About this file
This document is a supporting contract form for Mohave Educational Services Cooperative's Request for Proposal 26D-0305 for bread and baked goods procurement serving approximately 480 public agencies throughout Arizona. The RFP solicits proposals for a range of bread and baked products including hamburger buns, hot dog buns, whole wheat sandwich bread, sourdough, French rolls, and Texas toast, with all bread products required to be 51 percent whole grain rich and processed in the United States. Products must comply with the Healthy, Hunger-Free Kids Act of 2010 and Arizona Revised Statutes 15-242.01 regarding ultra-processed foods, with complete nutritional data sheets required for all offerings. Vendors must deliver products weekly between 6:00 a.m. and 2:00 p.m., Monday through Friday, in manufacturer's original undamaged packaging with a minimum shelf life of seven days from delivery date. The pre-proposal conference was scheduled for February 9, 2026, at 10:00 a.m. Arizona time, with a question submission deadline of February 24, 2026, and proposal response deadline of March 5, 2026, at 3:00 p.m. The contract will be awarded for an initial one-year term with four potential one-year extensions.
The supporting contract documents require vendors to provide comprehensive firm information including headquarters and Arizona branch office contact details, purchase order contact information, payment remittance addresses, Arizona Transaction Privilege Tax License numbers, and designated contacts for Mohave in six functional areas: member services, contract administration, accounting, open orders and unresolved invoices, audit requests, and escalation matters. All pricing must be submitted in an unlocked Excel format with specific prices for all products; pricing listed as "call for quote" is not acceptable, and all prices must include Mohave's 1% administration fee. Vendors must provide evidence of current insurance coverage with a minimum comprehensive general liability aggregate of $2,000,000, current certificate of insurance, IRS W-9, and financial information. The solicitation permits any qualified vendor meeting specified requirements to respond with no identified incumbent vendors or set-aside designations, and all offerors must certify compliance with federal requirements including the Byrd Anti-Lobbying Amendment, Executive Order 11246, the Clean Air Act and Clean Water Act, the Buy American provision, E-Verify employment verification, and civil rights compliance under USDA regulations.
