Addendum to 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: “The offeror agrees to hold the prices in its offer firm for 180 calendar days from the date specified for receipt of offers.”
Addendum to 52.212-1(d), Product Samples, is deleted in its entirety.
Addendum to 52.212-1(e), Multiple Offers, is deleted in its entirety.
Addendum to 52.212-1(g), Contract Award. The following sentence is added: “If the Contracting Officer determines discussions are to be conducted, a competitive range will be established.”
Addendum to 52.212-1(h), Multiple Awards, is deleted in its entirety.
NOTICE FOR ALL OFFERORS
Site Visit
A site visit will be scheduled for Monday, 15 September 2014 at 1:00 PM CST at JBSA Lackland, TX. Interested parties should send a request to SrA Daniel Straw at daniel.straw.1@us.af.mil NOT LATER THAN Friday, 12 September 2014 at 4:00 P.M. CST. The request should include the following information:
1) Name(s) of the individuals who will be in attendance
2) State or Federal issued Identification such as Driver’s License, State Identification, or U.S. Passport
3) Social Security Number.
This information will be used to obtain base access passes and safeguarded IAW procedures outlined in the Privacy Act of 1974 and DoD 5400.11R.
ADDENDA TO 52.212-1

1.0. To assure timely and equitable evaluation of proposals, Offerors must follow the instructions contained herein.
Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub factors. Failure to meet a requirement may result in an offer being ineligible for award. Taking exception to any provision in the solicitation will render the proposal ineligible for award. The offeror shall acknowledge all of the amendments issued to this solicitation. The proposal in response to this solicitation shall consist of three (3) separate volumes:
Volume I - Price Proposal, Volume II – Past Performance and Volume III - Technical Proposal. All documents submitted in response to this RFP must be fully responsive to and consistent with the requirements of the RFP. The maximum of allowable pages for the technical proposal, including attachments should be no more than 10 pages total. If any proposal exceeds 10 pages total for the technical proposal, the Government will only evaluate the first 10 pages.
To be considered, the complete proposal must be received not later than the date and time established in Block 8 of the SF 1449, at 502d CONS/JBKAA, Attn: Andrew S. Sorensen, 1655 Selfridge Avenue, JBSA Lackland TX 78236. Proposals received after exact date and time established for receipt of proposals will be late and will not be considered for award. All proposals must be in English. Fax or email proposals will not be considered.
All questions must be submitted in writing no later than Thursday, 18 September 2014. Questions submitted after this date will not be considered.
2.0 General Information
2.1 Proposal Format
2.2.1 Guidance
(a) This section provides general guidance for preparing proposals as well as specific instructions on the format and content of the proposal.
(b) The Offeror's proposal must include all data and information requested and must be submitted in accordance with these instructions.
(c) All the requirements specified in this solicitation are mandatory.
(d) You are representing that your firm will perform all the requirements specified in the solicitation by submitting your proposal.
(e) These instructions are intended to elicit brief and specific responses. However, simply stating that the
Offeror understands and intends to comply with this effort or paraphrasing statements in the solicitation is inadequate.
(f) The offer shall comply with the requirements as stated in the solicitation.
(g) Elaborate brochures or documentation, binding, detailed artwork, or other embellishments are unnecessary and are NOT desired.
(h) All pricing information shall be addressed ONLY in the Price Proposal Volume.
2.2.2 Original Proposal, Hard Copies and Electronic Copies
2.2.2.1 Proposal
Each hard-copy of Volumes I, II and III shall be submitted and formatted as follows:
(a) Proposal shall be bound in a three-ring loose-leaf binder, which shall permit the volume to lie flat when open.
(b) Each volume shall have a cover sheet, clearly marked as volume number, Offeror's name, solicitation title, and solicitation number.
2.2.2.2 Hard Copies (Provide 3 copies)
(a) Each hard copy volume of the proposal shall be separately bound in a three-ring loose-leaf binder, which shall permit the volume to lie flat when open.
(b) A cover sheet shall be bound in each book, clearly marked as to volume number, copy number, Offeror's name, solicitation title, and solicitation number.
(c) A Label shall be placed in the front cover of each binder clearly identifying volume number, copy number, Offeror's name, solicitation title, and solicitation number.
(d) A Label shall be placed in the spine of each binder clearly identifying volume number, copy number, Offeror's name, solicitation title, and solicitation number.
(e) The Offeror's proposal shall include a signed copy of the solicitation. Complete blocks 12, 17, 26, and 30 of the SF 1449. Signature by the Offeror on the Standard Form 1449 constitutes an offer, which the Government may accept. Complete all contract clause fill-ins if applicable. The "original" copy shall be clearly marked under separate cover. The Offeror's proposal shall include a signed copy of the Representations and Certifications. The Offeror may submit a signed copy from the Online Representations and Certifications in the System for Award Managagement (SAM) website in lieu of completing the representations and certifications included in the solicitation.
2.2.2.3 Electronic Copies (E-Copies) (Volume I only) (Attachment 4)
(a) General requirements.
(b) All electronic copies shall be submitted on CD-ROM.
(i) Offerors shall ensure electronic copies on CD-ROM are virus free.
(ii) Electronic copies of each volume shall be compatible with the following available Government equipment and software products:
a. Windows compatible PCs
b. Adobe Acrobat Reader 9.0
c. Microsoft Office 2010
d. Microsoft Excel 2010
(iii) If discrepancies are identified between the electronic proposal and the “original” paper proposal, the paper proposal shall prevail.
2.2.3 Sufficient Detail
(a) The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims.
(b) The proposal should not simply rephrase or restate the Government's requirements, but rather shall provide convincing rationale to address how the Offeror intends to meet these requirements.
(c) Offeror(s) shall assume that the Government has no prior knowledge of their capabilities and experience, and will base its evaluation solely on the information presented in the Offeror's proposal.
2.2.4 No Cross-Referencing
(a) Each volume shall be written on a stand-alone basis so that its contents may be evaluated without cross-referencing to other volumes of the proposal.
(b) Information required for proposal evaluation, which is not found in its designated volume, will be assumed to have been omitted from the proposal.
(c) Each response to each Sub factor shall be self-contained. No cross-referencing between Sub factor responses is allowed.
2.2.5 Table of Contents
(a) Table of Contents.
(1) Each volume shall contain a detailed table of contents to delineate the subparagraphs within that volume.
(b) The Table of Contents will not count against the page limitations for their respective volumes.
2.2.6 Page Format Restrictions and Limitations
2.2.6.1 Page Format
(a) Pages shall be formatted to 8.5 x 11 inches.
(b) Text pages shall be portrait oriented. Graphics and tables may be landscape oriented if required to properly present the information.
(c) Pages shall be typed with single line spacing.
(d) No condensed fonts allowed. Times New Roman, Arial, or similar font preferred. The font size shall be no less than 10 point.
(e) Margins on all four edges of each sheet will be at least one inch.
(f) Proprietary statements, security markings, and page numbers can fall within the defined margin.
(g) Except for the Pricing Volume, each required section within a volume shall be numbered consecutively.
(h) These page format restrictions shall also apply to responses to Evaluation Notices (ENs).
2.2.6.2 Page Limitations
(a) Page limitations shall be treated as maximums.
(b) Each side of each 8.5 x 11 inch sheet that displays printed material shall be counted as 1 page for page limitation purposes.
(c) If page limits are exceeded, the excess pages will not be read or considered in the evaluation of the proposal.
(d) Page limitations shall be placed on responses to ENs. The specified page limits for EN responses will be identified in the letters forwarding the ENs to the Offeror(s).
2.2.6.3 Pages Counted
Each page shall be counted except the following.
(a) Cover pages
(b) Table of contents
(c) List of figures
(d) Glossaries
(e) Tabs
(f) Dividers
(g) Blank pages
2.2.7 Status as a Service-Disabled Veteran-owned Small Business Concern
All offerors must meet the requirement of FAR 19.1403
3.0 VOLUME I – PRICE PROPOSAL
Volume Organization
I. VOLUME I – PRICE PROPOSAL
(a) General Instructions: The Contracting Officer (CO) has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the CO will review this determination and if, in the CO’s opinion, adequate price competition exists no additional cost information will be requested. However, if at any time during this competition the CO determines that adequate price competition no longer exists; offerors may be required to submit other than certified cost or pricing data to the extent necessary for the CO to determine the reasonableness of the price. Unbalanced pricing poses an unacceptable risk to the Government and may be a reason to reject an Offeror’s proposal. Compliance with these instructions is mandatory and failure to comply may render your proposal ineligible for award.
Offers shall be sufficiently detailed to demonstrate their reasonableness and balance. The burden of proof for credibility of proposed costs/prices rests with the Offeror.
(b) Section B (Bid Schedule): The Offeror shall complete the Section B (Bid Schedule) of the RFP and submit with Volume I Price Proposal and submit the electronic Bid Schedule provided at Attachment 4 of this
RFP. A unit price shall be proposed and inserted for each Firm Fixed Price (FFP) Contract Line Item Number (CLIN) for each performance period and shall be rounded up to two decimal places. The extended amount must equal the unit price multiplied by the number/quantity of units. The proposed prices shall be based on the Offeror’s own technical approach submitted in the Technical Proposal (Volume II) and the Government’s Performance Work Statement (PWS) requirements as set forth herein. Offerors who change the quantities stated in the solicitation will render the proposal ineligible for award.
II. VOLUME II – PAST PERFORMANCE
(i) The contractor shall provide a list of recent, relevant past performance efforts for at least two but no more than five of the most relevant contracts performed for Federal agencies and/or commercial customers. The government will use this information to evaluate the quality and extent of the offeror’s past performance deemed relevant to the requirements of this RFP. The list shall at a minimum contain the applicable contract number, performance dates, current points of contact (including phone/email), and whether a Contractor Performance Assessment Report (CPAR) was completed. The efforts provided shall have involved the same or similar services with a similar scope and magnitude of efforts and complexities as this solicitation requires. For all effortslisted the contractor shall request that other sources provide information to the Government on their behalf. Any past performance information provided by other sources shall be submitted on the Past Performance Questionnaire at Attachment 3. Questionnaires shall be submitted by the other source directly to the office designated in the questionnaire no later than the exact time specified for receipt of proposals. The Government will not consider any questionnaires submitted by or through the contractor.
(ii) The Government will not consider nor evaluate past performance that concluded more than three years from the original solicitation release date. Relevant contracts include Aircraft Wash and Related Services of similar size and scope to the requirements listed in the Performance Work Statement (PWS). Relevant performance involves wash, hazardous waste, and lavatory services at an aircraft wash facility housing C-5 or equivalent aircraft. Examples of equivalent aircraft include, but are not limited to, C-17, DC-10, KC-135, 777, 767, 757, 747, 737, or 727. The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. The offeror shall provide at least one contract where the prime performed 50% or more of the work.
III. VOLUME III – TECHNICAL PROPOSAL
(a) General Information: All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that you will perform all the requirements specified in the solicitation. It is not necessary or desirable for you to tell us so in your proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.
(i) The Technical Volume shall be specific and complete. In the Technical Volume, address your proposed approach to meeting the minimum performance or capability requirements of each technical Subfactor.
(ii) Marketing presentations are not wanted and will not be evaluated. Hard and compelling evidence of capabilities is required to receive credit for evaluation factors.
(iii) Legibility, clarity, and coherence are very important.
(iv) Your responses will be evaluated against 52.212-2, Evaluation Factors.
(v) Provide as specifically as possible, the actual methodology you would use for accomplishing and satisfying the requirements.
(vi) All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that you will perform all the requirements specified in the solicitation. It is not necessary or desirable for you to tell us so in your proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.
(b) Technical Factor 1: Mission Capability
(1) Technical Subfactor 1 – Program Management
(a) The Offeror shall submit an approach for structuring, staffing and retaining personnel to effectively accomplish the work under this contract to include the proposed manning mix of personnel. The approach shall also include the offeror’s approach for effectively utilizing key personnel to plan, control, direct the labor force and accomplish the services to meet the requirements of the performance work statement.
(b) The offeror shall submit procedures for staffing, responding to and performing on- call situations or extraordinary requirements outside the normal hours of operation.
(c) Technical Subfactor 2- Quality Control Plan
(1) The Offeror shall provide an approach and a process to efficiently and effectively measure, track, analyze report, and implement corrective actions. This process shall include:
(a) Collecting and reviewing complaints from flight crew and passengers (individuals that the offeror will not be in direct contact during the performance of the PWS).
(b) Documenting checks during aircraft servicing to prevent discrepancies in services.
(c) Evaluating discrepancies or complaints and correct or improve processes to prevent recurrence of such discrepancies or complaints.
(d) Preventing and correcting any discrepancies as they happen without stopping performance of the tasking at hand.
