Additional Notes/Recommendations
Parties are exploring the option of constructing a new panel to eliminate the leaks, updates will be available upon request. The leak should be monitored for any increase in severity until corrective action is taken. Both relief valves are due for calibration and are to be replaced with calibrated relief valves as part of schedule work to the system. The vessel isolation valve has areas of moderate corrosion. This item should be addressed before the corrosion impacts the function of the valve.
Other
There was a slight offset in the foundation concrete (1.25"-1.5"). It is recommended that this continue to be monitored.
There was not notable change in the offset severity. The relief valves were missing system reference designators. This is a violation of AFSPCMAN 91-710 LOSC Tailored 11.2.1.7.6.7. The vessel had dissimilar metals on the flange head. It could not be determined if the dissimilar metals were properly isolated as they were painted over. It is recommended that this area continued to be monitored for signs of accelerated corrosion. There was a minor leak at the bottom nozzle flange connection. The leak should be monitored for any increase in severity until corrective action is taken. There was no notable increase in severity.
