About this file

This document is an Itemized Service Budget (ISB) form template for TEFAP (The Emergency Food Assistance Program) service delivery under a state contract. The form is designed for use by an unspecified state agency and service providers to budget and account for expenses during the contract period of July 1, 2025 through September 30, 2025, with instructions indicating the initial contract period runs from July 1, 2025 through June 30, 2026.

The ISB template establishes budget categories for TEFAP service delivery, including personnel (broken down by position type such as TEFAP Admin, TEFAP CCC, TEFAP Admin Farm to Food, and TEFAP R&R), employee-related expenses, professional and outside services, travel, space/occupancy, equipment, materials and supplies, operating services, direct costs, and indirect costs. All line items currently show zero allocations, indicating this is a blank template awaiting completion. The instructions require offerors to identify total service costs, ADES costs allocated to service delivery areas, revenue sources, and to ensure costs are necessary and reasonable. Budget categories are defined with specific allowable cost types, including employee salaries allocated by FTE percentage (administrative versus direct service functions), fringe benefits, subcontractor services, mileage reimbursement, facility costs, equipment purchases or leases of $1,000 or more, consumable supplies, operating services such as telecommunications and training, and indirect administrative overhead costs.

Instructions

ISB Instructions
An Itemized Service Budget (ISB) for the first contract period anticipated to be July 1, 2025 to June 30, 2026, as follows:

* Utilize the ISB form to identify Total Service Costs and ADES Costs allocated to the Service Delivery Area. Total Service Cost column is to be utilized to account for all expenses involved in carrying out the service. CCSD Cost column is to be utilized to account for only the expenses that an Offeror will be charging to CCSD to carry out the service.

* Ensure that costs are necessary and reasonable for the delivery of the service(s);

* Place a zero (0) in the column(s) for any budget category where no costs are anticipated;

* Round all costs to the nearest whole dollar;

* Breakout the costs under each budget category;

* Identify all revenue sources (Offeror is to list out all the revenue sources include ADES and the amounts they are receiving from each source to provide this service); and

* Utilize the information in the following table to determine appropriate costs for each budget category.

6. EquipmentPurchase and/or rental/lease of equipment costing $1,000 or more (each); depreciation; equipment maintenance and repair costs.
7. Materials & SuppliesConsumable supplies such as: general office, kitchen, cleaning, hygiene, and program supplies; postage; reproduction and subcontracted printing services.
