2.1 General Requirements
1. Which Critical Access Hospitals or rural healthcare systems have you worked with directly, and what was the scope and duration of those engagements?
Currently, Ovation Healthcare has 146 critical access hospitals in 37 states as active clients. In total, Ovation has more than 500 active clients across its lines of business. The client base includes hospitals who have been clients since the 1970’s, and each year the company adds new critical access hospitals to its portfolio. These long-standing relationships demonstrate our deep understanding of the unique operational and financial challenges facing rural healthcare providers, from limited resources to complex regulatory requirements that demand specialized expertise.
Our engagement approach delivers comprehensive solutions across all our service lines -revenue cycle management, spend management, leadership advisory, and technology services.
The scope of our rural healthcare work spans end-to-end revenue cycle optimization that accelerates cash flow and reduces collection costs, strategic spend management that identifies operational efficiencies across organizational expenditures, and technology services that strengthen technological infrastructures while protecting critical systems. Our leadership advisory services specifically address the board-level financial and operational challenges that threaten rural hospital viability. With Ovation, rural healthcare systems gain a strategic partner who understands that sustainable success requires solutions tailored to community-specific needs, enabling them to reinvest administrative time into strategic growth initiatives that strengthen care access for the communities they serve.
The duration of engagements varies based on the services provided. The shortest engagements generally last 30 to 60 days, and some relationships have been in place for 30 years or more.
As client lists are confidential and proprietary information, Ovation will be pleased to review a list of current critical access hospital clients by way of a video conference with representatives from the State Department of Health.
2. How does your team stay current on CAH-specific Medicare and Medicaid regulations (cost-based reimbursement, swing beds, 96-hour rule, etc.), and can you point to recent examples where that knowledge shaped your recommendations?
Ovation’s regulatory expertise stems from our team of experienced rural healthcare leaders who have worked directly inside Critical Access Hospitals, giving us unparalleled insight into the operational realities of CAH compliance. We maintain current knowledge through a comprehensive approach that includes monthly coding forums, quarterly compliance packages, and continuous engagement with industry networks - ensuring our recommendations reflect the latest regulatory changes and real-world implementation challenges.
Our hands-on CAH experience translates into immediate, actionable solutions for clients navigating complex regulations like cost-based reimbursement, swing bed management, and 96-hour rule compliance. For example, our Chargemaster Integrity Review process has helped CAH clients optimize their Method II billing strategies while maintaining strict compliance with cost-based reimbursement requirements. We’ve also guided hospitals through swing bed acuity
OvationHC.com Page 3 management using our templates, validation tools, and governance frameworks - resources built from actual CAH operations rather than theoretical knowledge. Our structured audit process consistently identifies both compliance gaps and revenue opportunities, with clients achieving measurable improvements in reimbursement capture while reducing regulatory risk.
3. Can you share two or three case studies where your work measurably improved operational metrics or financial performance at a healthcare institution — ideally with before/after numbers?
Ovation’s track record demonstrates measurable financial transformation across diverse healthcare settings. At a 130-bed mountain state hospital, our revenue cycle management services delivered a 37.5% net collection rate increase in the first year alone, while a 500-bed southeast hospital achieved a 46% increase in self-pay cash collections. These results stem from our comprehensive approach that combines staff training, workflow optimization, and proven methodologies that consistently reduce accounts receivable days by 10-20 days within 6-12 months.
Our partnership with Cody Regional Health exemplifies sustained operational excellence over thirty years. Across our client portfolio, some institutions typically experience a 25% reduction in net days in accounts receivable, 12% reduction in supply spend, and up to 17% year-over-year revenue growth. At Lawrence County Memorial Hospital, our revenue cycle management implementation reenergized staff while dramatically improving collections performance through targeted training and new methodologies.
4. Walk us through your assessment and discovery process: how do you tailor strategy to a specific CAH’s circumstances rather than applying a templated approach?
Ovation follows a standardized format to complete assessment and discovery when engaging with a new client. However, upon engagement with a particular client the assessment and discovery work is tailored based on the needs and requests of the client along with the scope of work proposed. For Mississippi, the phases of work will include:
Phase 1: Data collection and validation.
Phase 2: Financial and operational risk scoring.
Phase 3: Identification of vulnerable hospitals.
Phase 4: Selection of at least 12 facilities for enhanced assistance.
Phase 5: Development of hospital-specific action plans.
Assessment criteria will include operating margin, days cash on hand, debt burden, payer mix, revenue cycle performance, workforce stability, service line viability, market demographics, outmigration trends, and quality indicators.
5. What training materials and workshop formats do you currently have ready to deliver, and what’s your typical approach for facilitating sessions with hospital staff (clinical, administrative, finance)?
Ovation’s educational approach is designed to be practical, interactive, and implementation-focused with an emphasis on helping participants apply concepts directly within their organizations. Each year, Ovation’s Learning Institute delivers more than 70 educational opportunities to 3,500 hospital leaders, staff, and governing boards through live webinars, on-
OvationHC.com Page 4 demand education, in person events, curated networking opportunities, and leadership conferences.
Ovation has experience facilitating:
• CFO roundtables,
• revenue cycle education sessions,
• governance education programs,
• operational improvement training,
• and strategic planning retreats.
Training materials are customized to the audience and may include:
• financial benchmarking tools,
• operational dashboards,
• workflow improvement frameworks,
• reimbursement education,
• case studies,
• implementation checklists,
• and best practice guidance.
Ovation’s team has the operational depth and staffing resources necessary to conduct both virtual and in-person educational sessions statewide, including collaborative cohort-based learning models designed specifically for rural hospitals.
2.2 Financial Analysis
1. What does your comprehensive financial assessment include — what data do you pull, what frameworks do you apply, and what’s the typical deliverable?
Ovation’s comprehensive financial assessment draws from multiple critical data streams across the hospital. The financial assessment includes a comprehensive review of the balance sheet, income statements, revenue cycle reports, volume statistics, local market data, regional and national comparative databases. The assessment provides a complete picture of the hospital’s financial position and opportunities.
Our methodology combines proven frameworks across revenue cycle optimization and spend management with strategic market analysis tailored to your specific service lines. We apply end-to-end revenue cycle evaluation techniques that have delivered 25% reductions in net days in accounts receivable and over 20% cash acceleration improvements for our clients.
Simultaneously, our spend management framework systematically identifies cost savings opportunities, achieving an average of 12% supply spend reduction per facility. This dual approach ensures we address both revenue enhancement and cost optimization simultaneously rather than treating them as separate initiatives.
Your deliverable includes a comprehensive market assessment report featuring demographics analysis; market projections for acute, outpatient, and physician services; and detailed recommendations for financial performance improvement. Following delivery, we conduct a detailed review session to discuss findings, answer questions, and refine recommendations based on the hospital’s strategic priorities.
2. How do you approach cost structure and revenue cycle reviews specifically for
OvationHC.com Page 5
CAHs given their cost-based reimbursement model? Any tooling or methodology you’d highlight?
Ovation recognizes that Critical Access Hospitals face unique financial challenges under the cost-based reimbursement model. Our specialized approach begins with a comprehensive Chargemaster (CDM) evaluation that ensures technical integrity and regulatory alignment, followed by detailed cost structure analysis that maximizes allowable cost recovery while maintaining strict compliance with Medicare cost reporting requirements.
Our proven methodology combines a 360 Claims Audit with real-time dashboard technology to provide complete visibility into your revenue cycle performance. We conduct structured reviews of all CDM entries, validate CPT and HCPCS codes, and ensure revenue code alignment while addressing CAH-specific complexities like Method II Billing and 340B program integration. Our technology solutions seamlessly integrate with existing EHR/PM systems, providing AI-driven workflow automation and predictive analytics that reduce manual intervention and enhance accuracy. This integrated approach has helped rural hospitals achieve measurable improvements in clean claim rates and net collection performance.
3. Which state and national benchmarking datasets do you draw from (e.g., Flex Monitoring Team, CAHMPAS, RUPRI, MedPAC), and how do you present benchmarks like cost per patient or revenue per service line?
Ovation takes a comprehensive approach to benchmarking by aggregating performance data from numerous external sources to develop localized perspectives on provider needs across identified service areas. Sources include:
• Flex Monitoring Team CAH Financial Indicators
• CAHMPAS
• CMS Cost Reports (HCRIS)
• MedPAC datasets
• AHA Annual Survey data
• HealthTrust benchmarking data
• State hospital association comparative datasets
• Proprietary Ovation client benchmarking database
4. Describe your approach to optimizing payer mix and reimbursement strategy for rural hospitals — any recent wins worth mentioning?
Ovation approaches optimizing payer mix and reimbursement strategy by focusing on three core components: a 360° Claims Integrity Audit that identifies documentation gaps and reimbursement variances, our Payer Contracting Optimization Program that enhances negotiation outcomes, and comprehensive chargemaster integrity reviews that ensure defensible pricing strategies. Recent engagements have consistently delivered financial improvements that offset engagement costs while enhancing ongoing reimbursement performance. We provide practical tools including our Pricing Infrastructure Toolkit with templates and validation resources, plus ongoing support through our Revenue Cycle Training Academy with KPI reporting and billing compliance programs.
A recent success story includes analysis using payer transparency data to demonstrate conclusively that one hospital system in a market received 30% to 50% higher reimbursement from Blue Cross/Blue Shield vs our hospital in the same market. This data led to productive
OvationHC.com Page 6 contract negotiations on behalf of the Ovation client.
2.3 Sustainability Planning
1. How do you structure an individualized sustainability plan, and how do you weight financial stability vs. operational enhancement vs. community engagement components?
Each participating hospital will receive a sustainability framework addressing financial performance, workforce challenges, operational efficiency, service-line strategy, community partnerships, telehealth opportunities, technology optimization, and funding opportunities. Plans will include short-term, medium-term, and long-term implementation priorities.
2. What funding strategies have you helped CAHs pursue — HRSA grants, USDA Community Facilities, state rural health programs, RHTP, foundation funding — and what’s your success rate?
Ovation works closely with its leadership and advisory client base to analyze and pursue outside funding opportunities. In particular, the company closely follows state driven RHTP funding opportunities, advises our clients, and assists them in grant request preparation. Ovation actively assists clients and prospects in identifying alternative financing options along with grant opportunities.
3. How do you incorporate telehealth, EHR optimization, and data analytics into sustainability plans without overextending limited CAH IT resources?
Critical Access Hospitals face the complex challenge of modernizing their technology infrastructure while operating with severely constrained IT resources and budgets. Ovation understands that CAHs cannot afford to overextend themselves pursuing multiple technology initiatives simultaneously, yet they must advance telehealth, EHR optimization, and data analytics to remain viable and serve their rural communities effectively.
Ovation’s structured, phased implementation approach eliminates the resource strain that typically overwhelms CAH IT departments. Rather than forcing your team to build processes from scratch, we provide ready-made frameworks that integrate seamlessly with your existing EHR/PM systems. Our prioritized workplan guides sequencing and accountability, ensuring you tackle the highest-impact initiatives first based on your specific resource constraints and strategic goals. This methodology has enabled similar CAHs to achieve measurable ROI within 6-12 months while maintaining operational stability.
Our approach leverages managed services and external expertise to provide the specialized knowledge your team needs without the overhead of additional full-time staff. We focus on solutions that emphasize automation and AI-driven workflow efficiency, delivering real-time dashboards and KPI tracking that provide immediate insights without requiring extensive IT infrastructure. Through strategic alignment with your mission and deep understanding of rural healthcare challenges, Ovation ensures your technology investments enhance both financial sustainability and clinical outcomes. Your CAH will be set up to achieve the digital transformation necessary for long-term success while preserving the resources needed to continue serving your community’s healthcare needs.
4. Describe your approach to facilitating community stakeholder partnerships. Who
OvationHC.com Page 7 do you typically convene, and what does a “support network” look like in practice?
Ovation’s approach to facilitating community stakeholder partnerships begins with a client hospital’s market assessment and strategic planning process. The market assessment identifies key stakeholders, while hospital leadership and the board provide input to ensure a comprehensive stakeholder profile. Stakeholder interviews are a critical component of the strategic planning process, providing insights that help shape the hospital's strategic direction.
The strategic plan serves as the foundation for annual business plans and is reviewed and revised on a three-year cycle. The hospital boards, medical staff, community health providers, governmental leaders, business leaders, patients, and lay community members generally comprise the stakeholder group used in strategic planning. Ovation guides hospital and board leaders to participate in community stakeholder service, relationship building, and mutual support.

3.1 Quality Assurance
1. Which industry standards and healthcare compliance frameworks does your work adhere to (e.g., CMS Conditions of Participation, TJC, state licensure, HIPAA), and how do you document compliance?
Ovation operates across 47 states serving over 500 healthcare clients, requiring comprehensive adherence to multiple regulatory frameworks including CMS Conditions of Participation, The Joint Commission standards, state licensure requirements, and HIPAA compliance. Our shared services model inherently supports regulatory compliance by strengthening infrastructure and protecting critical systems. Each Ovation service line hosts monthly meetings with clients to review compliance and results of operations. For revenue cycle clients, we measure accuracy and compliance through the process of charge capture, claims filing, and revenue collection.
For spend management clients, we review purchase contract compliance and identify new savings opportunities. Our IT management service provides thorough audit trails through all systems that reduce vulnerability and document transactions. Finally, for our leadership advisory clients, we hold monthly operating reviews to discuss hospital operations across the board.
2. What does your QA plan look like for an engagement like this? How do you measure progress and effectiveness of changes and training programs?
Across all service lines, Ovation’s QA plan for healthcare engagements centers on ensuring sustainable operational improvements. Monthly reviews of KPI’s and ad hoc reviews, as needed, ensure that the data points routinely monitored are acted upon and receive follow up to ensure sustainability. Ovation’s performance optimization team is available to serve as project managers for larger implementation plans, keeping all parties involved on track to complete initiatives and monitor their ongoing impact.
1. How do you systematically collect, integrate, and close the loop on CAH staff feedback during an engagement?
Ovation collects staff feedback through stakeholder interviews, leadership meetings, department-level discussions, and routine project touchpoints. Feedback is integrated into the assessment, recommendation development, and implementation planning processes to ensure proposed changes align with operational realities and staff workflows. Throughout the engagement, Ovation works closely with hospital leadership and frontline teams to communicate progress, address concerns, and refine initiatives based on ongoing input. This continuous feedback loop helps promote staff engagement, improve adoption of recommended changes, and support long-term sustainability.
4.1 Reporting and Documentation
1. What does a typical monthly progress report from your team include, and can you share a redacted sample?
Ovation’s monthly progress reports provide comprehensive visibility into the hospital’s operations through structured, actionable insights that drive measurable improvement. Multiple sample reports are attached to this submission.
2. How do you manage deliverable schedules across multiple workstreams (training materials, assessments, sustainability plans), and what tooling do you use to keep
OvationHC.com Page 9 clients informed of status?
For this project, Ovation will provide a dedicated Executive Strategic Advisor to both the state and the hospitals served. Ovation’s performance optimization team will assist the Executive Advisor in establishing a detailed project work plan and conduct regular meetings to ensure initiatives remain on track toward completion.
3. What’s the structure of your final comprehensive report, and how do you ensure findings and recommendations translate into actions the CAH can execute post-engagement?
Ovation’s final comprehensive assessment report will deliver a standardized assessment of the 28 critical access hospitals across all dimensions of performance and a recommended prioritization of which hospitals to be served more in-depth. Additionally, the report will deliver a plan of implementation for the other elements of the engagement over time.
Engagement execution is supported through Ovation’s performance optimization team. We provide structured workplans with clear accountability measures and follow-through protocols, backed by our proven track record of client relationships spanning over 30 years. Our approach includes regular review sessions to discuss implementation progress and necessary refinements, ensuring that strategic initiatives translate into measurable outcomes. This collaborative framework positions the hospital for sustainable improvement while maintaining the flexibility to adapt solutions as your community’s needs evolve.

4. What billing and coding curriculum do you offer for Critical Access Hospitals, and what is your delivery approach for ensuring course effectiveness across multiple sessions?
Billing and coding education will be delivered through a structured curriculum addressing CAH reimbursement, documentation improvement, coding accuracy, denial prevention, compliance requirements, and revenue integrity. Training will include baseline assessments, live instruction, supporting materials, and follow-up performance measurements.
5. What is your chargemaster review methodology and process for identifying and correcting errors in procedures, codes, and charges at Critical Access Hospitals?
The chargemaster review methodology is designed to help Critical Access Hospitals identify pricing inconsistencies, revenue code errors, and opportunities to improve chargemaster accuracy and compliance.
The review begins with an electronic analysis of the hospital's chargemaster using specialized revenue integrity software. This analysis identifies potential issues including revenue code discrepancies, revenue code-to-CPT mismatches, pricing inconsistencies, invalid code combinations, and other charge capture concerns. Following the software review, identified exceptions are evaluated and prioritized based on compliance risk, reimbursement impact, and operational significance. The review focuses on chargemaster accuracy, pricing structure, revenue code assignment, and opportunities to improve consistency across services.

Upon completion, the hospital receives a summary of findings and recommendations, including identified revenue code corrections, pricing considerations, and opportunities to strengthen chargemaster maintenance processes. Findings are reviewed with hospital leadership to support implementation and ongoing chargemaster integrity.
This approach provides hospitals with a practical and efficient review process that helps improve chargemaster accuracy, support compliant billing practices, and identify potential revenue integrity opportunities.
6. What is your approach to implementing changes and delivering services using a cohort-based model with multiple Critical Access Hospitals working together?
Ovation understands that Critical Access Hospitals face the dual challenge of maintaining operational independence while achieving the efficiencies that come from collaborative partnerships. Our cohort-based implementation approach addresses this by providing structured frameworks that enable shared learning and resource optimization without compromising each facility’s unique community focus.
Our methodology centers on a comprehensive toolkit built specifically for CAH environments, including practical templates, validation tools, governance frameworks, and prioritized workplans that guide sequencing and accountability across your cohort. This eliminates the need to build processes from scratch while ensuring each hospital can customize solutions to their local needs. We begin with detailed data intake and workflow validation across all facilities, followed by department interviews and comprehensive reviews that inform leadership-ready insights and execution tools tailored to each hospital’s operational context.
7. What is your facility-level outcome tracking methodology for establishing baselines, setting goals, and measuring outcomes throughout the engagement with each Critical Access Hospital?
Ovation’s facility-level outcome tracking methodology for Critical Access Hospitals begins with establishing comprehensive baselines through detailed market assessments that analyze demographics, insurance coverage, community health indicators, and five-year projections by service line. We leverage demand rates aggregated from numerous external sources to develop localized perspectives on provider needs, ensuring benchmarks are relevant to each community’s unique circumstances. Our collaborative approach starts with project kick-off meetings where we align with your leadership team on specific objectives, governance structures, and data requirements, creating a foundation for measurable success.
Our tracking methodology focuses on proven financial and operational KPIs that directly impact Critical Access Hospital sustainability. These outcomes are supported by our comprehensive toolkit featuring practical templates, validation tools, governance frameworks, and implementation guides that streamline progress monitoring while reducing your internal workload. Our prioritized workplans guide sequencing and accountability, ensuring sustainable improvement rather than just delivering reports.
Data accuracy and reliability are maintained through our technology services that strengthen infrastructure and protect critical systems, while our revenue cycle management prevents billing errors at the front end. We communicate results through structured PowerPoint and PDF reports, conducting collaborative review sessions with stakeholders to discuss findings, answer

8. What is your stakeholder communication management protocol for ensuring all required parties are included in project communications, including emails, conference calls, and video calls?
Ovation employs a comprehensive stakeholder communication management protocol that begins with a structured project kick-off meeting to establish governance, identify all required parties, and set communication expectations. Our protocol includes appointing dedicated point persons to coordinate deliverables and meeting schedules, utilizing practical templates and governance frameworks to ensure no critical stakeholder is overlooked, and maintaining structured engagement processes with data intake, department interviews, and workflow validation. We implement escalation procedures for addressing inadvertent exclusions and conduct regular audits through our compliance suite to continuously improve communication effectiveness. Ovation’s approach leverages virtual meeting capabilities and tailored communication strategies across our service areas, ensuring consistent stakeholder engagement while reducing internal workload through our proven toolkit built from extensive hands-on experience. Global Supply Update
All Vendors - Exam Glove Allocation
Global shipping disruptions in the Middle East—particularly the closure of the Strait of Hormuz—are aƯecting the supply of key raw materials used to produce nitrile gloves, including naphtha and butadiene. This is creating industry-wide challenges for nitrile glove manufacturing.
Although there is inventory available now, manufacturers are planning 4–6 months ahead due to the uncertainty. To maintain stable supply and prevent over-purchasing, nitrile gloves have been placed on 100% allocation across the industry, with many suppliers implementing similar restrictions.
