Sample Cover Letter for Offerors to provide to their customers and clients for past performance information.

We are currently responding to the Department of Defense (DoD) Dependents Schools’ Request for Proposal/Solicitation number HE1280-14-R-0008 (as a subcontractor to______ (If applicable)). The purpose of this contract is to provide Custodial, Grounds Maintenance, and/or Facilities Maintenance services to all of DoDDS-E schools and administrative offices located in Germany and Belgium.
There is an increased emphasis on past performance in the federal source selection process. DoDDS-E is requesting that customers and clients of the offeror provide the information as described within the attached questionnaire and return it to them for evaluation. We have identified you as one of our references and respectfully request that you complete Section 3 and sign the attached questionnaire and return it to DoDDS-E.
Questionnaires are due to DoDDS-E no later than the proposal due date, 2 September 2014; however, we would appreciate an earlier response if at all possible. Questionnaires received after the due date may not be considered.
Note: In the event that the proposal due date is extended all questionnaires shall be submitted by the revised due date, which will be identified via solicitation amendment.

SUBJECT: Solicitation HE1280-14-R-0008 – DoDDS-E Germany and Belgium School Maintenance Program (GBSMP), Past Performance Questionnaire
To Whom It May Concern:
The Department of Defense Dependents Schools Europe (DoDDS-E) very much appreciates your time and thoughtfulness in completing the attached questionnaire. The information obtained from this questionnaire will be utilized to evaluate the past performance of Offerors/Contractors who wish to be awarded a contract with DoDDS-E. Please be advised that neither your name, nor position or organization, will be divulged before, during or after this questionnaire has been completed.
The questionnaire is arranged by general performance areas, e.g., quality of service/performance, cost control, timeliness, management, etc. Each area consists of a few basic questions relating to these aspects of performance with regard to the Offeror/Contractor or Offeror’s major subcontractors with which you worked. Please complete the questionnaire as indicated on the form and in the English language.
Instructions for completing Past Performance Questionnaires: Section 1 of the Past Performance Questionnaire is to be completed by the Offeror/Major Subcontractor and then provided to the assessor (with this letter). The assessor, in turn, shall then verify Section I, complete Sections 2 & 3 of the questionnaire, and then submit the final document directly to the DoDDS-E Procurement Office.
Instructions for submitting Questionnaires: In an effort to expedite receipt of the requested information, the DoDDS-E Procurement Office respectfully requests that you do not mail hard copies. Instead, please e-mail the completed questionnaire(s) directly Mr. Andrew Christen at Andrew.Christen@eu.dodea.edu.
The Subject line of the e-mail shall clearly state, “Contractor Past Performance for Solicitation HE1280-14-R-0008”.
