form requires certification under penalty of perjury regarding correct taxpayer identification, backup withholding exemption status, and U.S. person status. A separate modification form is provided for vendors updating existing information such as address changes, contact information modifications, or tax ID number changes, all requiring authorized signature and date certification. Electronic payments are facilitated through Corpay Payment Automation enrollment, though vendors must receive one paper check before enrolling in the electronic payment system.

Vendor Authorization Request - Domestic Instructions PAGE 1
For ASRC Federal and Subsidiary Vendors ONLY System: Costpoint
Instructions For General Instructions refer to the IRS Form W-9 located on the IRS website (www.irs.gov).
ASRC Federal defaults to Net 45 payment terms for large business and Net 30 for small business, unless stated otherwise in the sales contract/purchase order.
ASRC Federal and its Subsidiaries (ASRC) may award procurement to the seller where the costs will be charged to a U.S. government prime or subcontract.
If so, the seller is advised that the U.S. government may impose a penalty against a firm misrepresenting its business size and/or disadvantaged status for the purposes of obtaining procurement that is to be included as part, or all of a goal contained in ASRC’s Subcontracting plan. Eligibility as a small business is based on the regulations issued by the Small Business Administration in CFR 13, Part 121 of the SBA Rules and Regulations and FAR 52.219-1.
Electronic Payments ASRC Federal has partnered with Corpay Payment Automation to facilitate electronic payments to our Vendors. To receive electronic payments, you must enroll with Corpay Payment Automation.
Note: You are required to receive one paper check prior to enrolling in Corpay Payment Automation
