About this file

This document is Attachment 6 - IH Risk Assessment Codes for the federal contract opportunity SP3300-25-Q-0034 Industrial Hygiene and Ergonomic Program Support Services. It provides definitions and guidelines for deriving Risk Assessment Codes (RACs) for health hazards. The RACs range from 1 (Critical) to 5 (Negligible) and indicate the likelihood and severity of potential outcomes such as death, disability, or property damage. The document outlines the procedures for assessing exposure points and medical effects points to determine the Health Hazard Severity Category, as well as assessing duration of exposure and number of exposed personnel to determine the Mishap Probability Category. This information is used to derive the overall RAC for health hazards identified under this contract requirement.

Attachment 6 Risk Assessment Codes

RAC 1: Critical, indicates death, permanent total disability, temporary total disability in excess of three (3) months or permanent partial disability is likely to occur or probably will occur in time. The RAC is “critical;” the project to abate the hazard should start immediately or within five (5) days. Interim corrective measures are acceptable until a permanent corrective measure can be performed.

RAC 2: Serious, indicates permanent partial disability or temporary total disability in excess of three (3) months, or major property damage which results in serious mission impairment in excess of one (1) month, probably will occur in time. The RAC is "serious"; the project to abate the hazard should start as soon as possible (not to exceed fifteen (15) days).

RAC 3: Moderate, indicates permanent partial disability or temporary total disability in excess of 3 months, or major property damage which results in serious mission impairment in excess of 1 month, is possible to occur in time. The RAC is "moderate" and the project to abate the hazard should start quickly (not to exceed thirty (30) days).

RAC 4: Minor, indicates a lost workday mishap or compensable mishap is possible to occur in time. A RAC 4 mishap also indicates that death, permanent total disability, permanent partial disability, temporary total disability in excess of 3 months, or major property damage that results in serious mission impairment in excess of 1 month, are unlikely to occur. The RAC is "minor" and the project to abate the hazard should start reasonably (not to exceed forty-five (45) days).

RAC 5: Negligible, indicates first aid or minor supportive medical treatment injuries are possible to occur in time, or that a violation of a recognized safety and health standard has occurred. A RAC 5 indicates that lost workday mishaps or compensable mishaps are unlikely to occur. The RAC is "negligible.” However, unsafe/unhealthful conditions identified under this RAC must be abated. The project to abate the hazard should be started in a period not to exceed 180 days.

DERIVING RAC’S FOR HEALTH HAZARDS

Step 1: Using the following procedures to assess points, determine the health hazard severity category (HHSC). The HHSC reflects the magnitude of exposure to a physical, chemical, or biological agent and the medical effects of exposure.

(a) Exposure Points Assessed

AER POSSIBLE?
Exposure Conditions

AER = Alternate Exposure Route, such as skin absorption, ingestion.

CT = DoD component threshold that triggers surveillance actions (e.g., microWatts/cm2, dB, parts per million) STD = DoD exposure limit, such as Threshold Limit Value and Permissible Exposure Limit.

(b) Medical Effects Points Assessed

Condition
Points
No medical effect, such as nuisance noise or nuisance odor
0
Temporary reversible illness requiring supportive treatment, such as eye irritation or sore throat
1-2
Temporary reversible illness with a variable but limited period disability, such as metal fume fever
3-4
Permanent, non-sever illness or loss of capacity, such as permanent hearing loss
5-6
Permanent, sever, disabling irreversible illness or death, such as asbestosis and lung cancer
7-8

(c) Determine the HHSC by totaling the points assessed and using the following guide:

Step 2: Using the following guides to assess points, determine the mishap probability category (MPC) for health hazards. The probability of mishap reflects the duration of exposure and the number of exposed personnel.

(a) Duration of Exposure Points Assessed
