About this file
This document is an Itemized Service Budget (ISB) spreadsheet template related to a state contract, specifically for the State of Arizona. The template provides a structured framework for budgeting across four primary cost categories: Personnel, Travel, Operational Costs, and Indirect Costs, with detailed guidance on allowable expenses for each category.
The budget template is designed to support a Positive Behavior Supports and Expulsion Prevention Program service, with specific provisions for documenting and allocating costs. Contractors must adhere to Arizona Department of Administration (ADOA) accounting guidelines, including state travel rates and approved indirect cost allocation methodologies. The document requires contractors to break down personnel expenses by specific service delivery requirements such as resource and referral, consumer education, complaint handling, child care provider support, and database management. While no specific contract amounts are listed, the template provides a comprehensive framework for financial planning and reporting, with links to official state accounting references for precise expense tracking and compliance.
