HILL AIR FORCE BASE, UTAH

ASBESTOS INSPECTION REPORT FOR BUILDING 720 & 722

DEMOLISH FACILITY

During April 2014, Ms. Constance Delamater, with 75 FSS requested an asbestos inspection of Building 720 & 722 to demolish the facilities and construct anew. The scope of this project is as follows:

Work Location: Building 720 & 722 Work Order: #54127

Statement of Work:

Demolish facilities.

Construct new clubhouse.

This inspection report is project specific and is not a valid report for any other project(s) associated with Building 720 & 722.

The information provided for this inspection report is based on samples collected by Traver Andreasen (Utah Certified Asbestos Inspector ASB-#4397) and Neldon Anderson (Utah Certified Asbestos Inspector ASB-#0054). Samples for this survey were collected on 4 November 2013, 3 April 2014, 17 April 2014, 30 April 2014, and 2 May 2014. The Science and Engineering laboratory, utilizing EPA Method 600R-93-116, analyzed all samples, which is PLM similar to NIOSH Manual of Analytical Methods 9002. Civil Engineering personnel also reviewed all available asbestos inspection reports databases of suspect asbestos containing building materials (ACM) that could potentially be encountered in this area. The information gathered from all current and previous inspections is shown below by homogenous area.

Footages are based upon the scope of work provided by the requestor.

Building 720

Drywall/joint compound wall system (200’sq) was sampled from the northwest corner of the stairwell, from the southeast corner of the 2nd floor, and from the southeast corner of the 2nd floor. These samples were analyzed as non-asbestos containing materials (GM132816, GM132817, GM132818).

Drywall/joint compound ceiling system (200’sq) was sampled from the southeast corner of the 2nd floor, from the southwest corner of the 2nd floor, and from the center of the north side of the second floor. These samples were analyzed as non-asbestos containing materials (GM132819, GM132820, GM132821).

Bldg. 720 & 722 Building Demolition

Carpet adhesive yellow/black (5,200’sq) was sampled from the center of the 2nd floor, from the southeast corner of the 2nd floor, and from the northwest corner of the east entry of 2nd floor.

These samples were analyzed as non-asbestos containing materials (GM132822, GM132823, GM1328024).

Cove base adhesive grey (1,000’ln) was sampled from the center of west side of stairwell, from the center of south side 2nd floor, and from the center of the north side of the 2nd floor. These samples were analyzed as non-asbestos containing materials (GM132825, GM132826, GM132827).

Floor tile 12” brown/yellow adhesive (50’sq) was sampled from the center of the bar storage closet, from the center of north side of the bar storage closet, and from the south side of the bar storage closet. These samples were analyzed as non-asbestos containing materials (GM132828, GM132829, GM132830).

Building 720

Ceiling tile 2’x4’ white pockmarks/pinholes pattern (1,500 sq’) was sampled from the main floor from the northwest corner of the southeast storage room, the center of the main floor in the bar storage room, the center of the south side of the food/drink pro shop, the southeast corner of the general manager’s office, the northwest corner of the pro shop offices, and the center of the assistant manager’s office. These samples were analyzed as non-asbestos containing material (GM141227, GM141228, GM141229, GM141230, GM141231, GM141232, and GM141233).

Ceiling tile 2’x2’ white textured pattern (700 sq’) was sampled from the northwest corner of the locker room, the southeast corner of the locker room, and the center of the east side of the locker room foyer. These samples were analyzed as non-asbestos containing material (GM141234, GM141235, and GM141236).

Ceiling tile 2’x2’ white pockmarks/pinholes (2,000 sq’) was sampled from the southeast corner of the basement lobby, the northwest corner of the basement lobby, the center of the south side of the pro shop, the center of the east side of the pro shop, and the southwest corner of the pro shop. These samples were analyzed as non-asbestos containing material (GM141237, GM141238, GM141239, GM141240, and GM141241).

Ceiling tile 2’x4’ white wormygrooves/pinholes pattern (50 sq’) was sampled from the center of the north side of the pro shop offices, the center west side of the pro shop food/drink area, and the northwest corner of the pro shop office. These samples were analyzed as non-asbestos containing material (GM141242, GM141243, and GM141244).

Ceiling tile 2’x2’ white wormygrooves/pinholes (100 sq’) was sampled from the northwest corner of the pro shop, the center of the west side of the basement lobby, and the southeast corner of the pro shop. These samples were analyzed as non-asbestos containing material

(GM141245, GM141246, and GM141247).

Bat insulation foil over black over pink (3,500 sq’) was sampled from the southeast corner of the men’s locker room, the center of the south side of the pro shop food/drink area, the northeast corner of the men’s locker room, the north side of the women’s shower room, the southwest corner of the pro shop, and the southwest corner of the assistant manager’s office. These samples were analyzed as non-asbestos containing material (GM141248, GM141249, GM141250, GM141251, GM141252, and GM141253).

Pipe insulation white over foil over brown (100 ln’) was sampled from the west side of the boiler in the basement mechanical room, the north side of the boiler in the basement mechanical room, and the west side of the boiler in the basement mechanical room. These samples were analyzed as non-asbestos containing material (GM141254, GM141255, and GM141256).

Duct sealant grey (100 ln’) was sampled from the northeast corner of air handler 1 in the basement mechanical room, the north side of the northwest attic mechanical room, and the south side of the southwest attic mechanical room. These samples were analyzed as non-asbestos containing material (GM141257, GM141258, and GM141259).

Vibration collar black (50 ln’) was sampled from the southeast corner of air handler 1 in the basement mechanical room, the southwest corner of the attic mechanical room, and the northwest corner of the attic mechanical room. These samples were analyzed as non-asbestos containing material (GM141260, GM141261, and GM141262).

Pipe insulation lagging white (100 sq’) was sampled from the northwest corner of the attic mechanical room, the southwest corner of the attic mechanical room, and the west side of the basement boiler room. These samples were analyzed as non-asbestos containing material (GM141263, GM141264, and GM141265).

Pipe fitting insulation white over yellow (35 ea.) was sampled from the northwest corner of the attic mechanical room, the southwest corner of the attic mechanical room, and the north side the basement mechanical room by air handler 1 in. These samples were analyzed as non-asbestos containing material (GM141266, GM141267, and GM141268).

Pipe insulation white over foil over yellow (1,500 ln’) was sampled from the west side of the basement boiler room, the northwest corner of the attic mechanical room, the center of the west side of the east attic area, the southwest corner of the attic mechanical room, and the center of the west side of the men’s locker room. These samples were analyzed as non-asbestos containing material (GM141269, GM141270, GM141271, GM141272, and GM141273).

Duct insulation foil over brown over yellow (150 ln’) was sampled from the southwest corner of the attic mechanical room and the northwest attic mechanical room. These samples were analyzed as non-asbestos containing material (GM141274 and GM141275).

Pipe wrap black (25 ea.) was sampled from the northwest corner of the attic mechanical room, WO # 54127

24 July 2015 13:30 Hours the west side of air handler 1 in the basement mechanical room, and the southwest corner of the attic mechanical room. These samples were analyzed as non-asbestos containing material (GM141276, GM141277, and GM141278).

Bat insulation brown over black over yellow (200 sq’) was sampled from the center of the west side of the east attic mechanical room, the southeast corner of the southwest attic mechanical room, and the east side of the northwest attic area. These samples were analyzed as non-asbestos containing material (GM141279, GM141280, and GM141281).

Boiler stack insulation green (30 ln’) was sampled from the north side of the boiler in the basement boiler room, the north side of the boiler in the basement boiler room, and the west side of the boiler in the basement boiler room. These samples were analyzed as non-asbestos containing material (GM141282, GM141283, and GM141284).

Pipe fitting insulation white over white (25 ea.) was sampled from the north side of the boiler in the boiler room, the west side of the boiler in the boiler room, and the west side of the boiler in the boiler room. These samples were analyzed as non-asbestos containing material (GM141285, GM141286, and GM141287).

Blow-in insulation white (3,500 sq’) was sampled from the northwest corner of the attic, the southwest corner of the attic, the center of the west side of the main first floor entry, the southwest corner of the main 1st floor entry, and the center of the west side of the east attic area.

These samples were analyzed as non-asbestos containing material (GM141288, GM141289, GM141290, GM141291, and GM141292).

Drywall/Joint compound double layer wall system (500 sq’) was sampled from the northwest corner of the men’s locker room, the center of the west side of the men’s locker room, and the northwest corner of the general manager’s office. These samples were analyzed as non-asbestos containing material (GM141313, GM141314, and GM141315).

Drywall/Joint compound wall system (10,000 sq’) was sampled from the southeast corner of the kitchen, the west side of the pro shop storage outside entry door, the northeast corner of the bar storage area, the southwest corner of the south side of the main floor near the patio door, the west side of the basement mechanical room, the exterior northwest corner of the pro shop office, and the southwest corner of the women’s locker room. These samples were analyzed as non-asbestos containing material (GM141316, GM141317, GM141318, GM141319, GM141320, GM141321, and GM141322).

Drywall/Joint compound ceiling system (10,000 sq’) was sampled from the northeast corner of the bar janitor closet, the center of the east side of the pro shop office closet, the center of the south side of the basement boiler room, the southwest corner of the speaker main floor meeting area, from above the south side of the basement mechanical room door, the southeast corner of the entry to the kitchen storage room, and the northeast corner of the pro shop storage room.

These samples were analyzed as non-asbestos containing material (GM141323, GM141324, GM141325, GM141326, GM141327, GM141328, and GM141329).

Building 722

Sampled 30 April 2014 by Neldon Anderson

Roof flashing material black (200 ln’) was sampled from the center of the west side of the roof, the northeast corner of the roof, and the southwest corner of the roof. These samples were analyzed as non-asbestos containing material (GM141330, GM141331, and MG141332).

Roof deck material black (4,000 sq’) was sampled from the center of the north side of south side of the roof, the center of the south side of the roof, the southeast corner of the roof, the northwest corner of the roof and the center of the west side of the roof. These samples were analyzed as non-asbestos containing material (GM141333, GM141334, GM141335, GM141336, and

GM141337).

Drywall/Joint compound wall system (500 sq’) was sampled from the northeast corner of the building, the south side of the west door of the building, and the southwest corner of the building. These samples were analyzed as non-asbestos containing material (GM141338, GM141339, and GM141340).

Drywall/Joint compound ceiling system (4,000 sq’) was sampled from the southeast corner of the building, the center of the building, the center of the west side of the building, the center of the east side of the building and the northeast corner of the building. These samples were analyzed as non-asbestos containing material (GM141341, GM141342, GM141343, GM141344, and

GM141345).

9” Floor tile and associated black mastics are assumed to be an asbestos containing material. Do not disturb this material.

All Hard Thermal System Insulation encountered above ceilings, behind walls, in wall chases, in attic spaces, and in mechanical rooms is assumed to be asbestos. This includes all insulation on pipe runs, pipe saddles, tanks, and pipe fittings.

All suspect ACM not identified in the body of this report must be sampled to determine asbestos content. If materials are encountered which have not been previously identified/sampled, stop work and contact Nickolas King at 586-7094.

Remember that the locations of these building materials (homogenous areas) indicate where these materials were found, but do not necessarily mean that these are the only locations where these materials may be encountered during the project. When any new suspect ACM is encountered, stop work immediately and contact 75 CES/CEOHA to identify any new suspect building materials. As per state and federal regulations, if the scope of work or the work location changes, then a new asbestos inspection and asbestos inspection report will be required.
