About this file

This is a Supplemental Bidder Attestation Form required by the Rhode Island Division of Purchases for the Department of Children, Youth and Families (DCYF) Youth Development Support (YDS) Services solicitation. The form serves as a certification document that must be completed and submitted by all bidders as part of their proposal package. Bidders are required to attest that their entity, including parent corporations, subsidiaries, affiliates, and subcontractors, do not have conflicts of interest with any official, officer, or agency involved in the RFP process and have not materially participated in or been consulted regarding the RFP's formation or technical requirements. The attestation must be signed by an authorized representative and submitted with the solicitation number and title. Proposals for YDS Services are due July 30, 2026 at 11:00 a.m. EDT, with the contract period beginning approximately September 1, 2026 for one year with options to renew for up to four additional 12-month periods contingent on vendor performance and fund availability.

The attestation form requires bidders to certify compliance with Rhode Island General Laws 37-2-13.1(b) and Chapter 14 of Title 36, which define conflict of interest standards. The form clarifies that being a current or past state vendor does not automatically constitute a disqualifying conflict of interest, and that formal responses to prior Requests for Information do not preclude future RFP participation. The document provides blank fields for the vendor entity name, authorized signatory information, date, and solicitation number/title to be completed at time of submission. This supplemental attestation is a mandatory compliance requirement for all bidders participating in the DCYF YDS Services procurement.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
