PLEASE NOTE: IN ADDITION TO PRICING PROVIDED ON THE BID SCHEDULE, YOUR RESPONSES ON THIS QUESTIONNAIRE (AND YOUR RESPONSES ON THE PAST PERFORMANCE QUESTIONNAIRE) ARE CRITICAL TO THE SELECTION OFFICIAL WHEN EVALUATING A PROPOSAL FOR AWARD. Please describe in detail how you plan to complete this project while adhering to the statement of work and period of performance and keeping costs in line with the bid schedule pricing provided in your proposal.

Question #2

Please provide a Project Management Plan and describe how you will institute quality workmanship for a project of this nature and magnitude.

Question #3

Please describe in detail your company’s assets and/or technical equipment/facilities that may be utilized for a project of this nature and magnitude.

Question #4

Please explain and list the materials/products your company will be installing per the Statement of Work specification details and as outlined in Appendix A attachment. (IF QUOTING AN “OR EQUAL” PRODUCT, MUST SUBMIT WITH QUOTE ALL SUPPORTING DOCUMENTS/SPECIFICATIONS FOR THOSE ITEMS).

Please describe how you will address safety on the project, in an area open to the public, while maintaining safety to both the public, park staff and contractor’s forces.

Question #6

Please describe your experience with all four of the following areas: wastewater treatment and disposal systems; water main installation; directional boring; and adaptive reuse of existing structures.

Question #7

Please describe your experience working in areas with high visitor use and interfacing with the public. Describe instances where you have restricted access to construction areas and how you have provided safety measures for the visiting public.

Question #8

Please describe your experience working with local regulatory agencies such as utility providers, municipal or county public works departments, and EPA entities.
