About this file
This is a vendor affidavit document issued by Hillsborough County Public Schools (HCPS) in Florida regarding compliance with human trafficking and labor coercion statutes. The affidavit is required for all nongovernmental contractors executing, renewing, or extending contracts with HCPS in accordance with Section 787.06(13) of the Florida Statutes. The document pertains to the Infrastructure Communications Failure Systems contract opportunity, which seeks experienced vendors to provide supplemental maintenance and repair services for Nortel/Avaya telephone systems and Honeywell Vista Series security panels, including repairs, renovations, upgrades, and all associated labor, materials, supervision, and equipment necessary to complete the work. Vendors must also demonstrate the ability to recognize and report telephone service provider failures and coordinate system repairs with other HCPS contracted vendors.
The affidavit requires an authorized company officer or representative to certify under penalty of perjury that the contractor does not engage in labor coercion practices, including physical force, unlawful restraint or confinement, debt bondage tied to labor, confiscation of immigration or government documents, threats of financial harm, fraud or deceit for recruitment, or provision of controlled substances for exploitation. The document must be signed and dated by the authorized signatory, with the signatory's name and title clearly identified. This affidavit is a mandatory compliance requirement for contract award and represents the contractor's commitment to ethical labor practices throughout the contract term.
