About this file

This is an Equal Employment Opportunity (EEO) Staffing Plan form required by Roswell Park Comprehensive Cancer Center for respondents bidding on a temporary staffing agencies contract in New York. The form serves as a template for vendors and their subcontractors to document their workforce composition across multiple EEO job categories, including executive/senior level officials and managers, first/mid-level officials and managers, professionals, technicians, sales workers, administrative support workers, craft workers, operatives, laborers and helpers, and service workers. Respondents must complete the form by entering the total number of employees for each classification, broken down by gender (male and female) and race/ethnic identification (White, Black, Hispanic, Asian, American Indian or Alaskan Native), as well as veteran status. The instructions specify that vendors may report either the workforce anticipated to be utilized on the Roswell Park contract or their total workforce, depending on whether the staffing for this specific agreement can be separated from their broader employee base. Each individual employee should be counted in only one race/ethnic category based on the person's appearance, identification, or community recognition.

The form emphasizes that failure to submit complete and accurate information may result in a finding of non-compliance and potential termination of any resulting agreement. The EEO Staffing Plan must be submitted as part of each respondent's proposal whenever required by the RFP. The identification categories provided include detailed definitions for each racial and ethnic group, as well as specific criteria for veteran classification, including honorable discharge and U.S. citizenship or lawful permanent residency status. The form requires the submitting company to identify itself as either a respondent or subcontractor, provide its federal tax identification number and address, and include contact information for the preparer, including signature, date, phone, fax, and email.
