About this file

This is an Equal Employment Opportunity (EEO) Staffing Plan form submitted to Roswell Park Comprehensive Cancer Center in New York as part of a Request for Proposal (RFP) for Managed Service Provider (MSP) temporary staffing services. The EEO Staffing Plan requires respondents and their subcontractors to submit detailed workforce composition data broken down by job classification, gender, race/ethnicity, and veteran status. Respondents must complete the form for either the workforce anticipated to be utilized on the resulting agreement or their total workforce if the project-specific workforce cannot be separated. The form includes ten EEO job categories: Executive/Senior Level Officials & Managers, First/Mid-Level Officials & Managers, Professionals, Technicians, Sales Workers, Administrative Support Workers, Craft Workers, Operatives, Laborers and Helpers, and Service Workers. Failure to submit complete and accurate information may result in a finding of non-compliance and potential termination of any resulting agreement.

The document provides specific identification categories and definitions for workforce classification, including White, Black, Hispanic, Asian, American Indian or Alaskan Native, and Veteran status, with clear guidance that no person should be counted in more than one race/ethnic group. The form must be signed and dated by an authorized preparer and include contact information. This EEO Staffing Plan is a compliance document required as part of the broader RFP for temporary staffing services, which seeks proposals to support approximately 49 FTEs annually across locum tenens/allied health, non-clinical/professional, and executive staffing categories. Roswell Park reserves the right to award a single MSP, partial services, multiple awards, or make no award under the RFP.
