Statement of Work Summary
This is a Statement of Work (SOW) for the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program administered by the Colorado Department of Early Childhood (CDEC). The program delivers evidence-based home visiting services using the Parents as Teachers (PAT) model to at-risk pregnant women and families with young children in Colorado. The contract period runs from October 1, 2026, through September 30, 2027. Vendors must maintain a minimum caseload of [insert number] families throughout the performance period while providing voluntary home visits, group connections, health and developmental screenings, and resource network services. Key deliverables include recruiting and retaining eligible families prioritized by at-risk criteria; providing a minimum of 12 home visits annually for families with zero or one stressor and 24 visits for families with two or more stressors; implementing services in accordance with PAT National Center fidelity standards; and submitting required data through the Visit Tracker system and to the state intermediary, Parent Possible. The contract program manager is Brittany Martens, MIECHV Program Manager, at brittany.martens@state.co.us.
The SOW requires vendors to hire and maintain qualified Early Childhood Supervisors and Certified Parent Educators, provide monthly individual reflective supervision and group reflective supervision/consultations, and ensure all staff complete mandatory training including PAT program training, mandatory reporter training, and annual CDEC orientation. Vendors must establish and maintain recruitment and retention plans, fiscal and programmatic policies, background check procedures, and HIPAA compliance protocols. Financial compliance requires monthly invoice submissions within 15 days of month-end through the OnBase platform, with supporting documentation and time and effort reports for all grant-funded staff. Vendors must engage in Continuous Quality Improvement initiatives, participate in Parent Possible monthly meetings and site visits, collect and report benchmark and demographic data biannually and monthly per federal requirements, and comply with three-year record retention standards. Additionally, vendors must provide appropriate referrals and follow-up services for families screening positive for intimate partner violence, depression and anxiety, developmental delays, and tobacco use, and submit a written plan by October 31, 2026, detailing how client support materials funds will be utilized to support program objectives.

GLOSSARY
● Visit Tracker (Visit Tracker Web) for Parents as Teachers- secure, cloud-based data management and documentation system.
● SMART- specific, measurable, actionable, relevant, and time bound goals. This goal setting strategy is utilized in the continuous quality improvement work throughout the performance period.
● OnBase- platform utilized by the Colorado Department of Early Childhood to digitize, manage, and automate records, documents, and workflows. This platform serves as a vendor invoice submission hub that MIECHV vendors must use.
INTRODUCTION/BACKGROUND
● The Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program provides pregnant women and families, particularly those considered at-risk, with the necessary resources and skills to raise physically, socially, and emotionally healthy and ready-to-learn children. From birth to kindergarten entry, the Health Resources and Services Administration (HRSA), in partnership with the Administration for Children and Families, funds states, territories, and tribal entities to develop and implement evidence-based, voluntary programs that best meet the needs of their communities.
● Colorado’s goal is to fund and support prevention services for families at risk of entering the child welfare system by implementing and expanding evidence-based home visiting programs with fidelity to the evidence-based model, using Continuous Quality Improvement plans to improve service delivery, and collecting program data that show progress towards the intended goals. MIECHV requires grantees to demonstrate measurable improvement in at least four benchmark domains.
o MIECHV benchmarks include:
o Improvement in maternal and newborn health, o Reduction in child injuries, abuse, and neglect, o Improved school readiness and achievement, o Reduction in crime or domestic violence, o Improved family economic self-sufficiency, and o Improved coordination and referral for other community resources and support.
SCOPE OF WORK
● The Maternal, Infant, and Early Childhood Home Visiting Program (MIECHV) gives pregnant women and families, particularly those considered at-risk, the necessary resources and skills to raise children who are physically, socially, and emotionally healthy and ready to learn.
Page 3 of 20 ExA SOW ECC Cost Reim FY27_1-26A
● The Parents as Teachers (PAT) model is the evidence-based, voluntary program used for this scope of work. It is designed to improve parenting practices by increasing parents' knowledge of early childhood development, promoting early detection of developmental delays and health issues, helping prevent child abuse and neglect, and increasing children’s school readiness and success.
● The PAT model has four dynamic components that together create a cohesive package of services for families with young children:
● Personal Visits: Certified parent educators share age-appropriate child development information with parents and caregivers and engage the family in activities that promote meaningful parent and child interaction.
● Group Connections: These meetings relay important information about child development and offer parents opportunities to interact with and support one another, fostering social connection and empowerment.
● Health & Developmental Screenings: Services are provided to detect any developmental delays or health issues early in a child’s life.
● Resource Network: Families are connected with resources to help them reach their goals and address their individual needs, including external resources and enhanced wrap-around services.
PERIOD OF PERFORMANCE
● October 01, 2026 – September 30, 2027
WHERE SERVICES WILL BE PERFORMED
● Provide the Parents as Teachers Program to a minimum caseload of [Insert Number] parents/families in Colorado.
