About this file
This is an affidavit form issued by the Florida Department of Transportation (FDOT) that nongovernmental entities must execute when entering into, renewing, or extending contracts with governmental entities in Florida, effective July 1, 2024. The affidavit requires an authorized representative of the nongovernmental entity to attest under penalty of perjury that the entity does not use coercion for labor or services as defined in Florida Statutes Section 787.06. The form requires completion of standard identifying information including the nongovernmental entity's name, address, phone number, and the authorized representative's name, title, and email address. The affidavit must be notarized, with the notary certifying that the authorized representative either was personally known or produced identification, and documenting the date and year of notarization.
This affidavit is a mandatory compliance document for the Hungerford Elementary School Sidewalk Gaps SRTS project being issued by the City of Winter Park, Florida, in Orange County. All prime contractors bidding on this federally funded Safe Routes to School project must submit this executed affidavit prior to contract award. The affidavit serves as a prerequisite certification alongside other required documents including DBE Affirmative Action Plans, prevailing wage certifications, E-Verify compliance statements, and debarment certifications. Submission of this form is required for all bidders regardless of contract size or classification and represents compliance with Florida's human trafficking and coercion prevention statutes as a condition of doing business with state and local governmental entities.
