About this file

This is a Proposer Checklist for a Request for Proposal (RFP) issued by the State of Oregon, Department of Consumer and Business Services, Workers' Compensation Division for Supplemental Disability Benefits (SDB) Processing Services. The RFP seeks a single contractor to provide comprehensive processing services for disability benefits claims, including claims processing, benefit calculation, reporting, and compliance with Oregon workers' compensation regulations. Proposals must be received by 2:00 pm (PT) on August 15, 2025, unless extended via Bid Amendment. The initial contract term is anticipated to be 5 years with options to renew up to a cumulative maximum of 10 years. Proposers must demonstrate significant technical infrastructure, claims management capabilities, and robust understanding of disability benefits administration, with detailed information required about key personnel, technological capabilities, and historical performance in similar disability benefits processing environments.

Proposers must submit one original copy of their proposal following specified formatting requirements (Adobe Acrobat, Microsoft Word, or Microsoft Excel) organized into five submission groups: Proposal, Wage Calculation Assessment, Price Proposal, Attachments Group, and a Redacted Proposal if applicable. Required submission items include an Executive Summary, Administrative Proposal, Key Persons Resume and Exhibit, Technical Proposal, Wage Calculation Assessment, Price Proposal, and multiple certified attachments including Offeror Information and Certification Sheet, References, Certified Disadvantaged Business Outreach Plan, and Responsibility Inquiry. The RFP includes specific requirements for small and disadvantaged businesses with an attached COBID Outreach Plan. Upon award, the successful proposer must submit a Certificate of Insurance, Taxpayer Identification Number (W-9), Oregon Business Registry documentation, and Pay Equity Certification. The checklist emphasizes that prospective proposers are solely responsible for reviewing and understanding all RFP requirements and that the checklist is provided only as a courtesy.
