About this file

This document is a Contract Amendment for Contract CTR040045, issued by the Arizona Department of Administration (ADOA) State Procurement Office, involving Gemalto Cogent, Inc. as the contractor. The amendment is specifically for Biometric Products and Services, with both parties located in Arizona (Gemalto Cogent in Pasadena, CA, and ADOA in Phoenix, AZ).

The amendment focuses on IVP (Identity Verification Process) Renewals, establishing a predefined fee structure for renewal transactions without additional charges. The renewal volume is estimated at 8,500 transactions annually, with two account types: Paid Employee and Volunteer. For Paid Employees, the renewal fee is $67.00 plus a $8.25 transaction fee, totaling $75.25, while Volunteers have a $65.00 renewal fee and $8.25 transaction fee, totaling $73.25. Gemalto Cogent will process these renewal transactions without a Charge Order for Labor or additional Transaction Fee, maintaining the same fee structure as the original background check application.
