About this file

This document is a Debarment, Suspension, Ineligibility and Voluntary Exclusion Certification form issued by the City of Long Beach for consultants and vendors seeking to conduct business with the city on projects that may receive federal grant funds. The certification requirement is mandated by Executive Order No. 12549 and Title 2 CFR Part 180 Subpart C to ensure compliance with federal regulations. All prospective bidders, consultants, and contractors must complete and submit the form with their bid, proposal, or agreement, certifying under penalty of perjury that neither their business entity nor any principals have been debarred, suspended, declared ineligible, or voluntarily excluded from federal funding. The form requires authorized representatives to sign and date the certification and may be requested on a routine or updated basis throughout the contract period.

The certification requires prospective recipients to verify five specific conditions: current non-suspension or debarment status; no suspension, debarment, or ineligibility actions within three years prior to submission; no pending debarment proceedings; no indictments, convictions, or civil fraud judgments within three years; and no termination of public transactions for cause or default within three years. Prospective bidders are instructed to search the System for Award Management at www.sam.gov to verify their company and associated personnel compliance before submission. If any exceptions exist, bidders must submit written explanations with their proposal; exceptions will be reviewed in determining bidder responsibility. Any reorganization, management changes, or status changes must be reported within 21 days. The City of Long Beach reserves the right to pursue remedies including suspension and debarment for any prospective recipient who knowingly submits erroneous certification.
