About this file

This is a Respondent Reference Form (Exhibit F) for solicitation HHS0017870 issued by the State of Texas Health and Human Services (HHS) Procurement and Contracting Services for the STS-Moodle Workplace COTS SaaS contract opportunity. The form requires all respondents to submit a minimum of three business references demonstrating their ability to perform the scope of work outlined in the invitation for bid. References must be for similar services provided within the last two years and should include contract dates, customer points of contact with complete contact information including names, titles, telephone numbers, and email addresses. Respondents must also clearly identify whether services were performed as a prime contractor or subcontractor, with subcontractors required to provide detailed descriptions of the scope of subcontracted activities.

The form provides space for up to four business references with structured fields for business name, point-of-contact information, phone, email, and detailed descriptions of services provided. This reference requirement serves as a qualification mechanism to ensure respondents possess relevant experience and can substantiate their capability to successfully execute the contract requirements. The form was revised as of December 6, 2022, and represents standard procurement practice for state contract solicitations in Texas.
