About this file
This is a Vendor Supplemental and Special Conditions form (Form 5622V, June 2022) issued by the Texas Department of Family and Protective Services (DFPS) that outlines contractual requirements and remedies applicable to vendors providing services under DFPS contracts. The document establishes the framework for contractor compliance and performance standards, including provisions for corrective action plans, suspension of contract performance, and removal of contractor personnel or subcontractor staff from service delivery roles or client contact positions.
The document indicates there are no supplemental conditions modifying the DFPS Vendor Uniform Terms and Conditions. The special conditions authorize DFPS to implement corrective action plans when noncompliance, poor performance, or deficiencies are identified, requiring contractors to respond in writing within specified timeframes and obtain DFPS approval before implementing remedial measures. DFPS reserves the right to suspend or terminate all or any portion of the contract and to require removal of any contractor employee, volunteer, agent, or subcontractor personnel from service provision or direct client contact. No pricing, funding amounts, contract term, or renewal options are specified in this supplemental document, as it functions as a conditions addendum to existing DFPS contractual frameworks.
