INSTRUCTIONS
NOTICE OF INTENT - PRIMARY PERMITTEE
For Coverage Under the 2008 Re-Issuance of the NPDES General Permits
To Discharge Storm Water Associated With Construction Activity
Please print or type the Notice of Intent (NOI) form.. Any NOI that contains illegible or incomplete information will not be accepted, will be returned and the construction site will not be granted Permit coverage. All information requested on the NOI must be submitted in order for the NOI to be valid. Any information requested on the NOI that is not applicable to the primary permittee or to the construction site must be marked “N/A.” Please do not leave any sections blank in the NOI.
Who must file a Notice of Intent Form - The Owner and/or Operator of a facility/construction site that has a discharge of storm water where construction activities occur must apply for a National Pollutant Discharge Elimination System (NPDES) Permit. The Georgia Environmental Protection Division (EPD) re-issued the General NPDES Permits for Storm Water Discharges Associated with Construction Activity on August 1, 2008. The Permits are available for review at the EPD District Offices and on the EPD website, www.gaepd.org. It is highly recommended that the permittees read and understand the terms and conditions of the Permits prior to submitting a NOI. Please contact the appropriate EPD District Office as listed on the following pages for assistance in completing the NOI.
Where to file a Notice of Intent Form - The NOI and the attachments, as applicable, must be submitted to the appropriate EPD District Office as listed on the following pages. Please submit only the first three pages of this document with the applicable attachments.
Section I - Site/Owner/Operator Information
The construction site name and location information (i.e., GPS location of construction exit, street address, city, county) must be sufficient to accurately locate the construction site. If the construction site does not have a street address, please provide sufficient information to accurately locate the construction site. If additional space is needed, attach the location information to the NOI.
A duly authorized representative may be either a named individual or any individual occupying a named position that the primary permittee has authorized to sign all reports, certification statements, or other information requested by EPD.
The facility/construction site contact is the person who the primary permittee has assigned the responsibility for the daily on-site operational control.
Please do not leave any blanks in this section. Any information requested on the NOI that is not applicable to the primary permittee or to the construction site must be mark “N/A.”
Section II – Construction Site Activity Information
For construction activities that began prior to the effective date of the Permits, the start date (month/date/year) must be the actual start date of construction activities.
Estimated disturbed acreage is the total number of acres, to the nearest tenth (1/10th) acre, that will be disturbed by the primary permittee and all secondary permittees.
Please do not leave any blanks in this section. Any information requested on the NOI that is not applicable to the primary permittee or to the construction site must be mark “N/A.”
Section III - Receiving Water Information
If the facility/construction site discharges storm water directly or indirectly to the receiving water(s), and not through a municipal separate storm sewer system (MS4), enter the name of the receiving water(s) and indicate whether the water(s) is a trout stream or a warm water fisheries stream. Attach a written description and location map identifying the receiving water(s).
If the facility/construction site discharges storm water to a municipal separate storm sewer system (MS4), enter the name of the owner/operator of the MS4 (e.g., city name or county name) and the name of the receiving water(s) at the point of discharge from the MS4. A MS4 is defined as a conveyance or system of conveyances (including roads with drainage systems, municipal streets, catch basins, curbs, gutters, ditches, man-made channels or storm drains) that is owned and/or operated by a city or county which is designed or used for collecting or conveying storm water. It may be necessary to contact the city or county that owns and/or operates the MS4 to determine the name of the receiving water(s). Indicate whether the receiving water(s) is a trout stream or a warm water fisheries stream. Attach a written description and location map identifying the receiving water(s).
Any permittee who intends to obtain coverage under the Permits for storm water discharges associated with construction activity into an Impaired Stream Segment, or within one (1) linear mile upstream of and within the same watershed as, any portion of an Impaired Stream Segment identified as “not supporting” its designated use(s), as shown on Georgia’s 2008 and subsequent “305(b)/303(d) List Documents (Final)” at the time of NOI submittal, must satisfy the requirements of Part III.C. of the Permits if the Impaired Stream Segment has been listed for criteria violated, “Bio F” (Impaired Fish Community) and/or “Bio M” (Impaired Macroinvertebrate Community), within Category 4a, 4b or 5, and the potential cause is either “NP” (nonpoint source) or “UR” (urban runoff). Those discharges that are located within one (1) linear mile of an Impaired Stream Segment, but are not located within the watershed of any portion of that stream segment, are excluded from this requirement. Georgia’s 2008 and subsequent 305(b)/303(d) List Documents (Final)” can be viewed on the EPD website, www.gaepd.org/Documents/305b.html.
If a Total Maximum Daily Load (TMDL) Implementation Plan for sediment has been finalized at least six (6) months prior to the permittee’s submittal of the NOI, the Erosion, Sedimentation and Pollution Control Plan (Plan) must address any site-specific conditions or requirements included in the TMDL Implementation Plan that are applicable to the permittee’s discharge(s) to the Impaired Stream Segment within the timeframe specified in the TMDL Implementation Plan. If the TMDL Implementation Plan establishes a specific numeric wasteload allocation that applies to an permittee’s discharge(s) to the Impaired Stream Segment, then the permittee must incorporate that allocation into the Erosion, Sedimentation and Pollution Control Plan and implement all necessary measures to meet that allocation. A list of TMDL Implementation Plans can be viewed on the EPD website, www.gaepd.org.
Please do not leave any blanks in this section. Any information requested on the NOI that is not applicable to the primary permittee or to the construction site must be mark “N/A.”

1.0
INTRODUCTION
This project specific Erosion, Sedimentation and Pollution Control (ESPC) Plan is intended to satisfy the requirements under the State of Georgia, Department of Natural Resources, Environmental Protection Division (GA EPD) General Permit No. GAR100001. This permit regulates point source discharges to the waters of the State of Georgia from stand alone construction projects that will result in land disturbance equal to, or greater than, one (1) acre occurring after the effective date of this permit.
Specifically, this plan covers the [insert the name of the specific construction project here] project at Robins Air Force Base (Robins AFB) located in Houston County, Georgia. The location of Robins AFB is presented in Figure 1-1. The general location (or vicinity map) of the construction project is presented in Figure 1-2. {Note: This location or vicinity map must delineate surrounding area roads and highways. If the project is being done in phases, each individual phase must be delineated and labeled.} The objective of this plan is to control erosion, sedimentation, and other pollutants at construction sites through the implementation of best management practices (BMPs). A description of the site and project, including the estimated sequence of events and impacted area, is provided in Section 1.0. The BMPs that will be implemented at the construction site are described in Section 2.0. The required inspections for the suggested BMPs are identified in Section 3.0. The procedures to ensure the timely maintenance of measures outlined in Section 2.0 are described in Section 4.0. The sampling procedures to confirm that installed control measures are functioning as designed are presented in Section 5.0. Finally, reporting requirements are specified in Section 6.0.
The proposed project disturbs [xx acres]. Therefore, this plan and all attachments are not to be submitted to GA EPD. {Note: The previous sentence does not apply if the proposed project disturbs greater than 50 acres. Furthermore, if the disturbed area is anticipated to be greater than 50 acres at any one time, a request must be sent to the EPD District Office, and additional BMPs must be included if the request is approved (see Section 2.6).} The Notice of Intent (NOI) will be submitted on [fill in the date of NOI submittal] and is included in Section I. of this document. CONTROLS
The following section includes a description of appropriate controls and measures that will be implemented at the construction site including: (1) initial sediment storage requirements and perimeter control phase BMPs, (2) intermediate grading and drainage phase BMPs, and (3) final stabilization phase BMPs. {Note: Generally, each phase must be addressed separately. However, if there will be no mass grading and the initial BMPs, intermediate grading and drainage BMPs, and final stabilization BMPs are the same, they are allowed to be combined into a single phase ESPC Plan. Add sentence: These BMPs are the same, and therefore, are presented in a single phase.} This section identifies the appropriate staging and access requirements for construction. Additionally, the appropriate control measures and the timing during the construction process are described for each major activity contained in Section 1.2. Installation, implementation, and maintenance of all controls described in this section are the responsibility of the construction contractor. Any deviation from this ESPC Plan must be documented in the checklists identified in Section 3.0.
Erosion and sedimentation control is vital to protect surface water on the Base from being impacted with sediment following precipitation events on the construction site. This ESPC Plan includes measures to perform the following:
· minimize the amount of disturbed soil;
· prevent runoff that originates in off-site areas from flowing across disturbed areas;
· slow down the runoff flowing across the site;
· remove sediment from on-site runoff before it leaves the site; and
· meet or exceed local or State requirements for sediment and erosion control plans.
Placement and maintenance of erosion control devices must be a vigilant pursuit by the construction contractor during all stages of construction, including the final stabilization phase until a permanent vegetative cover or other means of soil stabilization has been established.
2.1 EROSION AND SEDIMENT CONTROLS
The erosion and sediment control BMPs to be implemented by the construction contractor at the subject site are outlined in Figures 2-1, 2-2, and 2-3 {One figure for each stage of erosion and sedimentation control. These figures are to be provided by the construction contractor. Additional figures may be necessary. Note that a written rationale must be given when sedimentation basins are not used in the ESPC Plan}. The following implementation schedule should be followed based on the control measures shown in Figures 2-1, 2-2, and 2-3:

2.2 STABILIZATION PRACTICES
Any site where soils are exposed to water, wind, or ice can have soil erosion and sedimentation problems. Human activities can accelerate erosion by removing vegetation, compacting or disturbing the soil, changing natural drainage patterns, and by covering the ground with impermeable surfaces (pavement, concrete, buildings, etc.). Therefore, planning for temporary and permanent stabilization practices at a site needs to be implemented. Temporary stabilization measures can be taken, such as temporary seeding, mulching, chemical stabilization, and dust control. Temporary stabilizations have short-term advantages. For this reason, permanent stabilization measures, such as permanent seeding and planting, buffer zones, preservation of natural vegetation, and stream bank stabilization, need to be implemented to limit erosion and fluctuating drainage patterns. The site-specific stabilization practices for each stabilization phase of the project (as shown in Figures 2-1, 2-2, and 2-3) will be implemented by the construction contractor for this project.
2.3 STRUCTURAL PRACTICES
Structural practices used in sediment and erosion control divert storm water flows away from exposed areas, convey runoff, prevent sediments from moving off-site, and can also reduce the erosive forces of runoff waters. The controls can either be used as permanent or temporary measures. Practices used include, but are not limited to, earthen dike, silt fence, sediment trap, surface roughening, and temporary or permanent sediment basin. The site-specific structural practices utilized in each phase of stabilization during the project (as presented in Figures 2-1, 2-2, and 2-3) will be used for this project.
{Careful consideration should be given to the accuracy of the statements in this section. In the event that the site is greater than ten acres in area or in any other event that a sedimentation basin(s) is considered appropriate and/or necessary, sufficient narrative should be included here to explain the reason for including or omitting the sedimentation basin(s).}
2.4 STORM WATER MANAGEMENT
The measures shown in Figures 2-1, 2-2, and 2-3 will be installed prior to and concurrent with land disturbing activities. They are not necessarily intended to control pollutants in storm water discharges that will occur after construction activities have been completed and the site has undergone final stabilization.
2.5 OTHER CONTROLS
Practices other than erosion and sedimentation control will also be implemented at the construction site to prevent contaminants, such as petroleum, oils, and lubricants (POL) and antifreeze, from being discharged to the Base surface waters. Use of construction vehicles, hazardous chemicals, and non-storm water discharges potentially produce waste streams that contain contaminants that must not be discharged. BMPs will be developed, implemented, and maintained throughout the duration of the construction activities to prevent any unauthorized discharges to surface water.
Below is a list of general BMPs that will be followed during each stabilization phase of this construction project (Figures 2-1, 2-2, and 2-3 show specific BMPs):
· Worksite Housekeeping: Maintain good housekeeping practices at the project jobsite and equipment/material storage locations.
· Waste Disposal and Pickup: Locate waste disposal areas away from streets, gutters, watercourses, and storm drains. Locate waste disposal areas near construction site entrance to best extent practicable. Provide secondary containment for liquid waste collection areas. Regularly pick up and dispose of waste and recyclables.
· Equipment Maintenance: Ensure equipment is working properly; however, do not use the Base for routine maintenance of project equipment. Only emergency maintenance is allowed on project equipment and vehicles.
· Material Storage: Store containers, drums, and bags away from direct traffic routes, in accordance with manufacturer’s recommendations, and in a manner to protect against contamination of storm water.
· POL Spills and Leaks: Minor spills and leaks from construction equipment are a source of potential discharge. Do not use water to wash down POL spills from pavements. POL spills must be removed with appropriate spill clean-up equipment and materials.
· Spills Reporting: All leaks and spills regardless of the quantity must be reported by the contractor to the Robins AFB Point of Contact (POC). For spills over five gallons, the contractor will immediately call 911. The fire department will in-turn notify 78 CEG/CEV’s spill representative.
· Spill Kits: Have a full-service spill kit on-site for minor leaks and drips. Spills kits should include absorbent pads, spill booms, personnel protection equipment, and disposal bags.
· Drip Pan Use During Fueling: Use drip pans and absorbent pads when fueling construction equipment and providing emergency maintenance on equipment. Absorbents are to be handled in accordance with Resource Conservation and Recovery Act (RCRA) regulations.
· Drip Pan Use During Vehicle Storage: Use drip pans under heavy equipment left idle for two or more calendar days.
· Visual Inspections: Visually inspect construction equipment daily for leaks and spills.
· HAZMAT Storage: Store hazardous materials (including POL) on-site in covered areas with secondary containment (for example, flammable locker). Containers/tanks for fuel (mogas/diesel) should have secondary containment that meets regulatory requirements.
· Protecting Storm Drains: Do not dispose of waste in a storm drain (for example paint, oil, concrete, etc.).
· Vehicle Washing Prohibition: Wash vehicles off-Base. The exception to this is rinsing mud off tires at the construction exit. When doing this, make sure that nothing discharges to the storm drain and that all rinsate soaks into the ground under the construction exit.
· Vehicle Operation: Do not operate leaking equipment. Provide emergency repair to prevent further leaks.
· Concrete Washdown: Create a designated concrete washdown area at least 50 feet from storm drains, open ditches, or waterbodies. Do not allow runoff from this area, by constructing a temporary pit or bermed area large enough for liquid and solid waste, with plastic lining material. Only concrete from mixer truck chutes, tools, and hoppers is permitted – drum washdown at the site is prohibited. Once the concrete waste is allowed to harden, concrete should be broken up, removed, and disposed of in accordance with applicable state and federal procedures.
The specific BMPs listed in Figures 2-1, 2-2, and 2-3 will be followed to control the release of any solid waste, sanitary waste, and/or petroleum to the waters of the State. Specifically, the following “other measures” will be utilized at the site:
· Off-site vehicle tracking of dirt, soils, and sediments and the generation of dust shall be minimized or eliminated to the maximum extent practical. As shown in Figures 2-1, 2-2, and 2-3, the stone construction entrance and exit are the BMPs for minimizing off-site tracking of soils. Under conditions where soils have high moisture content, it may be necessary to build a wash area to remove solids from vehicles leaving the project site.
· All applicable State and local waste disposal, sanitary sewer or septic system regulations will be met. All portable toilets will be equipped with internal dual containment or will be placed in a bermed area.
· The use of any petroleum products on-site will comply with all State and local regulations for storage and handling. Portable or permanent petroleum dispensing units will have dual containment or be placed in an impervious bermed area that is able to hold 130% of the total volume of the largest tank. Any petroleum product storage will be inspected per the requirements specified in Section 3.0. Any spills will be cleaned immediately with absorbent material and disposed of in accordance with applicable regulations. See Figures 2-1, 2-2, and 2-3 for additional requirements or the designated location for petroleum dispensing units.
2.6 ADDITIONAL CONTROLS
Additional controls are required for sites that:
(i) discharge into Impaired Stream Segments or within one mile upstream of and within the same watershed of an Impaired Stream Segment, with ALL of the following characteristics:
a. Not Supporting its designated use;
b. Listed for criteria violated of “Bio F” (Impaired Fish Community) or “Bio M” (Impaired Macroinvertebrate Community);
c. Within Category 4a, 4b, or 5; and
d. Potential Cause of “NP” (nonpoint source) or “UR” (urban runoff); or

This site falls into the following category:
[x] discharge into Impaired Stream Segment as described above.
[x] approved to disturb 50 acres or more at any one time.
[x] does not apply. {This section does not apply to this site – delete remaining portion of this section} To ensure that the discharge does not cause or contribute to a violation of State water quality standards, the four BMPs marked below will be followed during each phase of this construction project (Figures 2-1, 2-2, and 2-3 show specific BMPs). {If the site falls into one of the applicable categories – choose four of the following twenty BMPs to incorporate into the Plan. Robins AFB strongly recommends the bolded items below as the chosen BMPs.}
a. [x] During construction activities, double the width of the 25 foot undisturbed vegetated buffer along all State waters requiring a buffer and the 50 foot undisturbed vegetated buffer along all State waters classified as “trout streams” requiring a buffer. During construction activities, EPD will not grant variances to any such buffers that are increased in width pursuant to this section.
b. [x] Increase all temporary sediment basins and retrofitted storm water management basins to provide sediment storage of at least 3600 cubic feet (134 cubic yards) per acre drained.
c. [x] Use baffles in all temporary sediment basins and retrofitted storm water management basins to at least double the conventional flow path length to the outlet structure.
d. [x] Place a large sign (minimum 4 feet x 8 feet) on the site visible from the roadway identifying the construction site, the permittee(s), and the contact person(s) and telephone number(s).
e. [x] Use anionic polyacrylamide (PAM) and/or mulch to stabilize areas left disturbed for more than seven (7) calendar days in accordance with Part III.D.1. of this permit.

g. [x] Comply with the applicable end-of-pipe turbidity effluent limit, without the “BMP defense” as provided for in O.C.G.A. 1276(a)(1).
h. [x] Limit the total planned site disturbance to less than 50% impervious surfaces (excluding any State mandated buffer areas from such calculations).
i. [x] Limit the amount of disturbed area at any one time to no greater than 25 acres or 50% of the total planned site, whichever is less.
j. [x] Use “Dirt II” techniques to model and manage storm water runoff (e.g., seep berms, sand filters, anionic PAM), available on the EPD website, www.georgiaepd.org.
k. [x] Add appropriate organic soil amendments (e.g., compost) and conduct pre- and post-construction soil sampling to a depth of 6 (six) inches to document improved levels of soil carbon after final stabilization of the construction site.
l. [x] Use mulch filter berms, in addition to a silt fence, on the site perimeter wherever storm water may be discharged.
m. [x] Apply the appropriate Georgia Department of Transportation approved erosion control matting or blankets or bonded fiber matrix to all slopes steeper than 3:1.
n. [x] Use appropriate erosion control matting or blankets instead of concrete in construction storm water ditches and storm drainages designed for a 25 year, 24 hour rainfall event.
o. [x] Use anionic PAM under a passive dosing method (e.g., flocculant blocks) within construction storm water ditches and storm drainages that feed into temporary sediment basins and retrofitted management basins.
p. [x] Install sod for a minimum 20 foot width, in lieu of seeding, along the site perimeter wherever storm water may be discharged.
q. [x] Use a surface draining skimmer designed to drain temporary sediment…
