About this file

This is an Affidavit Regarding Labor and Services form issued by the Florida Department of Transportation (FDOT) effective July 1, 2024. The form implements requirements under Florida Statutes 787.06(13), mandating that nongovernmental entities executing, renewing, or extending contracts with governmental entities provide a notarized affidavit attesting they do not use coercion for labor or services. The affidavit requires completion by an authorized representative of the nongovernmental entity and must be signed under penalty of perjury. The form collects basic entity information including the nongovernmental entity's name, address, phone number, authorized representative's name, title, and email address.

The affidavit must be notarized before a notary public, either through physical presence or online notarization, with the notary recording the date, year, method of identification verification, and commission expiration date. This documentation is a prerequisite compliance requirement for all new and existing contracts between private entities and Florida governmental agencies, establishing a standardized attestation process to ensure contractor compliance with Florida's human trafficking and labor coercion laws. The form serves as a protective mechanism for state and local procurements, including projects such as the Hungerford Elementary School Sidewalk Gaps SRTS project.
