About this file

This is an Electronic Funds Transfer Sign-Up Form issued by the Commonwealth of Massachusetts Office of the Comptroller that enables vendors to establish or modify banking arrangements for receiving state payments. The form serves as authorization for the State Treasurer, acting as fiscal agent for the Commonwealth, to initiate, change, or cancel automated clearing house (ACH) credit entries to vendor accounts. Vendors must specify the request type as initial, changing existing account, or closing account, and must provide complete banking information including the vendor bank name, transit number (ABA), and account number. The form requires vendors to certify that the designated account is under their direct control and to confirm compliance with International ACH Transaction rules regarding foreign bank account transfers. The authorization remains in full force until the Office of Comptroller receives written notification of termination, allowing the agency reasonable time to process such requests.

The form requires vendors to complete comprehensive vendor identification information, including tax identification number, business name, authorized contact details, and physical address. Vendors must provide an authorized signature from an appropriate representative along with printed name, title, and date of authorization. A voided check must be attached to verify banking information accuracy. The form is submitted to the specific Commonwealth department with which the vendor conducts business, and any changes to account information require submission of an updated form. This authorization structure ensures secure, standardized electronic payment processing while maintaining vendor control over their designated banking arrangements.
