1.0 SCOPE
1.1 This statement of work (SOW) defines the scope of a requirement of a construction and engineering activities at Hill Air Force Base . This SOW is a synopsis of the requirements defined in the Contract Documents which include; plans, specifications, and attachments dated 16 July 2015 and 02 May 2016, except as specifically altered by reference in this document.
1.2 The Contractor shall function as an integral team member in support of the Hill Air Force Base mission, to include the sharing of information with other Hill Air Force Base contractors and cooperation with communities, and other government entities.
1.3 Refer to the end of this document for the specific SOWs for the Asbestos and LBP Abatement requirements of this project, including sub-contractor qualifications. A Certified HAFB approved abatement contractor is an integral requirement of this contract.
1.4 Requirements of this contract include efficient project management including; accurate on-time submittals of contract deliverables and timely identification and solution of impediments to successful project execution.
Technical requirements include early involvement of all team members and sub-contractors in the process to allow for the development of the most cost-effective and technically sound solution. Hill AFB will rely on the Contractor’s expertise in recognizing and addressing problematic issues and finding solutions for a successful execution of this SOW. The Contractor shall perform all work in accordance with federal, state, and local statutes and regulations.
1.5 The Contractor is responsible for working with Golf Course operations staff to ensure minimal operational disruption. A minimum of 3 weeks scheduling and planning prior to construction is required. The site plan staging area for this project will be challenging due to the fact that golf course operations will continue throughout construction. Review and approval of a Staging Area Site Plan submittal is required prior to construction. Staging and Contractor access will occur in either the main parking area North of and/or East of the Golf Club house.

1.5 PROJECT DESCRIPTION
A. Clubhouse 095002 as designed includes the following:
The work is located at Hill Air Force Base, as indicated.
One building, Approximately 12,716 GSF which includes the following functions:
- Entry Vestibule
- Function Room
- Snack Bar
- Pro Shop
- Men's Restroom/Locker Room
- Women's Restroom/Locker Room
- Administration Offices
- Kitchen
- Mechanical/Electrical/Comm Rooms
- Storage Rooms
- Covered Patio/Dinning Deck
The design has been updated to include the spaces listed above.
The building is located just south of the existing golf clubhouse adjacent to the cart barn.
The primary access point to the site shall be off of Reservoir Road.
A parking lot has enough parking for the new building.
One loading zone shall be provided for food/merchandise deliveries.
The building shall set back from the parking a minimum of 86'-0" to comply with AT/FP requirements. No objects 6-inches in diameter shall be concealed within this perimeter.
Any plantings shall comply with UFC 4-010-01.
The main building entrance shall not face the controlled perimeter.
The building exterior shall consist of a brick tile wainscot and painted stucco above.
The roof shall be standing seam metal roof with gutters and downspouts located at the entrances only. Snow guards shall be required.
B. The other appropriations Companion Projects, KRSM 150043 and KRSM130012:
- Demolition of existing clubhouse and new retaining wall.
- Removal of Hazardous Materials.
- Extending of utilities to the site
- Upgrade/renovation of septic system
- Upgrade/renovation of fire hydrants including pumps
- Upgrade/renovation of domestic water supply
Critical periods of operation by tenants: The only period that the Golf Course operations will be shut down is for the fire pump house reconstruction and start-up of approximately 30 days within a 60 day time window.
A key component of this project includes occupancy of the existing Clubhouse during construction. Each phasing of work shall take the following non inclusive list into consideration to maintain continued operability of the facility during the construction process; tenant moves, swing space, work to be done during duty hours/after hours to mitigate impacts on the occupants, noise/dust control, temporary modifications to mechanical/electrical system to ensure continued operations are maintained, scheduling movement of furniture/computer equipment, etc.
The 18 month construction period takes this effort into consideration.

The scheduling of the construction effort is totally a contract requirement.
1) The bids must be good through 30 Sept 2016.
2) The new Facility must be fully operational before the demolition of the existing occurs.
3) The provided cut sheets are examples of products used by the Designers of Record to create the bid package. The contractor and sub-contractors may substitute equivalent products in accordance with specifications and drawings at the earliest time possible to maintain the schedule.
5) Abatement Sub-Contractors, other than those listed, will take an excessive amount of time to be approved. The process is complex and time consuming. Take the schedule into account in the selection process.
6) An understanding of and compliance with Antiterrorism Force Protection matters is required.
7) The contractor is expected to provide all requirements to complete a full and usable project per contract requirements meeting all codes and regulations referenced.
2.0 APPLICABLE DOCUMENTS
The Contractor shall identify and comply with all applicable federal, state, and local statutes. It is the Contractor’s fundamental responsibility to identify and comply with all mandatory federal and applicable DoD and Air Force requirements whether or not listed specifically in these two documents.
Refer to the Plans and Specifications for “Golf Clubhouse”, KRSM095002 “Replace Piping Utilities near Bldg 720”, KRSM150043 and ” Demo Golf Clubhouse Bldg 720”, KRSM130012 and any addendum and attached documents for all applicable requirements.
3.0 GOVERNMENT-FURNISHED INFORMATION, EQUIPMENT, AND PROPERTY (GFI, GFE,
GFP)
Refer to the Plans and Specifications and any addendum and attached documents for all applicable requirements.
Furniture will be purchased under a separate contract; however, the Contractor will be responsible for coordination and final power and data connections.
4.1 MANAGEMENT, PLANNING, AND REPORTING REQUIREMENTS
The Contractor shall implement the range of construction and engineering activities specified in this IFB and in accordance with all applicable compliance documents. The Contractor shall supply all labor, equipment, and materials necessary to accomplish the work assigned unless otherwise specified in this IFB. The Contractor

DATE: 31 May 2016 shall perform management and planning functions, including performance measurement and fund status reporting through the course of this effort.
4.2 Schedule
Work for subject project shall commence within ten calendar days after receipt by contractor of notice to proceed (NTP). Work shall be sequenced over a twenty month period, see Specification section 01 11 00 “Summary of Work” for explanation. Total contract shall be completed within 600 calendar days.
All construction and equipment associated with the renovation of the Fire pump system and house shall be on site and certified prior to beginning the outage process. Fire pump change over shall occur during Golf off season. The Contractor shall facilitate User continued operation during the entire construction period with the exception of the Fire Pump outage.
Occupancy of the new Clubhouse anticipated, 01 Feb. 2018, including access from North parking lot, shall occur prior to final sequence. Final sequence includes demolition of the existing Clubhouse and construction of the exterior stairs and turnaround area.
The contractor shall maintain a detailed working schedule that facilitates the management of the project work and provides the capability for early identification of potential schedule impacts. The schedule shall be maintained and updated monthly to accurately reflect construction progress and provide realistic forecast projections. The contractor shall provide schedule updates at either a detailed level or a summary level as requested by the Contracting Officer (CO) or Project Manager (PM). Additionally, schedule updates that reflect actual schedule progress shall be submitted on AF IMT 3065 or as approved by the CO.
4.3 Project Schedule and Planning Requirements.
The task is 100% designed.

GENERAL PLANNING CONSIDERATIONS
Project Funding:
ATA Authority from AFCEC. Notice to Offeror(s): ATA authorized. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.
Contractor access, staging, storage areas, dumpster locations, chutes and covers for debris removal
There is limited parking in the area of this project. A proposed staging plan will be required as a part of the submittal process after award.
Availability of Reference Materials: Site survey, Platt, Operation and maintenance manuals, building as-built drawings, HVAC control drawings/sequences, etc.
Refer to the project plans and Specifications.
4.4 Meeting and Conference Requirements
The Contractor shall perform a site visit and attend a pre-performance conference, preconstruction conference, and/or other meetings necessary to support construction activities. The Contractor also shall attend and support meetings and teleconferences to discuss technical or regulatory issues and project progress and status. The Contractor shall prepare, and submit for review, presentation materials and minutes for meetings and agenda’.
4.5 Contractor Documentation
The Contractor shall create and maintain a Master Document List and/or Submittal Register, i.e. AF Form 66, that includes all documents, whether the document is a deliverable or not, which are prepared during the course of this Contract. The Master Document List and its documents shall be maintained in libraries readily available for submittal to the Government. All Material Submittals shall be accomplished in accordance with the instructions pertaining to AF Form 3000, Material Approval Submittal.

4.6 Notification Requirements
The Contractor is required to notify the Contracting Officer and the Air Force Project Manager of critical issues that may affect the contract performance and/or human health and the environment. The types of issues that require notification include, but are not limited to, health risks, spills, changes in critical personnel, and finding unexploded ordnance (UXO). As an example, if UXO were discovered during field activities, the Contractor must immediately stop work, report the discovery to each of the following - the facility Point of Contact (POC), Contracting Officer and the Air Force Project Manager. The Contractor must implement appropriate safety precautions. Field activities shall not continue until clearance is received from the Contracting Officer. On critical issues, verbal notification should be made immediately, followed by written notification as soon as practical.
4.7 Permits
The contractor shall develop, coordinate, and assist in applying for and obtaining all, federal, state, local, and other applicable permits, access (including off-base easements and leases), agreements, licenses, and certifications required to perform and complete each the requirement. The Contractor shall maintain a library of these documents at the contractor’s site office on base as well as the corporate facility handling each the requirement. The Contractor shall comply with all applicable permit conditions
A SWPP will be required, Contractor to validate that area disturbed under this contract.
ENVIRONMENTAL Remarks
Hazardous materials may be present (e.g. asbestos, PCBs, lead-based paint)
The Contractor is cautioned that materials in and around this project may contain asbestos or be coated with Lead-Based Paint (LBP). The government will make every effort to locate and identify all Asbestos Containing Materials (ACM) and LBP prior to bidding; however, this is not always possible. These materials are often hidden and cannot be discovered until after demolition has begun. The failure of the government to identify all ACM and LBP in no way relieves the Contractor from his legal obligation to comply with state and federal regulations regarding the handling of asbestos, lead, or LBP.

The Contractor shall maintain a library of these documents at the Contractor’s site office as well as the corporate facility handling the requirement. The Contractor shall comply with all applicable permit conditions.
4.8 Photo Documentation
The Contractor shall prepare digital photo documentation, including site(s) and building(s) affected by the construction, field activities, and sample locations if applicable. Digital photos will be submitted using a minimum 5 mega pixel camera in JPEG format unless otherwise specified in this RFP. The Contractor shall provide an index for each set of photographs submitted, identifying the base or facility, project number, contractor, and a brief description. Photography of any kind must be coordinated through the installation, customer, or facility POC.
5.0 SITE WORK
The Contractor shall coordinate work site activities to ensure the protection of human health and the environment; the prevention of damage to property, utilities, materials, supplies, and equipment; and the avoidance of work interruptions.
The Contractor shall perform site work required under this requirement incompliance with the contract documents in coordination with the Clubhouse and companion projects.
5.1 Conservation
Activities shall be planned and implemented in a manner that protects existing site utilities, structures, surface features, service operations, monitoring and other types of wells, and the general site environment. This includes the protection of trees, shrubs, and other vegetation not in the affected zone from dust damage, soil compaction, and physical contact with machines and equipment. If appropriate, the Contractor shall conserve uncontaminated topsoil by removal, storage, or redistribution. All reasonable measures shall be taken to minimize and suppress fugitive emissions of dust, vapors, and other site materials during site work. All fill materials shall be non-contaminated. The Contractor shall conduct all operations and activities with the intent of reducing the amount of pollution generated. Specific areas to be focused on are generation of solid waste, use of hazardous materials, use of ozone-depleting chemicals, generation of hazardous waste, and use of energy and water. The Contractor shall plan, construct, operate, maintain, optimize, and decommission systems necessary to control storm water run-on and runoff; and transport surface water drainage to a treatment plant, discharge location, or any other destination.

5.2 Site Preparation
The Contractor shall perform site work as necessary to prepare sites for construction activities. Security and access controls shall be implemented to prevent unauthorized entry to sites and to protect wildlife from site exposure. The Contractor shall survey existing utilities to determine adequacy and need for modifications to support site activities. The Contractor shall obtain appropriate approvals and shall construct connections or new systems for electrical power, water, sewer, gas distribution, telephone, and other utilities, as required, to accomplish the activities specified.
5.3 Demobilization
The Contractor shall decommission facilities as necessary, and restore the site. The Contractor shall remove any temporary facilities and implement erosion control measures such as seeding, mulching, sodding, and erosion control fabrics; restore roads, structures, and utilities; and plant trees, shrubbery, grasses, and other vegetation. The Contractor shall document and report on activities and train Government personnel to perform required maintenance, as requested. Restoration of the Golf course grasses and irrigation systems shall be coordinated and approved by Golf Course personell.
6.0 CONSTRUCTION DOCUMENT PREPARATION.
6.1 Real Property Documentation: Each requirement shall require the Contractor to prepare Real Property Data utilizing the DD1354 Form. A draft DD Form 1354 and the “Building Information Checklist shall be developed in accordance with UFC 1-300-08. A draft 1354 is required to be submitted by the contractor 30 days prior to pre-final inspection. The CE PM will provide a spreadsheet to the contractor to assist in identifying line items with corresponding category codes that must be quantified on the 1354. The contractor shall furnish the required costs for each of the category codes as identified by the CE PM.
7.0 SUBMITTAL REQUIREMENTS
The submittal requirements are called out in the provided specification. During the initial mobilization stage of this project the Contractor is responsible for reviewing the contract documents and providing comment(s) if a requirement of referenced compliance in the design has not been met. Field engineering to integrate the sub-contractor submittals may be required to comply with the contract documents to address unclear design details.
The Schedule for submittals, in calendar days, will be indicated for each sub-task.
7.1 As-built Documents. Provide the following:
7.5.1 Final as-built drawings in CADD and PDF showing the as-built revision date. Revisions to the approved construction drawings shall be shown using a clouded symbol.
7.5.2 Final as-built specifications.
7.5.3 Final DD Form 1354 in compliance with UFC 1-300-08.

7.5.6.1 As-built Drawings: Three (3) half size to scale 11”x17” bound with clear plastic cover and 3 stud binding and semi ridged back sheet.
7.5.6.2 As-built Drawings: Three (3) complete electronic drawing file set on CD in bound AutoCAD (dwg) and pdf formats AutoCAD Files must be fully editable and match the drawings submitted. Partial files, files with missing attachments or layers or files formatted as read only or protected will not be accepted.
7.5.6.3 Specifications: One (1) set of revised specifications in Microsoft Word format on CD.
7.5.6.4 As-built Shop Drawings: One electronic copy of structural steel and fire suppression shop drawings in AutoCAD (dwg) and pdf format.
7.5.6.5 DD Form 1354: One electronic copy and one (1) hard copy.
7.6 Equipment submittals. Provide two copies and one digital in PDF format that contain sufficient literature, catalog cuts, brochures, Samples as determined, etc., to show compliance with the contract specifications and plans. Submit with standard AF form 3000, dated, signed and identify by individual specification section the item submitting for. This should correlate to the submittal registrar at project inception. Incomplete AF3000 or sloppy documentation of submittal items will be rejected and shall be re-submitted. Items of related equipment or materials shall be submitted at one time. Each copy shall be in a separate binder. Mark all submittals to show choices and applicable options. Equipment submittals shall be given to the Project Manager as soon as possible, but in no case shall submittals exceed twenty one (21) days after the construction start date. No payments will be authorized for materials or work, which do not have approved submittal requirements.
7.6.1 The government will within seven (7) calendar days return a minimum of one (electronic) copy of the submittal marked to indicate approval or disapproval or approved as noted. The Contractor shall make any corrections indicated on the submittals. If the Contractor considers any correction to constitute a change to the contract drawings or specifications, a written notice shall be given to the Contracting Officer. Disapproved items will require resubmission for approval within seven (7) calendar days of Contractor’s receipt. The Contractor will not be allowed to claim for time due to disapproved submittals which could have been foreseen and avoided upon review of the contract documents.

7.6.2 Submittals approved by the Air Force shall not relieve the Contractor from responsibility for complying with the requirements of this contract (See FAR 52.236-5). No substitutions for approved items, which meet contract requirements, will be allowed without approval of the Contracting Officer.
If submittals show variations from the contract requirements, the Contractor shall describe such variations in writing at the time of submission.

ASBESTOS/LEAD BASED PAINT (LBP) ABATEMENT
CONTRACTOR QUALIFICATION
REQUIREMENTS
PART 1: REGULATORY COMPLIANCE
The contractor must comply with all applicable provisions of the following:
A. 40 Code of Federal Regulations (CFR) 61 Subpart M (All notifications will be made to the Utah Division of Air Quality [UDAQ].)
B. 29 CFR 1910.1001 (Asbestos) 29 CFR 1910.1025 (Lead) C. Air Force Occupational Safety and Health (AFOSH) Standard 48-8 D. 29 CFR 1926.1101 (Asbestos) 29 CFR 1926.62 (Lead) E. Utah Asbestos Rule R307-801
PART 2: SUBMITTAL REQUIREMENTS
The following information must be submitted for prequalification in sequential order:
2.1 List of employees by date of last exams, who are medically qualified to work with asbestos (29 CFR 1926.1101) and Lead (29 CFR 1926.62). Include examiner's name, signature, medical qualifications and place of employment. At least 50% of the employees must be current at time of submittal. 100% of employees must show current medical qualification documents at the beginning of each abatement job.
2.2 Contractor's respiratory protection program (29 CFR 1926.1101, 29 CFR 1926.62, EPA
Publication 560/5-85-024).
2.3 Air sampling protocol used to support sampling requirements found in 29 CFR
1926.1101, 29 CFR 1926.62.
2.4 List of equipment to be used for asbestos/LBP work under this contract. Examples of equipment include negative air machines (with maintenance logs/ High Efficiency Particulate Air [HEPA] filter documentation), scaffolding, decontamination facilities, disposable clothing, air pressure differential measuring devices (with proof of annual calibration), HEPA vacuums, etc. Copy of manufacturers' certification of HEPA filter for vacuum. (EPA Publication560/5-85-024).
2.5 List of National Institute for Occupational Safety and Health (NIOSH)-approved respirators to be used on site by employees. Include NIOSH approval numbers and proof of approval (29 CFR 1926.1101, 29 CFR 1926.62, EPA Publication 560/5-85- 024).
2.6 Qualifications of the Independent Third Party Industrial Hygienist (IH) who will oversee the project to include certification date, college degrees, years of experience, and any other qualifications to perform this work. It is required that the IH overseeing the project be an American Board of Industrial Hygiene-Certified Industrial Hygienist or college graduate with at least four years of work related experience. The third party on site monitor must have current UDAQ Contractor/Supervisor Certification. If more than one IH consultant is used, list those who may oversee the asbestos/LBP work.
2.7 List of employees fit tested for respirator use in accordance with 29 CFR 1926.1101, 29
CFR 1926.62. List these employees by date and respirator. Include proof of employee fit testing and the name and qualifications of the fit tester. At least 50% of the employees must be current (i.e., WITHIN SIX MONTHS) at the time of submittal.
100% of employees must show current fit test documents at the beginning of each abatement job.
2.8 Asbestos - Proof of at least one year minimum work experience in Utah as a UDAQ certified asbestos abatement company. Provide a current list of employees who have received training in accordance with 29 CFR 1926.1101 and EPA Publication 560/5- 85-024. Proof of UDAQ certification is required for all asbestos work categories.
Include proof of training (i.e., written certification of completion) from a nationally recognized EPA asbestos training course for each employee. At least 100% of the employees submitted as having received training must be currently certified at the time of submittal. All workers, contractor/supervisors, project designers must show current UDAQ certification documents before the beginning of each abatement job.
2.9 Lead – Proof of at least one year minimum work experience in Utah as a UDAQ certified lead abatement company. Provide proof of employee training in accordance with 29 CFR 1926.62 (i.e., written certification of completion) from an EPA approved course for each employee. All workers, contractor/supervisors must show current UDAQ certification documents before the beginning of each abatement job.
2.10 Copies of air monitoring data taken during and after abatement from previous projects completed by contractor (not to exceed five projects). (29 CFR 1926.1101, EPA
2.11 List of prior asbestos abatement contracts performed during the past two years. Include the names, addresses, telephone numbers and dates the projects were performed. (EPA
2.12 Additional information to adequately show compliance with applicable laws and regulations in the following areas:
A. Medical certification B. NIOSH Analytical Lab Certification (independent lab) C. Disposal of waste D. Department of Transportation transport of asbestos waste E. Cleanup procedures
2.13 List of any asbestos/LBP abatement projects that have been prematurely terminated including those of any corporation, limited liability company, partnership, or sole proprietorship of which any director, officer, or agent of the contractor now making application was a director, officer, partner, or agent. Include the circumstances surrounding the termination. (EPA Publication 560/5-85-024).
2.14 List of any contractual penalties within the past two years including those of any corporation, limited liability company, partnership, or sole proprietorship of which any director, officer, or agent of the contractor now making application was a director, officer, partner, or agent, whom the contractor has paid for breach or noncompliance with contract specifications. (EPA Publication 560/5-024).
2.15 List any enforcement action levied by any Federal, State or local government agencies against the contractor for violations related to asbestos/LBP within the past two years including those of any corporation, Limited Liability Company, partnership, or sole proprietorship of which any director, officer, or agent of the contractor now making application was a director, officer, partner, or agent. . The citations will be identified by contractor including the name and location of the project, the date(s) and how the allegations were resolved. (EPA Publication 560/5-85-024).
2.16 Description of all legal proceedings, lawsuits or claims which have been filed or levied against the contractor or any of his past or present employees for asbestos/LBP related activities within the past two years including those of any corporation, limited liability company, partnership, or sole proprietorship of which any director, officer, or agent of the contractor now making application was a director, officer, partner, or agent.. PART 3: INSURANCE
3.1 The contractor will provide proof of insurance to work with asbestos-containing materials/LBP. Insurance must be occurrence type and provide up to $1,000,000.00 in coverage.
ALL INFORMATION SUBMITTED IS SUBJECT TO CONFIRMATION BY
GOVERNMENT. ANY INFORMATION FOUND TO BE FALSE, WILL BE GROUNDS
FOR IMMEDIATE DISQUALIFICATION OF CONTRACTOR.
UPDATE/ REQUALIFICATION OF ALL QUALIFIED CONTRACTORS IS REQUIRED
EVERY TWO YEARS, WITH THE EXCEPTION OF IDIQ CONTRACTORS WHICH
SHALL BE QUALIFIED FOR THE DURATION OF THE CONTRACT. (The update will require that all qualified contractors show proof of compliance with the sections listed above.)
THE GOVERNMENT RESERVES THE RIGHT TO DISQUALIFY ANY ABATEMENT
CONTRACTOR FOR INTENTIONALLY REMOVING ASBESTOS/LBP MATERIALS IN
VIOLATION OF APPLICABLE REGULATIONS.
ABATEMENT CONTRACTORS ARE REQUIRED TO COORDINATE ALL ABATEMENT
WORK/PROJECT DOCUMENTATION THROUGH THE 75 CES/CEHOA ASBESTOS/LBP
SHOP.

SOW for Asbestos and LBP Abatement
Asbestos Abatement: Asbestos containing materials (ACM)/debris may be impacted by this project, see provided inspection report for description of suspect asbestos containing materials that were sampled. Inspection reports are for informational purposes only, the contractor is responsible for verifying all quantities of asbestos containing materials to be removed. Any suspect materials discovered during the course of the project that are not identified on the inspection report shall be assumed to be asbestos containing materials until they have been sampled by the 75 CES/CEOHA office and analyzed as non-ACM. Any ACM that may be disturbed during the course of this project shall be abated by a certified abatement company that has submitted the required qualification documentation and has been approved to perform abatement work by the 75 CES/CEOHA office. An asbestos work plan must be submitted to and approved by the 75 CES/CEOHA office prior to starting any work. Pre and Post abatement visual inspection must be scheduled with the 75 CES/CEOHA office at least 24 hours in advance and any discrepancies found as a result shall be corrected by the contractor, at no additional cost to the government, before proceeding. The contractor will remove the ACM in accordance with 29 CFR 1926.1101, as outlined in DAQ R307-801, as well as base guidelines. Upon completion of the asbestos removal, all surfaces inside the regulated area shall be visibly clean of any debris and or dust. Entire work area will be HEPA vacuumed and wet cleaned prior to the final visual inspection and encapsulation. Critical barriers will cover all openings and penetrations inside the regulated work area and will remain in place until the areas have been released as clean, first by a visual inspection and then by aggressive air clearance sampling. A government representative certified as an asbestos inspector with 75 CES/CEOHA will provide this visual inspection. An independent, third party consulting firm hired by the contractor will run the aggressive air clearance samples. A three stage decon unit will be attached to each enclosure/phase prior to starting any asbestos removal. The decon unit will be equipped with hot and cold running water.
Shower and or residual water will be filtered down to 5 microns before being discharged into a sanitary sewer drain. Regulated areas will be demarcated with asbestos warning signs and barrier tape. Contractor will remove and properly package the ACM while adequately wet.
Daily abatement project documentation/logs and all air monitoring results must be submitted to the 75 CES/CEOHA office at the completion of the project. All asbestos waste shall be disposed of at an approved landfill and all waste manifests shall be signed by the 75 CES/CEOHA office with a signed copy being returned within 30 days of the waste being disposed of.
LBP Abatement: Lead based paint (LBP) may be impacted during this project, see provided inspection report for description of items tested for LBP. Inspection reports are for information purposes only, the contractor is responsible for verifying all materials have been analyzed and the quantities of materials requiring abatement. Any suspect LBP material not identified in the inspection report shall be assumed to be LBP until the 75 CES/CEOHA office has analyzed as non-LBP. Any LBP that may be disturbed during the course of this project shall be abated by a certified abatement company that has submitted the proper qualification documents and has been approved by the 75 CES/CEOHA office. A LBP work plan must be submitted to and approved by the 75 CES/CEOHA office prior to starting any work. Pre and Post abatement visual inspections must be scheduled with the 75 CES/CEOHA office at least 24 hours in advance and any discrepancies found as a result shall be corrected by the contractor, at no additional cost to the government, before proceeding. The contractor will remove the LBP in accordance with 29 CFR 1926.62 and base guidelines. Upon completion of the LBP removal, all surfaces inside the regulated area shall be visibly clean of any debris and or dust. Entire work area will be HEPA vacuumed and wet cleaned prior to the final visual inspection. Critical barriers will cover all openings and penetrations inside the regulated work area and will remain in place until the areas have been released as clean by a visual inspection. A government representative certified as an LBP inspector with 75 CES/CEOHA will provide this visual inspection. The contractor shall provide proper hygiene facilities for use by employees when exiting the regulated area. All LBP waste shall be properly packaged and labeled in containers provided and disposed of through the Hill AFB Hazwaste facility (Bldg. 514). Daily abatement project documentation/logs and all air monitoring results must be submitted to the 75 CES/CEOHA office at the completion of the project.
Specifications for containments requirements can vary depending on the ACM/LBP materials being abated as well as the methods of abatement being used. Work plans shall provide clear explanation of containments to be established and removal methods. Any questions as to acceptable base abatement practices should be submitted in writing to the contracting office to be answered by the 75 CES/CEOHA office.
*NOTE*
As of February 3, 2014 the following contractors listed below are approved to perform asbestos and LBP abatement and Hill AFB and associated sites. The contractor is not required to use the abatement contractors listed below and may utilize any contractor as long as they meet the requirements as follows. Any contractors not already approved must be currently licensed by the state of utah, have held the license for one years time, performed abatement work in the state of utah, and submit the required documentation and be approved by the 75 CES/CEOHA office prior to any work being done.
