1. Introduction:

Contractors are to comply with US Air Force, federal, state, and local environmental regulations, along with the Cannon Air Force Base (AFB) Environmental Management System (EMS). Environmental issues to be considered while completing your contract on base includes, but is not limited to:

• Hazardous material usage and management;

• Recovered materials used and recycled;

• Waste recycling/disposal and management

• New construction site organization

• Renovations and major repairs on existing structures or systems;

• Demolition/removal of structures or systems;

• Asbestos containing materials (ACM);

• Lead based paints (LBP);

• Performance of contract maintenance or services;

• Dust control (caused by activities such as land stripping, trenching, sandblasting, etc.);

• Painting or cleaning operations;

• Stormwater control and discharges;

• Protection of native plants and birds;

• Natural and cultural resources conservation;

• Use of reclaimed wastewater;

• Use of backflow prevention devices to prevent cross-connections;

• Submission of microbiological results.

2. Rules and Regulations Contractors are responsible for ensuring all subcontractors adhere to all environmental regulations while working on base. Employees and subcontractors must be informed of, and adhere to all environmental laws, regulations, and instructions as well. Pertinent regulations include, but are not limited to:

• U.S. Title 29 Code of Federal Regulations (CFR) – Labor

• U.S. Title 40 CFR – Protection of the Environment

• U.S. Title 49 CFR – Transportation

• Executive Order (EO) 13423 – Strengthening Federal Environmental, Energy, and Transportation Management

• USAF Instruction (AFI) 32-7086 – Hazardous Materials Management

• US AFI 32-7001 – Environmental Management

• US AFI 32-7042 – Solid and Hazardous Waste Compliance

• Federal Acquisition Regulation (FAR Clauses 52.223-3 to 52.223-19) 1

• New Mexico Administrative Code, Title 20 – Environmental Protection

3. Responsibilities Contractors, subcontractors and concessionaires are responsible for protecting the environment while executing contracts on Cannon AFB or Melrose AFR. In regards to the pre-work conference, the prime contractor shall brief employees and/or subcontractors on environmental regulations and policies as applicable to the project. Project sites are subject to inspection by the 27SOCES Environmental Element (27 SOCES/CEIE ) to ensure compliance with environmental regulations and practices.

Contractors shall have the Safety Data Sheet (SDS) 3 for each hazardous material brought onto Cannon AFB. The SDS shall be provided to the COR for eventual submission to 27 SOCES/CEIEC. Accidentally spilled or released hazardous substances in quantities that are beyond the contractor’s ability to immediately control and remediate must be immediately reported to the Cannon AFB Fire Department (Dial 911).

The Cannon AFB Fire Department will report these spills to 27 SOCES/CEIE.

• Contractors shall report all accidents involving Air Force people, property, or equipment, and any contractor personnel injured in the performance of the Air Force contract to the Contracting Office.

• Contractors shall take immediate corrective action to comply with environmental regulations if informed of a violation by the Contracting Office.

• Reportable spills are also defined as exceeding the reportable quantities (RQs) of Appendix A of 49 CFR

172.101. Additional information on RQ spill reporting is presented in Appendix C of this document.

• When necessary, appropriate actions will be initiated through the Contract Office via the assigned Contracting Officer Representative (COR) to correct substandard environmental practices or conditions.

1. Summary of Common Environmental Requirements and Contractor Responsibilities

Tables 1, 2, and 3 present examples of common contractor environmental responsibilities prior to, during, and at conclusion of contract performance. The tables are not all-inclusive of environmental requirements that could be encountered during a construction project. For additional guidance concerning a specific contract, the contractor should contact the Contracting Office or the COR.

At Contract Award Recovered Materials Certification

When the contractor is installing items that must contain recycled material (carpet, floor tile, park benches, restroom dividers, etc.).

By signing the contract, the contractor certifies that “Recovered Materials” to be used in the project will be at least the amount required by the contract specifications (FAR 52.223-4). Refer to www.epa.gov/cpg for further information.

De facto submission by the signing of the contract.

Before Construction/Performance Environmental Management System Awareness Level Training

Always unless deemed not applicable by the installation EMS Cross-Functional Team. EMS Awareness Level Training must be completed prior to any work being accomplished on Cannon AFB or Melrose AFR.

As required by EO 13423, the contractor shall complete EMS Awareness Training on the Environmental, Safety & Occupational Health Training Network (ESOHTN) website at:

Follow the instructions in Appendix E. The registration password is “esohtn”.

Submit a copy of the certificate of completion to the COR. The COR will provide copies of the certificates to the Cannon AFB EMS Coordinator.

Hazardous Material Review

In accordance with FAR 52.223-3 and AFI 32- 7086 section 2.4.2.2.1, “Develop installation-specific procedures and contract requirements (for inclusion in contract documents) to ensure HAZMAT brought onto the installation by contractors are properly authorized, managed, and tracked.”

At least 3 weeks prior to the start of construction, submit anticipated hazardous material usage (e.g., paint, adhesives, sealants, paint remover, refrigerants, pesticides, acids, compressed gases, etc.), a Safety Data Sheet (SDS) for each hazardous material, and a contractor AF-EESOH-MIS worksheet for each hazardous material to be used during the project.

1) AF Form 3000, for all hazmats.

2) Cannon AFB & Melrose AFR Contractor Hazmat Worksheet

(Attachment A-1) for each hazmat, by container size, weight, or volume.

3) An SDS for each hazmat, attached to the Contractor Hazmat Worksheet, for each hazmat.

4) A Cannon AFB & Melrose AFR Contractor Hazmat Usage Worksheet (Attachment A-2) attached to the Contractor Material Worksheet, for each hazmat.

Refer to Appendix A for guidance on the submission anticipated usage, SDSs and contractor AF-EESOH-MIS worksheets.

All prospective contractors must ALWAYS, submit all the information and supporting documentation prior to any materials arriving on Cannon AFB or Melrose AFR to the COR.

Comply with Federal, State and Local Requirements for Storage of Flammable & Combustible Materials.

When a fuel storage tank is used on Cannon AFB or Melrose AFR.

On-call fueling service for construction equipment is preferred.

However, when the contract allows for fuel storage in tanks on Cannon AFB or Melrose AFR, the contractor will comply with federal, New Mexico, and Air Force requirements, including, but not limited to secondary containment, labeling, and tank integrity inspections. All fuel storage is subject to inspection by the Cannon AFB Fire Department and the Environmental Element.

At least 3 weeks prior to the start of construction, submit filled out Attachment A-4, Fuel Tank Approval Worksheet, to the Contracting Office to obtain approval for fuel storage tank(s) from the Environmental Element and Fire Department.

Submission

Protection of Native Birds and Wildlife

Always when land will be disturbed (even if the area is less than one acre)

It is unlawful to kill, possess, or trade migratory birds, their parts, nests, or eggs (Migratory Bird Act Treaty).

No resident Threatened or Endangered species, as federally listed under the Endangered Species Act (ESA), are currently known at Cannon AFB. One recognized federal listed species, Lesser Prairie Chicken, occurs on Melrose AFR. When practical, the habitat for this species should be avoided during contracted activities.

However, there are mitigating steps for contacted activities that may occur in the habitat.

Federal species of concern have been observed on Cannon AFB:

• Burrowing owl

• Yellow-billed cuckoo In addition, the loggerhead shrike, a state sensitive species, and ten federal species of conservation concern were found during the 2004-2005 and 2007 surveys:

• Northern harrier

• Swainson’s hawk

• Ferruginous hawk

• Prairie falcon

• Long-billed curlew

• Burrowing owl

• Cassin’s sparrow

• Brewer’s sparrow

• McCown’s longspur

• Chestnut-collared longspur

The contractor shall notify the Contracting Office if any birds are noted to be burrowing, nesting, or living within, or immediately adjacent to, the planned or active construction area(s). The contractor shall also notify the Contracting Office of any other wildlife of concern in the planned or active construction area(s). The Contracting Office will advise the Environmental Element.

None. If no natural resources survey has been conducted a new survey must be completed before construction can begin.

Submission

Asbestos Containing Materials (ACM) Determination

Prior to renovation or demolition activities.

National Emission Standards for Hazardous Air Pollutants (NESHAP) regulations apply to minimize the release of ACM into the air.

If a project has been programmed for renovation or demolition, Cannon AFB will inform the contractor of the ACM determination.

If a project is a Simplified Acquisition of Base Engineering Requirements (SABER) project or a design/build project, the contractor will be responsible for the ACM determination as follows:

• The contractor shall test for ACM prior to renovation or demolition activities using an EPA accredited Asbestos Building Inspector.

• If ACM are present, the contractor shall submit an Asbestos NESHAP Notification/Permit Activity Application for Renovation/Demolition Activities to the New Mexico Environment Department (NMED) at least 10 days prior to the start of work. Permit application fees may apply.

• The contractor shall ensure that ACM workers are properly trained and that the ACM inspector is certified.

• The use of ACM in new construction is strictly prohibited.

Submit a copy of the Asbestos Survey Report to the Contracting Office.

If ACM are present, submit a copy of the Asbestos Removal and Demolition Activity Permit obtained from the NMED to the Environmental Element Toxic Substances Control Act (TSCA) Program Manager (PM) and the COR.

Submission

Lead-Based Paint (LBP) Determination

Testing for LBP is necessary prior to renovation, repair, demolition, sanding, sandblasting, or maintenance activities that will involve or disturb painted surfaces.

If a project has been programmed for renovation or demolition, then Cannon AFB will inform the contractor of the LBP determination.

If a project is a SABER project or a design/build project, then the contractor is responsible for the LBP determination as follows:

• LBP data may be available for review for some buildings.

Contact Bioenvironmental Engineering and the TSCA PM to determine if LBP data exists for a specific building or work area.

• If data do not exist or are inadequate, testing for LBP is required.

• If it is determined that LBP is present above the action level and LBP will be disturbed, the contractor shall ensure activities are performed in conformance with regulations governing LPB in the construction industry (OSHA 29 CFR 1926.62). Required abatement and protection procedures shall be incorporated into work plans and contract documents.

• Bioenvironmental Engineering and the LBP PM will review contractor submittals and work plans, including training certifications, for project activities that disturb LBP.

The use of paint with a lead content of 0.05% by weight, or greater, is prohibited.

Submit a copy of the LBP Survey Report to the Contracting Office.

If LBP is present, submit a copy of the LBP Removal/Demolition Activity to the Contracting Office.

Submission

Base Civil Engineering Work Clearance Request

Prior to any work; not limited to excavation, trenching, digging, or boring.

Complete Blocks 1, 3, 5, 6, and 7 of the Base Civil Engineering Work Clearance Request – 27 SOCES FORM 103v9, 24 Feb 2015 (or latest version).

As specified in Block 1, Form 103 must be accompanied by a drawing/sketch showing the street, cross street, building(s) if applicable, stating if the area(s) is to be excavated, trenched, dug, or bored, as well as the dimensions of the disturbed soil.

Work Clearance Requests are reviewed weekly and provide for a near one-stop process for Cannon AFB or Melrose AFR work clearance approval.

Completing AF Form 103 does not relieve the contractor from the responsibility for contacting New Mexico One Call.

Submit 27 SOCES FORM 103v9, 24 Feb 2015 (or latest version) and location drawing to the Contracting Office.

Work will not begin without an approved AF Form 103 and New Mexico One Call clearance.

Solid Waste Disposal, Diversion, and Recycling Plan

For contracts that will generate solid waste (construction & demolition (C&D) debris, universal waste or hazardous waste) or recyclable materials.

Solid waste must be minimized, properly managed, transported, and disposed. Waste minimization is intended to reduce consumption of energy and natural resources and should include material recycling. The Solid Waste Disposal, Diversion and Recycling Plan requirements are detailed in Appendix B. The plan will describe the segregation of waste from recyclable materials, material transport, and identify the businesses or organizations that will receive the materials

Submit the plan to the Contracting Office at least three weeks prior to the start of construction.

Submission

Review of Radioactive Material/sources or Ionizing Radiation Producing devices.

Prior to entry or use of radioactive material or device into Cannon.

Approval for use by the Installation Radiation Safety Officer. Contact 27 SOAMDS/SGPB (Bioenvironmental Engineering).

As appropriate, specification sheet, NRC documentation (permit or license), Generally Licensed Device information, operating parameters and use.

Review of lasers, ultraviolet light sources, and radio frequency producing equipment.

Prior to entry or use of class 3B, IIIb, 4, and IV laser systems. Prior to entry or use of ultraviolet light (UV) sources where exposure to DoD personnel is possible and not used IAW manufacturer directions.

Prior to entry or use of radio frequency producing equipment not intended for commercial use by the public; e.g. cellular phones used IAW manufacturer direction.

Approval for use by the Installation Laser Safety Officer and Electromagnetic Field Radiation PM (IAW AFI 49-109). Contact 27 SOAMDS/SGPB (Bioenvironmental Engineering). SGPB must assess health risk to non-contract personnel (AF military/civilians, dependents, and general public) . Use of radio frequency producing equipment may be subject to other restrictions.

Specifications sheets, operating parameters and use.

SOAMDS/SGPB

(Bioenvironment al Engineering) if operations will disturb disposal or legacy spills sites.

Immediately upon determination that disposal or legacy spills sites may be or have been disturbed. SGPB recognizes that these sites may be identified prior to performance of work or may be discovered during work.

SGPB is required to perform a health risk assessment for non-contract personnel (AF military/civilians, dependents, and general public) as these sites are impacted. SGPB may also need to coordinate survey/ sampling efforts during contractor operations to support the health risk assessment.

Contractor must detail proposed operations that will disturb these sites. CE must provide information on material in/at these sites.

Contact 27

SOAMDS/SGPB

(Bioenvironment al Engineering) if any work is to be done on potable water system infrastructure.

Immediately upon determination that the potable water system may be affected.

Coordination with SGPB is required IAW AFI 48-144 for drinking water surveillance.

Contact SGPB.

Requirements will vary IAW NMED and local Cannon AFB Instruction requirements.

Spill Control Plan

For contracts that require the use of hazardous materials.

This plan will include the procedures, instructions, and reporting requirements used in the event of a hazmat spill regulated under CERCLA, RCRA, New Mexico or local laws and regulations.

Submit this plan to Contracting Office at least three weeks prior to the start of construction.

New Source Review (NSR) Permitting

Stationary sources of air pollution must be permitted prior to construction.

Stationary sources of air pollution may require a Notice Of Intent (NOI) and/or air quality permit. (Examples: fuel burning equipment and non-personal comfort heaters, gasoline tanks.)

Submit list of potential air polluting equipment and associated technical information to the Contracting Office prior to construction.

Prohibition of Sewage Holding Tanks on Office Trailers

Use of sewage holding tanks is not permitted under the Cannon AFB Ground Water Discharge Permit issued by the NM Environment Department.

Domestic waste holding tanks are regulated by section 20.7.3 of the New Mexico Administrative Code, Liquid Waste Disposal and Treatment. Adding individual holding tanks to the Cannon AFB discharge permit would pose an unacceptable burden on Cannon AFB staff.

A contractor working in office trailers should order and use portable chemical toilets.

None.

Air Pollution Control Plan

(APCP)

For contracts that generate dust, or airborne debris.

This plan will detail measures to assure that dust, debris, materials and trash do not become airborne and travel off the project site.

Submit to the Contracting Office at least three weeks prior to the start of

Erosion and Sediment Control Plan or Storm Water Pollution Prevention Plan

(SWPPP)

One acre or more of soil disturbance for US Army Corps of Engineers (USACE) projects require a SWPPP.

An SWPPP is part of the USACE Army Corps of Engineers Environmental Protection Plan. The plan will include the type and location of erosion and sediment controls to be provided, and monitoring and reporting requirements to assure that control measures are in compliance with state and federal laws and regulations.

Submit to the USACE Army Corps of Engineers Contracting Office, at least three weeks prior to the start of construction.

Any soil disturbance requires an Erosion and Sediment Control Plan.

The Erosion and Sediment Control Plan will include the types and locations of erosion and sediment controls to be provided.

Submit to the Contracting Office at least three weeks prior to the start of

Contractor usage of hazmats on Cannon AFB and Melrose AFR must be tracked and reported to the Environmental Element on Attachment A-2 by the

Asbestos Removal

If asbestos was determined to be present (renovation or demolition activities).

The Contractor shall:

• Follow the requirements of NESHAP notification.

• Properly manage ACM waste.

• Obtain certification of ACM removal inspection.

• Ensure that the TSCA PM has checked and inspected the containment area prior to starting the abatement work and prior to the removal of the material.

• Ensure that the TSCA PM inspects the ACM in the transport vehicle and signs the manifest prior to the ACM removal from Cannon AFB or Melrose AFR to an EPA approved landfill.

• Ensure that the TSCA PM inspects the ACM in the trailer prior to departing the installation and signs off on the manifest.

Submit the NESHAP notification and manifest(s), disposal tickets and certification of asbestos removal/inspection records to Contracting Office.

Proper Management of Waste and Recyclables (Hazardous Waste, Universal Waste, Garbage, Scrap Metal, etc.)

Always. Refer to Appendix C for guidance regarding Hazardous Waste, PCB waste, and Universal Waste.

Encountering Unexpected Waste

When soil disturbance reveals strong odors or buried waste.

Land stripping and/or earthmoving (greater than 1 acre); trenching (greater than 300 feet); road construction (greater than 50 feet);

abrasive blasting, crushing/screening, etc.

Follow Air Quality Activity Permit (permit required prior to beginning construction activities).

When dust and/or particulate matter is released into the air.

Storm Water Pollution Prevention (SWPP)

Any project involving soil disturbance and 1 acre or more of soil disturbance.

Follow the Erosion and Sediment Control Plan or the SWPPP, as applicable.

Management of Investigation-Derived Waste (IDW)

IDW is waste generated during environ- mental investigation work. IDW must be properly managed.

IDW, such as soil cuttings from borehole drilling and other solid waste generated during environmental investigations, must be properly managed at the moment of generation.

Management of IDW must be addressed in the environmental investigation work plan.

Refer to Appendix D for guidance.

Submit the work plan to the Contracting Office.

Comply with Federal, State and Local Requirements for Storage of Flammable & Combustible Materials.

When a fuel storage tank is used on Cannon AFB or Melrose AFR.

Sanitary Sewer Overflows

When untreated sewage is released into the environment.

There is no minimum overflow quantity.

Contact the Water Quality PM. The Cannon AFB National Pollutant Discharge Elimination System (NPDES) permit requires that all overflows be reported to US EPA and the NMED. Depending on severity, reporting is required within 24 hours or when quarterly reports are submitted.

Provide the following information to the Water Quality PM:

(1) date, time, duration, location, estimated volume, and cause of the overflow; (2) observed environmental impacts from the overflow. The Water Quality PM can help you make this impact determination, provided the PM is shown the extent of the overflow; and

(3) actions taken to address the overflow (at a minimum, lime will be placed on the overflow and solids removed).

Submission

Proper Management of Wastewater

Always. Cannon AFB is subject to a discharge permit issued by the US EPA.

All discharges of wastewater must meet minimum discharge limits.

Contact the Water Quality PM for more information.

Industrial Ventilation and Confined Space

Always required for industrial ventilation systems.

Contractor shall ensure that the Contracting Office notifies Bioenvironmental Engineering prior to any industrial ventilation systems (systems which control a hazard) being evaluated for acceptance.

The Contractor shall ensure that the Contracting Office notifies the installation Air Quality Program Manager for any installed air emission controls to ensure compliance with base air operating permits.

Notify the Contracting Office to schedule close-out survey with Bioenvironmental Engineering.

Bioenvironmental Engineering must observe or perform all tests for any new or renovated systems prior to initial start-up to determine if the system will control the hazard. Bioenvironmental Engineering must be present for the tests of fan speed and rotation, and fan motor load, and must be present for or conduct tests for air flow in all hoods or branches.

Preservation of Cultural Resources

Required when a building or structure is over 50 years of age or was built or used during the Cold War Era, or when an archaeological site is encountered.

The New Mexico State Historic Preservation Office (SHPO) is required to review all Cannon AFB and Melrose AFR projects as mandated by the National Historic Preservation Act (NHPA), the Archaeological Resources Protection Act (ARPA), and the Native American Graves Repatriation Act (NAGRPA).

If a building, structure, or archaeological site is encountered during construction, the contractor s h a l l cease work and notify the COR, who, in turn, shall notify the Environmental Element to determine if the resource is known or is to be classified as a cultural resource site.

Submission

Use of reclaimed water for contractor fill stand located along abandoned runway outside

WWTP

compound

Use reclaimed water to the extent practical during construction activities.

Contractor shall use treated wastewater (Class 1B reclaimed water per NMED) by operating the fill stand system. The following requirements apply:

1. Water shall not be used for soil compaction around potable water lines (a sign at the fill stand notes this restriction)

2. Transport vehicles and storage tanks containing reclaimed water shall contain signs in English and Spanish identifying the contents as non-potable and advising against consumption,

3. Aboveground use of reclaimed water shall not result in excessive ponding of wastewater and at times when the receiving area is saturated or frozen.

4. Spraying and misting of reclaimed water shall be minimized.

NOTE: Spraying for construction site dust control is allowed ("spraying" insinuates that water will be directed into the air, such as for crop irrigation versus directed to the ground when applied for dust control).

Prior to first time use of reclaimed water, for a particular project, the contractor shall notify Water Quality PM at least three (3) work days prior to using the fill stand. Water Quality PM will request that the prime contractor acknowledge, in writing, receipt of these requirements.

The amount of water used for construction activities must be reported to the NMED; Cannon AFB will be responsible for this.

The WWTP operators maintain the fill stand system, but will not dispense water for the contractor. If problems with the fill stand arise, the contractor shall immediately notify the WWTP operators at (575) 784-3990 (located in Bldg 4081).

Submission

Use of backflow prevention devices to prevent cross-connections (contamination from contractor vehicle and tank systems and potable water supply)

Whenever contractor connects hoses to fire hydrants, fill stands supplying potable water (does not include reclaimed water fill stand), or other source of potable water.

Use of fire hydrants for construction activity is discouraged and will only be permitted on a case-by-case basis as approved by the base Fire Department. Fire Chief or designated representative will grant written permission (including tracking number) for use of a specific fire hydrant. Contractors should plan on using the fill stands available to them: potable hydrant near the WWTP (west of Bldg. 4081), potable systems located at wash rack across from the Pavements & Equipment Shop (Bldg. 252), or the reclaimed (non-potable) water fill stand discussed above.

Contractors shall use the appropriate backflow prevention device and assemblies for the application. Devices shall be tested prior to use to ensure functionality and reliability. A reduced pressure principal device shall be used when connections to fire hydrants.

A properly configured air gap shall be used for top-filling contractor vehicles and tanks.

Backflow prevention devices must be approved by the State or local regulatory agencies. If there is no State or local regulatory agency requirements, the backflow prevention devices must be listed by the Foundation for Cross-Connection Control & Hydraulic Research, or any other approved testing laboratory having equivalent capabilities for both laboratory and field evaluation of backflow prevention devices and assemblies.

Reduced pressure principal assemblies, double check valve assemblies, atmospheric (non-pressure) type vacuum breakers, and pressure type vacuum breakers shall meet the above requirements.

Backflow preventers with intermediate atmospheric vent shall conform to American Society of Sanitary Engineering (ASSE) 1012.

Reduced pressure principal backflow preventers shall conform to ASSE 1013. Hose connection vacuum breakers shall conform to ASSE 1011. Pipe applied atmospheric type breakers shall conform to ASSE 1001. Pressure vacuum breaker assembly shall conform to ASSE 1020.

Appendix A – Guide for Obtaining Hazmat Approval and Usage Reporting

Hazardous Materials: A hazardous material o r hazmat is defined6 by the Air Force a s i n c l u d i n g all items covered under the Emergency Planning and Community Right-to-Know Act (EPCRA) and other federal, state or local tracking requirements, the OSHA Hazard Communication Standard7, and all Class I and Class II ozone depleting substances (ODS)8. OSHA defines a hazardous material as any substance to which exposure “results or may result in adverse effects on the health and safety of employees” or “any chemical which is a physical hazard or health hazard.” Neither munitions nor hazardous waste are considered hazmats.

A contractor may not bring a hazmat onto Cannon AFB or Melrose AFR, nor use a hazmat on those installations until the contractor receives all required authorizations9.

Listed below are several categories of hazmats that require Cannon AFB approval:

• Compressed gases (all types)

• Adhesives

• Sealants

• Paints (including aerosols)

• Lubricants / oils / fuels

• Welding materials

• Solvents

• Chemicals used in testing or as additives

• Any fluids (except water) added to machinery or government-owned or leased equipment

• Pesticides / herbicides / insecticides / rodenticides

• Portland cement / concrete mix

Information Submittal Requirements: At least three weeks prior to the start of on-installation activities, the contractor must provide hazmat information noted in Table A to the Contracting Office.

Detailed instructions for completing these forms are provided below. The Contracting Office will forward these items to the HMMP Team for review.

After review of the contractor submittals, Hazmart and the HMMP Team will provide the Contracting Office with approval or rejection responses on the AF Form 3000, and descriptive responses on the Cannon AFB Contractor Hazmat Review form. The Hazmat Review form will only be provided if Hazmart identifies deficiencies in the contractor’s submittal. An example Hazmat Review form is shown below.

After the contractor’s submittals are deemed complete, the HMMP Team will provide a completed Cannon AFB & Melrose AFR Contractor Hazmat Usage Worksheet (Attachment A-2) to the Contracting Office, which will forward the worksheet to the contractor. An example Hazmat Usage Worksheet is shown below. The contractor will use the Hazmat Usage Worksheet to report the amount of Hazmats used on Cannon AFB or Melrose AFR during the project.

The contractor shall submit the Hazmat Usage Worksheet to the Contracting Office as follows:

• For contracts lasting less than 60 days, the contractor will submit the form at the completion of the work.

• For contracts greater than 60 days, the contractor will submit the form monthly, on the indicated day of the month, and at the completion of the work.

If the contractor needs to bring hazmat(s) onto Cannon AFB or Melrose AFR that were not included in the original submittals, the contractor must submit the new information to the Contracting Office.

Instruction for completing AF Form 3000, Material Approval Submittal

The AF Form 3000 is available electronically from the Contracting Office. General instructions for completing AF Form 3000 are printed on the bottom of the second page of the form. The contractor should enter the exact name from the SDS of each hazmat on the lines in the column, “Description of Material.” The completed form should be returned to the Contracting Office for review by the HMMP Team, including Hazmart.

Instructions for Completing Attachment A-1, Cannon AFB & Melrose AFR Contractor Hazmat Worksheet

Attachment A-1 information is required for the HMMP Team to authorize and track hazmats on Cannon AFB and Melrose AFR. A worksheet must be filled out for each hazmat and container combination, and an SDS must accompany each Attachment A-1. For example:

• If you have 5 hazmats for your contract performance, you will have 5 Attachment A-1 forms and 5 SDSs.

• If you have 1 hazmat that will be used in two different containers sizes, you will have 2 Attachment A-1 forms and 2 SDSs.

• All blocks must be filled in, unless noted.

For example, if a box contains 12 tubes of caulk, the container type is “tube.” For un-containerized bulk loads to be delivered by truck, such as ready-mix concrete, enter “Un-containerized Bulk Delivery” after “Other (Specify): .”

If completing the form electronically, delete non-applicable container types.

Container Volume or Weight Circle the units of weight or volume measure of the container. For un-containerized bulk loads to be delivered by truck, enter the bulk unit of measure, i.e., cubic yards or tons.

If completing the form electronically, delete non-applicable units.

Total No. of Containers Enter the total number of containers expected to be used under the contract. For un-containerized bulk loads too be delivered by truck, enter “Bulk Delivery.”

Container Size Enter the size of the container. For example, if a tube of caulk holds 4 lbs., enter “4 lbs.” For un-containerized bulk loads too be delivered by truck, enter “Bulk Delivery.”

Total Volume or Weight Multiply the total number of containers times the volume or weight of each container. For example, for 72 tubes of caulk, with each tube weighing 4 pounds, enter “288 pounds” (72 tubes x 4 pounds). For un-containerized bulk loads to be delivered by truck, enter the total bulk amount and the bulk units.

Process Type Consult the table at the end of this appendix for a listing of Air Force-defined Environmental Process Types. Enter the number and process type. Enter only one process that best describes the process in which the hazmat will be used.

Process Description Enter a brief description of the process in which the hazmat will be used.

Examples may include: “Priming & painting,” “Plumbing soldering,” “Paving, walkways & parking areas,” or “Abate rodent infestation.” Avoid very general descriptions, like “Construct building” or “Landscaping.”
