About this file

This is a Physical Location Certification Form (VR3442B) issued by the Texas Workforce Commission (TWC) Vocational Rehabilitation Services division for entities seeking to establish or maintain service delivery locations under bilateral contracts with TWC. The form is submitted either in response to Electronic State Business Daily postings or as an update to existing location information. Applicants must complete comprehensive documentation for each physical location where services will be delivered, including facility address information, mailing address if different, and entity identification details such as legal name, doing business as name, and Employer Identification Number or Social Security Number. The form requires submission of occupancy certificates or permits, building permits, and fire inspection reports from local fire departments or the Texas Department of Insurance State Fire Marshal's Inspection Services Division.

The form establishes mandatory safety and accessibility standards that all physical locations must meet to serve vocational rehabilitation customers. Entities must certify that locations include working smoke detectors with visible and audible signals, non-expired fire extinguishers in accessible locations, posted fire escape route diagrams, evacuation plans for customers requiring physical assistance, emergency medical services arrangements, accessible aisles and work zones, hazardous materials management plans, and equipment safety protocols. Accessibility requirements include handicapped parking, accessible exterior routes, interior accessibility features, and accessible bathrooms, with entities required to provide accommodations when minimum standards do not meet individual customer needs. The legally authorized representative must certify the accuracy of all information, compliance with TWC VR Standards for Provider Manual, and agreement to update the form and submit incident reports as required. Failure to maintain compliance or any safety incidents must be reported to the contractor's assigned Quality Assurance Specialist or Regional Program Support Specialist.
