About this file

This document is a Seller Checklist for Small Order Purchases issued by the City of Philadelphia Procurement Department through the PHLContracts platform. The checklist provides step-by-step guidance for vendors submitting quotes for the OFM WIZARD Performo Software-Upgrade Annual Hosting micro purchase in Pennsylvania. This is a sole source procurement that will be awarded as a whole or by line item, whichever serves the City's best interest, to the lowest responsible bidder. Sellers must quote all items to be eligible for award and should not deliver services or products until receiving formal notification from the department.

The checklist outlines thirteen sequential steps vendors must complete to submit a compliant quote, beginning with ensuring proper seller role assignment and proceeding through item pricing, mandatory question responses, terms and conditions acceptance, and required attachment submissions. Critical requirements include providing a Unit Cost for each listed item, downloading and saving all bid attachments, and submitting an executed Consent & Authorization form—without which no quote will be considered. Vendors must also answer all mandatory questions, accept the bid's terms and conditions, and have the opportunity to submit an optional LGBTQ Applicant Opportunity Data Form. The Local Business Entity (LBE) certification question must be answered despite the LBE Program not applying to small order purchases. All validation warnings must be resolved before final submission through the PHLContracts.phila.gov platform.
