WORK NARRATIVE
DEMOLISH BASE PERSONNEL OFFICE (B3125)
FTQW 13-2205
16 January 2015
1. PURPOSE: The purpose of this Statement of Work (SOW) is to describe the requirements to complete the demolition of Building 3125 (Base Personnel Office) on Eielson AFB.
2. SCOPE
2.1. Provide services for the demolition of Building 3125 to include all underground utilities, and foundations as well as site restoration services to include backfilling, grading, leveling, topsoil placement, and hydro-seeding.
2.2. The contractor shall provide all necessary personnel, facilities, equipment, and materials to complete the tasks identified in this SOW.
2.3. In addition to this SOW, contractor shall adhere to guidelines provided in Eielson’s Division Specifications (Attachment 3).
3. DESCRIPTION OF WORK
3.1. Contractor shall provide a detailed work plan for the demolition of the facility and utilities to include, but not limited to:
3.1.1. Outline of intended demolition and disposal procedures;
3.1.2. Plan to protect existing buildings and utilities;
3.1.3. A description of temporary utilities and outages required. An estimation of these timelines shall be included within the proposed project schedule.
3.1.4. Estimated timeline of completion (project schedule).
3.1.5. Demolition of building and below grade foundations and footers.
3.1.6. Create uniform grade after demolition completion.
3.1.7. Removal and dispose of demolition debris to an off base landfill
3.1.8. Restore site to preconstruction conditions.
3.1.9. Submit preliminary asbuilt drawings showing location of concrete structures that are left buried. Upon Government approval of the preliminary asbuilt drawings the Contractor shall submit final asbuilt drawings.
3.2. Contractor shall not demolish any utilidors or underground utility lines still in service, but shall remove all lines up to the next utilidor. Electrical lines shall be removed up to the main electrical line north of B3125. Lines shall be capped and any hole in the utilidor shall be patched.
3.3. Contractor shall demolish the utilidor branch (concrete structure) that serves B3125 to the footprint of B3126. Contractor shall wall off / seal the remaining portion of the branch utilidor to prevent ground water intrusion.
3.4. Contractor shall demolish all concrete building foundations, sidewalks, footers, pavements and pads to include below grade footings in the area specified on the map provided with this SOW (Attachment 1).
3.5. Contractor shall repair sidewalks / pavements where damaged by demolition of adjacent sidewalks / pavements and repair curbing to make continuous where demolition of adjacent pavements creates a break in curbing.
3.6. Contractor shall remove any electrical components servicing B3125. Any lines servicing B3124 or B3126 shall not be disturbed.
3.7. Demolition area and excavation sites shall be enclosed with a temporary chain link perimeter fence. The fence shall provide 20 feet of standoff distance from the outer edge of the work area.
3.8. All structures and materials slated for demolition become property of the contractor. The contractor shall be responsible for the proper removal, handling, storage, containerization, transportation, and disposal of all waste and debris generated as a result of the demolition.
3.8.1. All non-hazardous Construction & Demolition (C&D) debris generated by the project shall be disposed of at the Fairbanks North Star Borough Sanitary Landfill, Reeves C&D Landfill, or other state permitted off-base landfill.
3.8.2. Asbestos-Containing Material (ACM) shall be identified by the contractor prior to demolition. Contractor shall identify, abate, transport and dispose of all friable ACM at Eielson AFB’s asbestos landfill. All other ACM shall be transported and disposed with demolition debris at an off base, state-permitted landfill (permitted to take Class 2 materials). Contractor shall coordinate with the Insulation Shop Supervisor, at 377-1577 for asbestos disposal procedures. See section 5.3 of this Work Narrative and specification section 01030 and 01125 (in Attachment 3), for further guidance on asbestos abatement.
3.8.3. Clean concrete (no asbestos or other contaminants present) shall be separated from other demolition debris. Contractor shall transport and dispose of all clean concrete at an Eielson AFB location to be designated at the time of disposal by the Eielson Natural Resources Office.
Contractor shall coordinate with the Eielson Natural Resources Office at 377-5182 to arrange for the disposal of clean concrete.
3.8.4. Hazardous Wastes: See Section 5.2 of this Work Narrative and specification section 01030 (in Attachment 4) for hazardous waste handling and disposal.
3.8.5. Contractor shall be responsible for all landfill application and permitting procedures for disposal of C&D debris, ACM, clean concrete, and hazardous wastes.
3.8.6. Contractor shall be responsible for notifying the applicable agencies during the demolition or disposal of all EPA and ADEC monitored materials.
3.9. Contractor shall restore project area following demolition to preconstruction conditions. Site restoration for all disturbed areas shall include, but is not limited to, filling, compacting, and bringing the project area to grade of adjacent undisturbed areas, topsoil emplacement, hydro-seeding, and irrigation.
3.9.1. Contractor shall create a uniform grade after demolition completion.
3.9.2. Topsoil and gravel are available in limited quantities on Eielson AFB for acquisition in support of site restoration activities. The contractor shall coordinate with the Eielson Natural Resources Office at 377-5182 to determine material requirements, availability, and proper pit procedures.
If using Eielson AFB borrow materials, the contractor shall provide pit development, rehabilitation, and/or expansion work as required by the Natural Resources Office for the privilege of using the borrow pits.
3.9.3. Contractor shall plant grass in graded area according to the following specifications:
3.9.3.1. Work includes spreading topsoil and furnishing all seeding as required.
3.9.3.2. Fertilizer shall meet Federal Specification O-f-241C(1).
3.9.3.3. No planting shall be done in freezing weather and grass cover shall be planted prior to 15 August.
3.9.3.4. Substitutions will only be considered if written proof of unavailability from local vendors is provided.
3.9.3.5. Seeding Material:
Lawn seed shall meet the following standard:
Variety Proportion Germination
Arctared Creeping Red Fescue 50% 90% Park Kentucky Bluegrass 50% 80%
Rate of application: 4 pounds per 1000 SF.
3.9.3.6. Nugget fertilizer shall be delivered in manufacturer's unopened containers with label stating guaranteed analysis. Fertilizer shall be dry, free of lumps and free flowing with a guaranteed analysis of 20-20-10S. Fertilizer shall be spread at a rate of 3/4 lb. per 100
SF.
3.9.3.7. Mulch for seeding shall be screened topsoil, free of large woody material.
3.9.3.8. Seeded areas shall have a minimum 6” of topsoil.
3.9.3.9. Contractor shall spread topsoil over area to be seeded evenly, smooth to an even surface and fertilize as required above.
Remove rocks and other deleterious material, roll to a smooth surface.
3.9.3.10. Contractor shall broadcast seed by hand, rake seed into bed, and roll with a 200 pound roller.
3.9.3.11. Contractor shall cover planted seed with 1/4" to 1/2" screened topsoil for top mulch and roll lightly.
3.9.3.12. Contractor shall irrigate as required to assume uniform germination and reseed any areas that fail to produce a uniform growth. Water shall not be excessive or cause erosion.
3.9.3.13. Contractor shall hydro-seed according to manufacturer’s specifications with sufficient mulch as required to support plant growth. Seed mixture shall be identical to that described above applied at a rate of 4 pounds per 1,000 square feet. Contractor shall provide water and perform general maintenance for a minimum of 30 days. If grass does not show substantial growth at that time, reseeding shall be done in those areas at contractor expense.
3.9.3.14. Contractor shall maintain all seeded areas for at least 60 days from planting, at which time all material shall show an established healthy condition. Areas that show a germination rate lower than specified or a growth rate less than other seeded areas shall be replaced. The Contracting Officer will make a preliminary inspection after the initial 60 day maintenance period listing any defects to be corrected.
3.9.3.15. All work and material shall be guaranteed for the period of one year from the date of preliminary acceptance.
3.9.3.16. Contractor shall remove all dead plant material. If damaged material cannot be satisfactorily repaired, remove and replace.
Seeded areas which have not shown vigorous growth shall also be replaced at no additional cost to the Government.
3.9.3.17. Contractor shall take precautions to preserve trees, but if a tree must be removed: Transplanting existing trees with a trunk diameter of one inch as measured 36 inches above the ground surface shall not be attempted. Trees and shrubs shall be replaced with the same species unless otherwise authorized by the Contracting Officer. Additional replacement trees are required to replace the value of a large diameter tree. Trees with a trunk diameter greater than two inch shall be replaced with 2 trees. Trees with a trunk diameter greater than three inches shall be replaced with 3 trees. Trees with a trunk diameter greater than four inches shall be replaced with 4 trees. To determine a tree’s trunk diameter measure the trunk 36 inches above the ground surface. Round up when the trunk is an odd inch, e.g., a trunk diameter of 2.75 inches would round to 3 inches and be replaced with three (3) trees. Transplanted plant material shall be guaranteed to be in vigorous growing condition for a period of 12 months. A plant shall be replaced one time under this guarantee.
Location of replacement trees shall be coordinated with the 354 CES/CEIE through the Contracting Officer.
4. GENERAL REQUIREMENTS
4.1. Attend Pre-performance meeting (timing to be coordinated with the Contracting
Officer).
4.2. A digging permit, Air Force Form 103, is required and must be executed by the contractor. This must be coordinated with the Civil Engineer Squadron customer service at 377-2100.
4.3. Work is not in a restricted area. Contractor shall perform work in a manner to minimize disruption to the government. Work will be scheduled and coordinated with the base POC. In addition, contractor shall maintain area in a neat and orderly manner. Contractor shall provide 72 hour notice of scheduled demolition date with Contracting Officer to allow for coordination of base personnel.
4.4. The performance period shall be in accordance with the successful offeror’s proposed schedule with approval by the Government. (Winter Exclusion for this contract, if determined necessary at the time of award, shall begin 1 November 2015 and end 30 April 2016. No work is required during this period but work must begin at the end of it.
4.5. The Contractor’s proposed schedule is permitted to use an extended work week, Monday-Saturday 7am-7pm. The hauling of debris off the installation shall be coordinated with base Security Forces office to avoid commute during peak traffic hours.
4.6. The Government shall provide a storage/staging area. Contractor shall be responsible for any storage trailers and/or temporary fencing needed and for restoring storage areas to preconstruction state.
4.7. Contractor shall provide portable toilet facility for work crew.
4.8. Permits: Before work is started, contractor shall obtain all necessary permits required by Federal, State, and local jurisdictions for project completion to include, but not limited to, storm water construction permit and sedimentation and erosion control.
4.9. Character of Work: The work shall be executed in the best and most workmanlike manner possible by qualified, careful, and efficient craftsmen skilled in the applicable trade, and in strict accordance with the contract documents and standards of the industry.
4.10. Location: Eielson Air Force Base is located approximately 26 miles south of Fairbanks, AK on the Richardson Highway.
4.11. All work shall be coordinated with the Civil Engineering construction inspector and Base Contracting through the Contracting Officer.
4.12. Government Furnished Information and Materials:
4.12.1. Project location plan;
4.12.2. Utility maps of the project site (electrical, water, sewer, and gas);
4.12.3. As-builts and photos.
4.12.4. Local Site Survey***
***The provided site survey shall not preclude the contractor from conducting their own asbestos demolition survey or identifying hazardous wastes. The contractor shall still be responsible for properly identifying, abating, and removing all asbestos, contaminated soil, and other hazardous wastes encountered during demolition.
4.13. Utility Outage: If a utility outage is necessary to perform the contract work, the outage shall be coordinated in advance with the Civil Engineering construction inspector. The contractor shall give written notice 14 days in advance of scheduled outage.
4.14. Salvage: It is recommended that the contractor pursue selling salvaged materials; however, the contractor is prohibited from selling any salvaged materials on government property.
4.15. Any damage to existing building or utilities shall be the responsibility of the contractor to repair at no additional cost to the Government.
5. ENVIRONMENTAL REQUIREMENTS
5.1. Compliance with Laws: The contractor shall comply, and ensure that all subcontractors comply, with all applicable federal, state, and local laws, regulations, ordinances, and standards related to environmental matters. The contractor shall also comply, and ensure that all subcontractors comply, with all specific instructions or directions given to the contractor by the base regarding environmental matters.
5.2. Hazardous Waste Handling and Disposal:
5.2.1. The contractor shall identify, characterize, containerize, store, and dispose of all hazardous wastes per spec. section 01030 and all local guidelines, and as specified.
5.2.2. Hazmat reports for B3125 are included in Attachment 5.
5.2.3. The following hazardous wastes must be turned into the Eielson Hazardous Waste Facility, Bldg 4880. It is located near the end of Flightline Avenue next to the airport terminal building. The facility supervisor may be reached at 377-2574. They are open Monday thru
Friday, regular business hours (8am-5pm) except on Federal holidays.
Drop off time for hazardous wastes is on Wednesdays from 8-11 am.
5.2.3.1. Typical universal hazardous wastes to be encountered during demolition may include, but is not limited to, the following:
- Fluorescent bulbs (must be containerized)
- Light ballasts that contain PCBs
- Thermostats
- HID lamps
5.2.3.2. The EPA requires notification of all PCB-related activity. The contractor shall be responsible for properly notifying the Region 10 EPA PCB Point of Contact of all PCB identification, testing, abatement, removal, and disposal activities. For more information on PCB compliance, please contact Eielson’s Hazardous Waste Program Manager at 377-1659.
5.2.3.3. PCBs may be present in the caulk used in windows, door frames, masonry columns and other masonry building materials in buildings built or renovated between 1950 and 1978. Any caulk within the facility known or suspected to contain PCBs shall be identified, tested, and separated from other demolition debris.
Contractor shall coordinate with their selected state-permitted landfill to determine the preferred disposal method. If necessary, contractor shall provide testing of PCB containing materials to determine proper disposal method.
5.2.3.4. Due to the age of the facility, Lead-Based Paint (LBP) may be present. Eielson AFB shall provide a TCLP data analysis for B3125 suspected to contain LBP. If TCLP results exceed acceptable limits for LBP, the contractor shall locally remove and abate LBP and dispose at the Eielson AFB Hazardous Waste Facility. Contractor shall then provide their selected state-permitted landfill with documentation confirming the abatement of LBP, allowing for the remaining debris to be disposed of in a C&D Landfill. For more information on LBP and LBP disposal compliance, please contact Eielson’s Hazardous Waste Program Manager at 377-1659.
5.2.3.5. The Contractor is responsible for identifying LBP prior to conducting repairs in B3125. Contractor shall abate LBP in B3125 only to the extent required to protect their workers during completion of work in that building.
5.2.3.6. If LBP is abated the contractor shall submit a lead based paint closure report prior to project closeout. The LBP closure report shall include LBP removal procedures, type of product removed and a map showing location of removal actions and quantities removed.
5.2.4. The contractor is responsible for properly storing, marking, labeling, securing, and transporting hazardous wastes. All hazardous wastes shall be collected in contractor furnished DOT/UN approved containers and taken to Eielson's Hazardous Waste Facility for disposal. Call the Hazardous Waste Facility prior to transporting wastes to the facility to coordinate delivery of the waste materials. The contractor shall not store hazardous waste on base for more than 30 days.
5.2.5. A Uniform Hazardous Waste Manifest shall be used by the contractor to document all parties and locations involved in the transportation, storage, and disposal of all hazardous and special wastes. This form shall be provided to the government by the contractor and signed by the Base Environmental Coordinator before the waste is transported from the limits of government property. A copy of the manifest shall be signed by the receiver of the waste and submitted to the Contracting Officer not later than forty-five days after disposal has taken place. For further questions concerning the hazardous waste manifest, contact Eielson’s Hazardous Waste Program Manager at 377-1659.
5.3. Asbestos Handling and Disposal: As part of the contract, the contractor shall submit an Asbestos Plan for asbestos abatement and removal to the Eielson’s Hazardous Waste Program Manager at 377-1659 prior to the beginning of any demolition.
5.3.1. An asbestos plan shall include an Asbestos Manifest. All asbestos waste manifests shall be signed by the Insulation Shop Supervisor, at 377-1577, prior to removal of such waste from the base. A copy of the completed manifest (signed by the receiving landfill) shall be submitted to the Insulation Shop.
5.3.2. The Contractor shall follow Section 2.1.13 of the Eielson Air Force Base Asbestos Management and Operations Plan (AMOP). A copy of the AMOP may be obtained from Eielson’s Hazardous Waste Program Manager at 377-1659.
5.3.3. The Contractor is:
5.3.3.1. Responsible for identifying asbestos containing material (ACM) prior to conducting abatement, demolition or repair activities.
5.3.3.2. Responsible for properly abating all identified ACM.
5.3.3.3. Responsible for making proper notification and providing copies to the Hazardous Waste Program Manager and Insulation Shop.
5.3.4. The site survey in attachment 5 lists known asbestos containing materials in B3125. In addition to the items listed in the site survey the Contractor shall assume the pipe insulation contains asbestos and the sewer pipe is transite in the utilidor that is to be demolished. The Contractor shall also assume the utilidor lids that are to be demolished includes asbestos mastic. Contractor shall plan the abatement and demolition accordingly.
5.3.5. Prior to final approval of job completion, an asbestos closure report shall be submitted by the contractor. The asbestos closure report shall include asbestos removal procedures, type of product removed and a map showing location of removal actions and quantities removed. This is required for updates to the facility asbestos register maintained by the Insulation Shop in accordance with 40 CFR 763, AFI 32-1052 and
AFOSH 161-21.
5.3.6. The EPA requires demolition notification for all buildings that are to be destroyed regardless of the amount of asbestos contained in the building (this requirement includes buildings that have never contained asbestos).
The contractor shall prepare and submit proper notification to the EPA.
A copy of the notification shall be given to the Eielson Hazardous Waste Program Manager at 377-1659
5.4. Underground Storage Tanks: The contractor must adhere to both statutory requirements and generally accepted industry standards for the decommissioning of underground storage tanks. The contractor shall be familiar with and strictly follow guidelines in 18 AAC 78 Underground Storage Tank (UST) regulations to include (but not limited to):
5.4.1. File an intent to close and a post-closure report with the Alaska Department of Conservation for the tank removals. All reports shall include complete copies of field notes.
5.4.2. Provide a State of Alaska UST certified worker to supervise and sign the applicable paperwork;
5.4.3. Complete a Site Assessment and Release Investigation Summary Form.
5.5. Contaminated Soil:
5.5.1. Contaminated soil is not known to be present within the area of the proposed demolition.
5.5.2. If contaminated soil is encountered, the contractor shall provide all labor, supervision, materials, equipment, tools, analytical/laboratory services, transportation and thermal remediation for treatment of contaminated soils generated in accordance with 18 AAC 75. Contractor shall assume contaminated soils will not exceed 30 Tons for bidding purposes.
5.5.3. Though contaminated soil is not expected, the contractor shall follow the Eielson AFB base-wide Sampling and Analysis Plan (SAP).
5.5.4. The contractor shall notify the Contracting Officer upon encountering any material not identified in the contract documents thought to be hazardous that could jeopardize the safety of workers or personnel in the area. Any discovery of potentially contaminated soils or water (odors, free product, sheen, staining, field screening readings > 20 ppm), not previously documented by the base, shall be reported immediately to the 354 CES/CEIE at 377-SPIL. Eielson AFB Reporting Procedures Guidance shall be followed. 354 CES/CEIE shall make all required agency spill reporting notifications. Suspect soil must be tested to determine if it contains any contaminants prior to stockpiling it. Testing of soil shall follow Alaska Department of Conservation 18 AAC 75 (Oil and Other Hazardous Substances Pollution Control). If test results determine “other than clean”, the material shall be stockpiled on the jobsite. The Contracting Officer shall provide the Contractor written notification of soil handling procedures. Excavated soil that is contaminated shall not be placed back into the excavation and shall be properly stockpiled as required in 18 AAC 78.230(4).
5.6. Spill Reporting/Clean-up:
5.6.1. Spill reporting and clean-up shall be handled per spec. 01030 (attached to this Work Narrative).
5.6.2. The contractor shall display (e.g. job site trailer, bulletin boards, etc.) the Eielson Spill Response placard developed for construction projects.
5.6.3. The name and qualifications of the individual who shall be responsible for implementing and supervising the containment and cleanup shall be provided.
5.6.4. Location of temporary fueling facilities, associated spill control measures and secondary containment provisions shall be required for any container or group of containers co-located holding 55 gallons or more of petroleum products.
5.7. Hazardous Materials: Per spec. 01030 (attached to this Work Narrative).
5.8. Nuisance and Polluting Activity: Polluting, dumping, or discharging of any harmful, nuisance, or regulated materials (such as concrete truck washout, vehicle maintenance fluids, residue from saw cutting operations, Solid Waste and Hazardous Substances) into building drains, site drains, streams, waterways, holding ponds or to the ground surface shall not be permitted and the contractor shall be held responsible for any and all damages which may result. The contractor shall conduct its activities in such a fashion which avoids creating any legal nuisance including, but not limited to, suppression of noise and dust, control of erosion, and implementation of other measures as necessary to minimize off-site impacts of work activities.
5.9. Fugitive Dust Emissions: The contractor shall not allow fugitive dust to migrate off the jobsite and shall handle in accordance with spec. 01030 (attached to this Work Narrative).
5.10. Site Maintenance and Cleanup:
5.10.1. Site Maintenance: The contractor shall protect adjacent property, buildings, and their contents from dust, dirt, or other materials.
Contractor is to wet down dry materials and settle debris enough to prevent dust or material from blowing into surrounding parking areas and buildings. Work areas shall be maintained in a neat and safe condition.
5.10.2. Cleanup: The contractor shall collect all trash, debris, refuse, garbage, etc., which he generates and place it in appropriate containers with lids or approved covers on a daily basis. Disposal shall be outside the limits of government property. Disposal shall be by sanitary landfill or other approved methods and shall conform to all local, state, and federal guidelines, criteria, and regulations.
5.11. Storage and Parking:
5.11.1. The contractor shall confine all operations (including storage of materials) on government premises to areas authorized or approved by the Contracting Officer. The contractor shall hold and save the government, its offers and agents, free and harmless from liability on any nature occasioned by the contractor’s performance.
5.11.2. Temporary buildings (e.g., storage sheds, shops, offices) and utilities may be erected by the contractor only with the approval of the Contracting Officer and shall be built with labor and materials furnished by the contractor without expense to the government. The temporary buildings and utilities shall remain the property of the contractor and shall be removed by the contractor at its expense upon completion of the work.
With the written consent of the Contracting Officer, buildings and utilities may be abandoned and need not be removed.
5.11.3. The contractor shall, under regulations prescribed by the Contracting Officer, use only established roadways, or use temporary roadways constructed by the Contractor when and as authorized by the Contracting Officer. When materials are transported in prosecuting the work, vehicles shall not be loaded beyond the loading capacity recommended by the manufacturer or the vehicle or prescribed by any federal, state, or local law or regulation. When it is necessary to cross curbs or sidewalks, the contractor shall protect them from damage. The contractor shall repair or pay for the repair of any damaged curbs, sidewalks, or roads.
5.11.4. All fuel or petroleum products shall be stored in doubled-walled tanks or within a containment area with an impermeable liner. The storage area shall be marked with appropriate signage (i.e. “Flammable Storage - No Smoking within 50 ft.”). All fuel tanks and petroleum storage containers shall be grounded and bonded in accordance with NFPA. The containment area shall be sized at 110% of the largest storage container with a minimum of one foot of freeboard for earthen berms. Any evidence of fuel or oil spills shall be immediately cleaned up by the contractor at no additional expense to the government. Bermed areas shall be removed at the completion of the job and the area restored to its original condition. All contamination shall be removed from the site and disposed of in conformance with all federal and state regulations. Costs of all soil tests as a result of spills shall be a responsibility of the contractor.
5.11.5. All other storage areas shall be kept free of debris, leaks, stains, or splashes and kept in a neat, clean, and safe condition. Any hazardous substance which may cause contamination of the storage area shall be stored in a contractor supplied trailer which shall be secured when not in use.
6. SAFETY
6.1. The contractor shall comply with all applicable Air Force Occupational Health and Safety Standards and Regulations. Compliance with the American General Contractors’ Safety Manual and Occupational Safety and Health Act shall also be a requirement of the contractor of this project.
6.2. The contractor shall not have or allow glass food or beverage containers on the construction site. Glass lined thermal containers are permitted when contained in an appropriate metal or plastic outer shield.

7. POST AWARD SUBMITTALS
7.1. Demolition Plan to include, but not limited to, an outline of intended demolition and disposal procedures, a plan to protect existing building and utilities, site grading description, and description/design of additional site restoration work to be completed after grading. Submit to Contracting Officer 30 days prior to planned demolition.
7.2. Safety Plan. Submit to Contracting Officer 2 days prior to start of onsite activity.
7.3. Uniform Hazardous Waste manifests (As required).
7.4. Asbestos Plan and Survey
7.5. Lead Based Paint Plan and Survey
7.6. Environmental Protection Plan
7.7. Other Submittals as Outlined in the AF Form 66 (Attachment 4)
8. AS-BUILT DRAWINGS
