About this file
This is a Certification of Current Capacity form (Form 525-010-46) required by the Florida Department of Transportation (FDOT) for Program Management services related to a Local Agency Program (LAP) project. The form serves as a pre-bid certification document in which contractors must declare that any proposal submitted for the Hungerford Elementary School Sidewalk Gaps Safe Routes to School (SRTS) project does not exceed their firm's current capacity, defined as the maximum capacity rating minus total uncompleted work. The certification requires contractors to report all active contracts and subcontracts, whether prime or subcontracts, whether in progress or awarded but not yet begun, and regardless of the contracting entity. The report must reflect uncompleted work as of the 15th day of the preceding month if the letting occurs before the 25th of the month, or as of the 15th day of the current month if the letting occurs after the 25th.
The form requires contractors to provide detailed information on all projects, including the owner, location, description, contract amounts, amounts sublet to others, balance of contract amounts, and uncompleted work to be performed. All amounts must be shown to the nearest $100, and contractors may consolidate and list as a single item all contracts that individually do not exceed three percent of the total and collectively amount to less than twenty percent of the total. The certification must be sworn to and subscribed by an authorized representative of the firm, with the contractor certifying that the information provided is accurate and that the "Status of Contracts on Hand" report was prepared in accordance with FDOT guidelines. This document is confidential under Florida Statute Chapter 337.14(1).
