WHEN FILLED IN THIS DOCUMENT IS SOURCE SELECTION SENSITIVE IAW FAR 3.104
Please provide concise comments regarding your overall assessment of the contractor's performance for the past three years on the contract identified. Please respond to each question as completely as possible. Ms. Erin Bevan, the Procuring Contracting Officer (PCO), is the primary point of contact (POC) and Mr. JW Terry is the alternate POC for this acquisition.
