About this file
This document is an Itemized Service Budget (ISB) template for the Community Services and Facilities Program (CSFP), designed for service providers contracting with what appears to be a state agency (ADES referenced) for the contract period of July 1, 2025 through September 30, 2025. The budget template provides a standardized format for contractors to detail all anticipated costs associated with service delivery under the CSFP agreement.
The ISB requires contractors to categorize and itemize all expenses across ten budget categories: Personnel, Employee Related Expenses, Professional & Outside Services, Travel, Space (Occupancy), Equipment, Materials & Supplies, Operating Services, Direct Costs, and Indirect Costs. Contractors must identify both total service costs and amounts to be charged to the contracting agency, clearly distinguish between administrative functions and direct service delivery expenses, and document all revenue sources funding the service provision. All costs must be necessary and reasonable, rounded to the nearest whole dollar, with specific guidance provided for allowable expenses within each category. The template includes instructions for contractors to identify revenue sources from multiple funding streams, including ADES, and to allocate employee time (expressed as Full-Time Equivalent percentages) appropriately to the contracted service.

Instructions
| ISB Instructions |
| An Itemized Service Budget (ISB) for the first contract period anticipated to be July 1, 2025 to June 30, 2026, as follows: |
* Utilize the ISB form to identify Total Service Costs and ADES Costs allocated to the Service Delivery Area. Total Service Cost column is to be utilized to account for all expenses involved in carrying out the service. CCSD Cost column is to be utilized to account for only the expenses that an Offeror will be charging to CCSD to carry out the service.
* Ensure that costs are necessary and reasonable for the delivery of the service(s);
* Place a zero (0) in the column(s) for any budget category where no costs are anticipated;
* Round all costs to the nearest whole dollar;
* Breakout the costs under each budget category;
* Identify all revenue sources (Offeror is to list out all the revenue sources include ADES and the amounts they are receiving from each source to provide this service); and
* Utilize the information in the following table to determine appropriate costs for each budget category.
| BUDGET CATEGORY | ALLOWABLE TYPES OF COSTS |
| 1. Personnel | Applicant’s employees’ salaries that relate directly to the delivery of projects. |
| · FTE Level: Is the percentage of an employee's time that will be allocated to the contracted service. For example if you have an eligibility specialist that works with different programs, you may estimate as an example that the employee spends 30% of their time providing the contracted service, therefore the FTE level would be .3 on the budget. | |
| · *A = % Administrative Functions: the percentage of expenses that are budgeted towards indirectly supporting the delivery of a service. | |
| · D = % Direct Service Functions Expenses that are budgeted towards the actual delivery of a service. | |
| 2. Employee Related Expenses (ERE) | Applicant’s fringe benefits to employees, including but not limited to: Social Security (FICA); unemployment insurance; worker’s compensation; health and life insurance; and retirement (Apply only to that portion of employees’ salaries directly related to the agreement.) |
| 3. Professional & Outside Services | Subcontractor services for direct service provision and/or support services (e.g., interpreter services) that relate directly to the proposed project plans, but are not provided by agency staff. Services may be provided by individuals or organizations. |
| 4. Travel | Mileage reimbursement not to exceed the federal rate as may be amended. |
| 5. Space | Facilities costs such as: rent; depreciation or use allowance; building services and utilities such as: fuel; electricity; water; trash removal; maintenance (subcontracted services only); sewer; insurance for buildings and contents. |
| 6. Equipment | Purchase and/or rental/lease of equipment costing $1,000 or more (each); depreciation; equipment maintenance and repair costs. |
| 7. Materials & Supplies | Consumable supplies such as: general office, kitchen, cleaning, hygiene, and program supplies; postage; reproduction and subcontracted printing services. |
| 8. Operating Services | Telephone; (ADES pre-approved) training for employees; library purchases; general liability insurance. |
| 10. Indirect Costs | Costs incurred for the benefit of multiple programs, functions, or other cost objectives and therefore cannot be identified readily and specifically with a particular program or other cost objective. Indirect costs typically support administrative overhead functions such as fringe benefits, accounting, payroll, purchasing, facilities management, utilities, etc. |
