herein known as the Medical Treatment Facility (MTF) located at
HURLBURT FIELD, FL 32544
1. This IMFE supplements and takes precedence over the Hospital
Aseptic Management System (HAMS) Performance Work Statement (PWS).
It expands on the PWS requirements or deletes PWS requirements not
applicable to this particular facility. It also establishes the
frequency of performance of task required at a frequency less often
than daily or weekdays.
2. This medical treatment facility is listed in the USAF MEDICAL LOGISTICS DIRECTORY
as 16th Medical Group FM4417 (no beds).

The contractor shall complete DD forms 1348-6 (for each item of government
furnished supplies or materials required) to the HAMS QAE prior to two
working days ahead of the established facilities management ordering day.
7. HOURS THAT THE SITE MANAGER IS REQUIRED TO BE IN THIS
FACILITY: (Ref PWS para 2.1.29.4)

DAYTIME DEDICATED FTE1.0EACH
7.1.1. The Contractor shall assign a Contract Manager to serve solely as the
full-time duty resident from 07:30 to 16:30, the working hours in the medical
treatment facility (MTF) and who shall represent the Contractor and be responsible
for the performance of the work. The name of this individual shall be designated
in writing to the A.C.O. at Altus AFB OK.
7.1.1.1. The Contract Manager or alternate shall serve as an on-site manager with
full authority to act for The Contractor on all contract matters relating to daily
operation of this contract.
7.1.1.2. The Contract Manager or alternate shall be available during normal duty hours
within 60 minutes to meet on the installation with government personnel designated
by the CO to discuss problem areas.
7.1.1.3. The Contract Manager and alternate(s) must be able to read, write, speak,
and understand English.
7.1.1.4. The Contractor shall appoint shift leaders for each shift. The shift leaders
shall be capable of interpreting and implementing all functions required within the PWS.
The names of these individuals shall be provided to the QAE.

8. HANDLING OF GOVERNMENT FURNITURE/EQUIPMENT: (REF PWS PARA 2.1.27)
The contractor is contractually obligated to clean all rooms/areas
and their contents, incuding all furniture and equipment not listed
in PWS paragraph 2.1.27.1 or below. The devices on the following list
are not to be cleaned, moved, or otherwise handled by the contractor
personnel:

11. REGULATED MEDICAL WASTE/RECYCLABLE TRASH: (Ref PWS para 1.3.31)
PWS Para 2.1.19.1 - 2.1.19.4 The HAMS' contractor shall collect and place all medical
waste, including but not limited to, infectious/contaminated, chemical,
pathological, or isolation waste into the infectious waste contractor's
container at the rear of the clinic.
Radiographic waste (isotopes) shall not be handled by the HAMS' contractor.
RS # 11 (incineration) IS NOT APPLICABLE.
TRASH AND RECYCLABLES HANDLING SERVICE:
The special handling of recyclables is a requirement.
The contractor shall perform this requirement in accordance with a
pre approved contractor written work instruction that supplements (but
does not take presidence over) base or clinic regulations.
Approval/coordination is to be accomplished at clinic level.
PWS Para 2.1.19.2 - Trash handling service for the hospital is broken down
into recyclable and nonrecyclable waste. The recyclable waste program
currently reclaims all corrugated cardboard, paper, glass and mixed cans.
Nonrecyclable waste is considered to be all material
that is placed into or adjacent to the section trash containers. This
waste shall be removed from the hospital daily and placed into the refuse
collection dumpsters at the rear of the hospital.
Recyclable trash is collected from the various recycling
bins placed throughout the clinic. Bins are used to
collect paper, glass and mixed cans. The HAMS' contractor shall
collect the segregated recyclables from the bins daily and maintain
item segregation until disposal into the appropriate sections of the bulk
recycle collection container located at the rear of the hospital. The
contractor shall not be responsible to sort or otherwise segregate recyclables
that have been mispitched into the bins. Disposable, moisture-proof
liners may be used in these bins in lieu of the daily cleaning/disinfecting
provided that they are replaced when soiled.
The contractor shall remove daily all corrugated card-
board from all administrative and treatment areas provided that it is
collapsed and placed adjacent to either the wastebaskets or recycle bins by
section personnel. The contractor shall not be required to collapse any
cardboard boxes that have not been unpacked by contractor employees.
Cardboard recyclable conntainer(s) for corrugated cardboard is located at the
rear of the hospital.
USAF CLINIC HURLBURT (AFSOC)
LINEN SERVICES ARE REQUIRED 5 DAYS PER WEEK AS NECESSARY TO MEET
PWS 2.1.20 , AS TAILORED TO THE SPECIFIC FACILITIES AS FOLLOWS:
PWS Para 2.1.20.4 - The Eglin Federal prision collects dirty Hurlburt Clinic
linens and transports to the Eglin Base Laundry five days per week. The Federal prison also
returns the cleaned linen to the Clinic twice per week.
Draperies and curtains shall be counted by the contractor prior to turn-in
for cleaning by the laundry. Quantities shall be reconciled with the laundry
cleaning tickets when property is returned. All remaining linen items will be
weighed on the Base Laundry scale prior to washing. The weight tickets
shall be verified by HAMS contractor personnel. All verified item cleaning
tickets and weight tickets shall be forwarded to the HAMS contract QAE on
a monthly basis after verification is completed.
THE HAMS' CONTRACTOR IS NOT RESPONSIBLE FOR PICK-UP AND DELIVERY
TO BUILDING 91020 AND 91041. THE CONTRACTOR IS RESPONSIBLE FOR INTERNAL
LINEN DISTRIBUTION IN THESE BUILDINGS.
PWS Para 2.1.20.14 - The contractor shall return unserviceable linen to the
soiled linen room where the contractor shall count, record quantities by
item, and store separately from serviceable soiled linens.
The contractor shall not use any linen items,
serviceable or unserviceable, for cleaning rags or for purposes other
than the item's intended use.
PWS Para 2.1.20.3 - AF Form 581 shall be maintained in each section's linen
storage area. The form's currency, accuracy and posting are the individual
responsibilities of the sections. The contractor shall ensure the form is
visibly posted and shall notify the QAE whenever a discrepancy occurs. The
contractor shall have the option of using AF Form 105-F2 cards instead of
the AF Form 581 for his linen control master inventory record. When an
actual or potential inventory shortage becomes evident, the contractor
shall notify the HAMS' QAE by submittal of completed DD FORM 1348-6.
PWS Para 2.1.20.3.1 - A monthly delivery report shall be submitted by the
contractor not later than one work day after the end of each month. The
report shall indicate the total poundage of linen items that were issued to
the various linen customer cost centers for the month. Government-furnished
daily use and monthly totals forms shall be used for this requirement.
