About this file

This is a Disqualification Disclosure form required by the State of California pursuant to Public Contract Code Section 10162, applicable to the City's On-Call Survey Consultant Services solicitation. The form requires prospective bidders to certify under penalty of perjury whether the bidder, any officer of the bidder, or any employee with proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate yes or no in response to this questionnaire, with instructions to explain circumstances and attach supporting documentation if the answer is affirmative.

This disclosure requirement is a standard compliance document designed to ensure bidder integrity and regulatory compliance on California public works and professional services projects. The form does not contain pricing information, set-asides, contract terms, or other substantive project details; rather, it serves as a preliminary qualification screening tool to identify any prior disqualifying conduct before bid evaluation proceeds.
