About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with Public Contract Code Section 10162, for the Parking Citations and Permit Management Solution RFP. The form requires prospective bidders to declare under penalty of perjury whether the bidder, any officer of the bidder, or any employee with a proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate either "Yes" or "No" in response to this question.

If a bidder answers affirmatively, they are required to provide a detailed explanation of the circumstances and attach supporting documentation to the disclosure form.
