Attachment 1- Asbestos Survey Report

Building Number: DLASJ Tracy, Warehouse 17, “Bubble”, Main Office (North End), Break Room (North End), Class Room, and the South Office

Date of Evaluation: 13 December 2016

Subject: Suspect Asbestos Containing Material (SACM) Laboratory Analytical Results

Background

On 13 December 2016, bulk samples were collected from suspect asbestos containing materials (SACM) inside the DLASJ Tracy, Warehouse 17, “Bubble”, main office (north end), break room (north end), class room, and the south office. Robert Miller, a Division of Occupational Safety and Health Certified Asbestos Consultant for the State of

California, CAC # 95-1821, collected the bulk samples. The bulk samples were submitted to Micro Analytical Laboratories, Inc., Emeryville, California for analysis. The AHERA-recommended analytical procedure polarized light microscopy (EPA Method/600/R-

93/116) was used for the laboratory analysis of the bulk samples.

Main Office (North End)

This room will include replacement of the 720 square feet of floor materials that include carpet tiles and significantly damaged asbestos containing 9-inch by 9-inch green floor tiles and associated asbestos containing black adhesive. The 30 linear feet of covebase adhesive should also be removed as part of the asbestos abatement.

Break Room (North)

Bulk Sample: 1213-17-01 and 1213-17-02: 12-inch by 12-inch floor tiles and associated adhesive. The adhesive contains 5% Chrysotile Adhesive, therefore both the adhesive and the floor tiles should be considered asbestos containing.

Bulk Sample 1213-17-03: Drywall and associated taping compound. The drywall system contains 3% Chrysotile asbestos.

Bulk Sample 1213-17-04: Cove base Adhesive. No asbestos was detected.

Class Room

The 1991 VERSAR report identified the drywall system throughout the Warehouse 17

“Bubble” as asbestos containing materials.

South Office

The VERSAR Report identified the drywall system throughout the Warehouse 17

“Bubble” as asbestos containing materials.

This room includes recently installed carpet tiles on a clean concrete floor.

Discussion and Recommendations

If any of the asbestos containing materials identified in this report are disturbed during the renovation of the DLASJ, Warehouse 17, “Bubble”, main office (north end), break room (north end), class room, and the south office the asbestos containing materials must be removed as OSHA Class II Asbestos Abatement Operations by a California Licensed

(C-22) Asbestos Abatement Contractor.
