SECTION SF 30 BLOCK 14 CONTINUATION PAGE
SUMMARY OF CHANGES
A. The required response date/time has changed from 08-May-2008 04:00 PM to 15-May-2008 04:00PM B. Delete Attachment 3 entitled “Contractor Performance Questionnaire on Construction Contracts,” and incorporate the attached revised Attachment 3, entitled “Contractor Performance Questionnaire on Construction Contract,” dated 1 May 08.
C. In Section 01000, General, at Para 4, incorporate the following text as the last sentence: “It should be noted that the drawings do not show privacy fencing between housing units. This fence can be wood, vinyl or masonry and shall be demolished as part of the unit.”
D. In Section 01000, General, at Para 4.2, incorporate the following text as the last sentence: “All plantings, landscape beds and edging that are adjacent to walls, walks, patios, fences, and driveways shall be completely removed even if they are larger than five feet in width. Stand alone landscape beds away from the units shall be protected and shall remain.”
E. In Section 01000, General, at Para 4.4, incorporate the following text as the last sentence: “In Berkeley Village, the water and sewer mains are located under the sidewalk in front of the units on either side of the street. In Wayne Manor the sewer main is located in the backyards and the water main is located in front of the units on either side of the street. Only Wayne Manor has gas service. It is mostly located in the back yards, but there are a few places where it is in the front. It shall be the contractor’s responsibility to clamp off the poly service lines at the main and provide a permanent cap. The contractor shall also coordinate with Piedmont Natural Gas before doing the work. If/when the contractor wants the water service shut off, they must coordinate with the project inspector at least 48 hours in advance. If the water main services an occupied unit, the time it can be out of service is limited to 4 hours. Government personnel will shut off water valves to isolate the work as requested. They will turn the valves back on after contractor is complete with the utility termination.”
F. In Section 01000, General, incorporate the following paragraph: "4.5.1 The electrical service to the housing unit should be connected and turned on. If it is found to be turned off, the government will turn it back on if the infrastructure is still in place. If not the contractor will be required to either run extension cords to adjacent vacant units or use contractor supplied generators. The contractor will not be allowed to run extension cords to occupied units."
G. In Section 01000, General, at Para 4.10, delete the first two sentences that read: “Lead-based paint test results for the units are not available. Contractor should be aware due to the period of construction for the facilities (circa 1958), lead-based and/or lead-containing paint can be anticipated.”
H. In Section 01000, General, incorporate the following paragraph: “14.3 In the phase 1B area, for the four units on March Lane, the contractor should expect the driveway aprons will be gone and that new curb/gutter will be installed in their place. Contractor shall protect this new curb/gutter and/or replace if damaged, at no cost to the government.”
I. In Section 01000, General, incorporate the following paragraph: “14.4 Only equipment for the demolition and transportation of rubble will be allowed. Plants or equipment to refine material, such as noisy crushers, will not be permitted.”
J. In Section 01000, General, at Para 15.2, in the second to the last sentence, change the sentence from: “If awarded, option will be awarded within 90 days of contract award.” To: “If awarded, options will be awarded within 60 days of contract award.”
K. In Section 02051, Asbestos Removal, Cleanup, & Disposal, incorporate the following paragraph:
“5.1 Asbestos and lead based paint assessment letters for the units being demolished under this contract are incorporated as Attachment 1 to the Specifications. Quantities are estimations only and the contractor shall make their own judgments based on these letters and the contract drawings” (Attachment 2 to this Amendment).
L. In Section 02485, at Para 6.1, incorporate the following text in the second sentence: “hydro-seeding” after “….hand seeders…”
M. Drawings: On Sheets A-5, A-7, A-9, and A-11 of VKAG 07-5012P1, Legend and General Demolition Notes; delete all references to asbestos containing material. All units in Wayne Manor have NO asbestos containing material. See Attachment 1 to the Specifications for assessment letter.
