All bids must be submitted on the provided Pricing Pages (Attachment A) with prices per ton, and bidders are required to factor all equipment, materials, delivery, and labor costs into their quoted prices. The contract includes a 90-day bid hold period following the bid opening date, with payment processed through West Virginia's Purchasing Card program or Electronic Funds Transfer. Vendors are subject to liquidated damages of $75.00 per day per ADO if delivery deadlines are exceeded beyond 15 calendar days after the specified delivery date, with exceptions for force majeure events. Vendors must designate a primary contract manager, maintain adequate inventory, provide quarterly reports as requested, and notify WVDOT of any company name changes or acquisitions within 10 days.
