SUMMARY OF CHANGES
(End of Summary of Changes)
(1) Whether the offeror, and/or any of its principals, has or has not, within the last five years, in connection with the award to or performance by the offeror of a Federal contract or grant, been the subject of a proceeding, at the Federal or State level that resulted in any of the following dispositions:
(i) In a criminal proceeding, a conviction.
(ii) In a civil proceeding, a finding of fault and liability that results in the payment of a monetary fine, penalty, reimbursement, restitution, or damages of $5,000 or more.
(iii) In an administrative proceeding, a finding of fault and liability that results in--
(A) The payment of a monetary fine or penalty of $5,000 or more; or
(B) The payment of a reimbursement, restitution, or damages in excess of $100,000.
(iv) In a criminal, civil, or administrative proceeding, a disposition of the matter by consent or compromise with an acknowledgment of fault by the Contractor if the proceeding could have led to any of the outcomes specified in paragraphs (c)(1)(i), (c)(1)(ii), or (c)(1)(iii) of this provision.
(2) If the offeror has been involved in the last five years in any of the occurrences listed in (c)(1) of this provision, whether the offeror has provided the requested information with regard to each occurrence.
(d) The offeror shall post the information in paragraphs (c)(1)(i) through (c)(1)(iv) of this provision in FAPIIS as required through maintaining an active registration in the System for Award Management database via https://www.acquisition.gov (see 52.204-7).

52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (FEB 2012)
Addendum to 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: “The offeror agrees to hold the prices in its offer firm for 180 calendar days from the date specified for receipt of offers.”
Addendum to 52.212-1(d), Product Samples, is deleted in its entirety.
Addendum to 52.212-1(e), Multiple Offers, is deleted in its entirety.
Addendum to 52.212-1(g), Contract Award. The following sentence is added: “If the Contracting Officer determines discussions are to be conducted, a competitive range will be established.”
Addendum to 52.212-1(h), Multiple Awards, is deleted in its entirety.
NOTICE FOR ALL OFFERORS
Site Visit
A site visit will be scheduled for Monday, 15 September 2014 at 1:00 PM CST at JBSA Lackland, TX. Interested parties should send a request to SrA Daniel Straw at daniel.straw.1@us.af.mil NOT LATER THAN Friday, 12
September 2014 at 4:00 P.M. CST. The request should include the following information:
1) Name(s) of the individuals who will be in attendance
2) State or Federal issued Identification such as Driver’s License, State Identification, or U.S. Passport https://www.acquisition.gov/ mailto:daniel.straw.1@us.af.mil
3) Social Security Number.
This information will be used to obtain base access passes and safeguarded IAW procedures outlined in the Privacy
Act of 1974 and DoD 5400.11R.
ADDENDA TO 52.212-1
Proposal Preparation Instructions
1.0. To assure timely and equitable evaluation of proposals, Offerors must follow the instructions contained herein.
Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub factors. Failure to meet a requirement may result in an offer being ineligible for award. Taking exception to any provision in the solicitation will render the proposal ineligible for award. The offeror shall acknowledge all of the amendments issued to this solicitation. The proposal in response to this solicitation shall consist of three (3) separate volumes: Volume I
- Price Proposal, Volume II – Past Performance and Volume III - Technical Proposal. All documents submitted in response to this RFP must be fully responsive to and consistent with the requirements of the RFP. The maximum of allowable pages for the technical proposal, including attachments should be no more than 10 pages total. If any proposal exceeds 10 pages total for the technical proposal, the Government will only evaluate the first 10 pages.
To be considered, the complete proposal must be received not later than the date and time established in Block 8 of the SF 1449, at 502d CONS/JBKAA, Attn: A1C Daniel Straw, 1655 Selfridge Avenue, JBSA Lackland TX
78236. Proposals received after exact date and time established for receipt of proposals will be late and will not be considered for award. All proposals must be in English. Fax or email proposals will not be considered.
All questions must be submitted in writing no later than Thursday, 18 September 2014. Questions submitted after this date will not be considered.
2.0 General Information
2.1 Proposal Format
2.2.1 Guidance
(a) This section provides general guidance for preparing proposals as well as specific instructions on the format and content of the proposal.
(b) The Offeror's proposal must include all data and information requested and must be submitted in accordance with these instructions.
(c) All the requirements specified in this solicitation are mandatory.
(d) You are representing that your firm will perform all the requirements specified in the solicitation by submitting your proposal.
(e) These instructions are intended to elicit brief and specific responses. However, simply stating that the
Offeror understands and intends to comply with this effort or paraphrasing statements in the solicitation is inadequate.
(f) The offer shall comply with the requirements as stated in the solicitation.
(g) Elaborate brochures or documentation, binding, detailed artwork, or other embellishments are unnecessary and are NOT desired.
(h) All pricing information shall be addressed ONLY in the Price Proposal Volume.
2.2.2 Original Proposal, Hard Copies and Electronic Copies
2.2.2.1 Proposal
Each hard-copy of Volumes I, II and III shall be submitted and formatted as follows:
